Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

The recall was influenced by the manufacturer with about 1,024,407 units affected.

Description:: Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequeces:: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
Corrective action:: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

2024 LEXUS LX Complaints
Lexus LX 600 2024 While driving the cars the main monitor screen which displays important information, a splash screen pops up asking for a 6 DIGIT PIN # and obscure the information beneath it. This totally takes you out of the GPS app. Loss of the GPS app while driving in a full freeway in busy traffic is potentially life threatening. Not only that clearing this PIN request the driver finds the GPS App gone. The driver is requited to cancel out of the demand for a “6 digit PIN number only to find this screen obtrusive screen only pop up at infinitum. This puts the driver and other traffic at risk. I have informed the local Lexus dealer but they do not offer a solution for this hazard. No warning lamps Dealer aware;
2024 TOYOTA PRIUS PRIME Complaints
The contact owned a 2024 Toyota Prius Prime PHEV. The contact stated that while driving at 40 MPH, the vehicle was involved in a head-on collision with another vehicle that was traveling towards the contact's vehicle at approximately 50 MPH. The contact stated that during the impact, the steering wheel detached and hit the driver's body. The air bags failed to deploy. There was no warning light illuminated after the crash. The vehicle went into Crash mode and automatically connected with OnStar, which called emergency units. The contact stated that the entire front ends of both vehicles were destroyed. Both vehicles were towed to an impound lot, where they were totaled. The contact's vehicle was deemed a total loss by the insurance company. A police report was filed. There was no property damage. The contact was transported by ambulance to the ER, where he was treated for multiple injuries: fractured ribs, fractured right foot, seat belt injury in the lower abdomen that caused internal bleeding and damage to the stomach, mild concussion, several body bruises and lacerations. Additionally, the contact stated that he was receiving treatment for lower back pain due to a displaced disc. No further information was available. The failure mileage was approximately 35,981.;
2024 TOYOTA RAV4 PRIME Complaints
I am reporting a serious safety issue involving my vehicle where the braking system did not respond as expected during an emergency. While traveling southbound on I-575 in Georgia, I experienced a sudden tire blowout. As I moved to the shoulder and applied the brakes firmly, the vehicle did not slow down as expected. Despite pressing the brake pedal continuously and with significant force, the vehicle continued traveling for approximately 1–2 miles before finally coming to a stop near Exit 11. This was a highly dangerous situation, as the brakes appeared to be ineffective during a critical moment. Fortunately, there was no crash or injury. After the incident, the vehicle was towed to a Toyota service center (Cherokee Toyota) where the damaged tire was repaired/replaced. I was informed that no other mechanical issues were found. However, I remain concerned that the braking system may have a defect or intermittent issue that was not detected during a standard inspection. Given the severity of the incident, I request a thorough investigation into the braking system performance under such conditions. A failure or significant delay in braking response presents a major safety risk.;
2024 TOYOTA VENZA HYBRID Complaints
After coming to a stop at a STOP sign, the vehicle accelerated unintentionally, crossing the highway and hitting a parked car, causing injuries and totaling my vehicle. While my vehicle sustained a crumbled chassis, no airbags were deployed, causing injuries to the driver and passenger. Additionally, the emergency automatic brakes did not stop the car from ploughing into the parked car. The car was examined by the insurance carrier (AAA) and Toyota (the manufacturer), who sent out a technician to read the CDR data. The report was generated by BOSCH (a 3rd party hired by Toyota). The report generated by BOSCH claimed the accelerator was fully engaged at 100% and brakes were applied only 0.85s prior to the impact. This was the first such event; no prior warning lights/signals were reported on the dash. This was a new car purchased from the dealership with no prior accidents or incidents.;
