  • INTOXILOCK IGNITION INTERLOCK
    - Brooklyn, NY

An Intoxalock ignition interlock device was installed in my 2023 Mercedes-Benz E-Class on May 7, 2026. Following installation, the vehicle experienced repeated electrical/no-start problems and incidents in which it became immobilized and could not be moved, requiring towing. On one occasion the vehicle became disabled on a roadway/highway, leaving me in a dangerous position with traffic passing around me. When the vehicle is immobilized, I cannot simply move it to a safe location, and Intoxalock has not been able to restore vehicle operation remotely. I have had to wait for towing assistance, sometimes for extended periods. Independent diagnostic/service facilities have evaluated the vehicle. One attributed battery drain to the Intoxalock-related problem. Another attributed starter damage to it. Service documentation also reflects diagnosis of the Intoxalock system and reinstallation/reconnection of the device and wiring. Documentation is available, and the equipment/vehicle can be made available for inspection upon request. The vehicle now displays numerous dashboard warning lights and is not operating normally. Safety-related systems are malfunctioning, including ABS, parking assistance, camera functions, and automatic braking/collision-avoidance functions that previously operated normally. I am concerned that the Intoxalock device, its installation/wiring, or its interaction with this vehicle’s electrical/electronic systems may be contributing to these failures. I am requesting that NHTSA investigate whether this aftermarket ignition-interlock equipment presents a safety or compatibility risk, particularly when a vehicle can become immobilized in traffic and safety systems may be affected.

  • LIPPERT 7000LB BRAKE AXLE
    - Novelty, OH

I am requesting that NHTSA investigate a potentially serious and recurring safety defect involving Lippert trailer axles, wheel bearings, hubs, and inadequate factory lubrication. My original cargo trailer, equipped with Lippert axles, experienced repeated wheel-end, bearing, and axle-related problems. In March 2026, it suffered a catastrophic wheel-end/hub/bearing failure while traveling on the highway. The incident caused the tow vehicle and trailer to jackknife, damaged the trailer, tow vehicle, and cargo, and ultimately resulted in the trailer being declared a total loss.A replacement Lippert axle was subsequently installed. After only approximately 1,400 miles, the wheel end began smoking. Inspection revealed severely blackened/burnt grease, indicating excessive heat and another potential bearing/lubrication problem.Because of these repeated failures, when I purchased a brand-new 2026 36-foot enclosed cargo trailer in June 2026, also equipped with Lippert axles, I specifically requested that the bearings, hubs, spindles, and grease levels be inspected BEFORE I took possession or operated the trailer. That pre-delivery inspection revealed very little grease in the wheel-end assemblies and at least one bearing was already damaged. These conditions were discovered before I operated the new trailer, eliminating normal wear, mileage, or my maintenance as potential causes. My understanding is that Lippert supplies these axle assemblies to the trailer manufacturer already assembled. I therefore believe NHTSA should investigate Lippert's manufacturing, bearing installation, lubrication, assembly, and quality-control procedures. These conditions create a serious highway safety risk. Inadequate lubrication or defective bearings can cause overheating, bearing seizure, hub/spindle failure, wheel separation, fire, trailer instability, jackknifing, loss of control, serious injury, or death. This is now more than one Lippert-equipped trailer with serious bearing/lubr

  • BRAUN WHEELCHAIR LIFTS
    - Middleburgh, NY

The outer roll stop is very slow to deploy, takes minutes. You can step on it and make it go down but that is not proper. NY bus sales has looked at it numerous times and is still extremely slow. Other districts within our area have the same problem. DOT does not have a time specification that this must deploy, but that does not make it right. This is a Braun Millenium series. Braun has offered suggestions but nothing has worked.

  • SEALIGHT LED HEADLIGHT BULB
    - Orlando, FL

I purchased LED headlight bulbs on Amazon under order number: 114-2056235-0446623. These bulbs are excessively bright and cause severe glare to oncoming vehicles. I nearly collided with an on-coming car while driving, which is extremely dangerous. I wonder why such hazardous products are allowed to be sold on Amazon.
