Taylor Made Ambulances is recalling certain 2017-2018 Ford E-350, F-450, E-450, F-550, and GMC G3500-based ambulances equipped with Whelen brake lights. Depending on the specific wiring of the vehicle, these brake lights may illuminate when the brake pedal has not been pressed.

The recall was influenced by the manufacturer with about 17 units affected.

Description:: Taylor Made Ambulances is recalling certain 2017-2018 Ford E-350, F-450, E-450, F-550, and GMC G3500-based ambulances equipped with Whelen brake lights. Depending on the specific wiring of the vehicle, these brake lights may illuminate when the brake pedal has not been pressed.
Consequeces:: If the brake lights incorrectly illuminate, it can confuse other drivers, possibly increasing the risk of a crash.
Corrective action:: Taylor Made will notify owners and Whelen will replace the brake lights, free of charge. The recall began June 5, 2019. Owners may contact Taylor Made Ambulances customer service at 1-800-468-1310.

TL* THE CONSUMER OWNS A 2018 FORD E-350. THE CONSUMER STATED THAT WHILE TRAVELING AT A LOW RATE OF SPEED, THE CONSUMER ATTEMPTED TO STOP THE VEHICLE BY DEPRESSING THE BRAKE PEDAL; HOWEVER THE VEHICLE FAILED TO OPERATE AS NEEDED. THE VEHICLE INSTEAD ACCELERATE FORWARD WHILE TRAVELING APPROXIMATELY 30 MPH. THE VEHICLE WENT INTO AN EMBANKMENT, DID NOT COLLIDE OR HIT ANYTHING, NO FRONT END DAMAGE, HOWEVER THE REAR OF THE VEHICLE WAS DAMAGED. THE AIR BAGS DID NOT DEPLOY DUE TO NO COLLISION, A POLICE REPORT WAS FILED(BR-19-13326). THE CONTACT WAS TAKEN TO A LOCAL HOSPITAL AND SUSTAINED A FRACTURED BACK. THE CAUSE OF THE VEHICLE FAILURE WAS UNKNOWN. THE VEHICLE WAS TOWED TO A LOCAL BODY SHOP AND THEN TO A DEALER(TRI-LAKES MOTORS 180 STATE HIGHWAY F, BRANSON, MO 65616 417-334-7543). 2018 FORD E-450 Complaints
I received a recall letter stating the awning (Solera Smart Arm manufactured by Lippert) on my newly purchased Forest River RV could begin to open up as the vehicle was in motion. Because of the extreme safety issue, I immediately took it into the service department for fear it would rip off while driving and cause a collision and possible serious injury to others. After attempting to fix the recall issue my awning has never worked properly. After 7 attempts, several replacement parts and three years later, my awning still does not work. After each time the awning was serviced, I would bring it back to storage. A month or two later I would go on a trip. Once I arrived at my location, I would attempt to open my awning and it was inoperative. It would take one to two months to get a service appointment and then a month or more for the service technician to attempt to fix the issue. This occurred five times after the initial attempt to fix the recall. Each time moisture got into the exterior controls and no amount of sealant would rectify the problem. I am unsure if the safety issue has even been rectified and now have to travel with zip ties around my awning arms. Once again, my awning is inoperative.;

incidents Summary: The vehicle experienced a significant engine compartment fire while in operation. The fire originated in the engine valley near the secondary fuel filter, consistent with the failure mode described in NHTSA Investigation PE24014. 1. Component Failure & Part Numbers: The fire necessitated an $18,000 repair (RO #443426). Based on the repair invoice, the following safety-critical components were heat-destroyed and replaced: Secondary Fuel Filter (PC3Z9N184C) Main Engine Wiring Harness (HC3Z12B637A) Inlet Manifold (GC4Z9424A) High-Pressure Fuel Pipes (HC3Z9J280J and BC3Z9E964A) Cowl Grilles (HC3Z25022A68AB/69AC) 2. Safety Risk & Pre-Existing Symptoms: Prior to the fire, the engine exhibited a "surging" idle and erratic RPM behavior, which was reported to the dealer. After the replacement of the aforementioned fuel and electrical components, the surging issue was immediately resolved, confirming the original symptom was a precursor to the fuel system failure. While no injuries occurred, the fire presented an immediate risk of occupant injury and total vehicle loss. 3. Disposal of Evidence (Spoliation): Despite an active liability dispute and an ongoing manufacturer investigation, the dealership (AutoNation Ford Valencia) admitted in writing that they disposed of the failed secondary fuel filter and the main wiring harness. These primary pieces of evidence are currently unavailable for federal or forensic inspection due to the dealer's actions. 4. Confirmation: The fire damage was confirmed by the dealership, and I have paid the repair invoice UNDER PROTEST to retrieve the property. I am seeking a review of this incident under the scope of PE24014.;
2018 FORD F-550 Complaints
The contact owns a 2018 Ford F-550. The contact stated while the vehicle was parked, the contact stated that the frame was cracked between the bed and the cab. There were no warning lights illuminated. The vehicle was taken to a dealer where it was confirmed that the vehicle was previously repaired under a related unknown recall and declined to perform the recall repair on a second occasion. The dealer determined that the frame needed to be replaced. The vehicle was not repaired due to the cost. The manufacturer was not made aware of the failure. The failure mileage was approximately 73,000.;
2018 GMC G3500 Complaints
VEHICLE LEFT THE ROAD WHILE DRIVING DUE TO INADEQUATE HEADLIGHT FUNCTION, FORTUNATELY NO DAMAGE OR INJURY THIS TIME. COULD NOT SEE ROADWAY ADEQUATELY TO MAINTAIN CONTROL. LOW BEAM AND HI BEAM HEADLIGHTS PROVIDE VERY INADEQUATE LIGHTING FOR SAFE NIGHTTIME DRIVING.;
