THE STEERING INTERMEDIATE SHAFT COULD HAVE BEEN IMPROPERLY ASSEMBLED CAUSING THE SHAFT LOWER COUPLING TO LOOSEN AND ROTATE ON THE STEERING GEAR INPUT SHAFT.
