  • ROADMASTER NIGHTHAWK TOW BAR
    - Mims, FL

    While responding to a safety recall concerning the RoadMaster Nighthawk, I discovered that the manufacturer installed a shorter bolt on one side of the hitch. The shorter bolt barely connects to the nut. I contacted the seller, Etrailer and was told that because of the original safety recall they cannot help me. They told me to contact the manufacturer. I contacted RoadMaster and they stated I should contact Etrailer the retailer to purchase the correct bolt. The danger is great if the shorter bolt lets go as the towed vehicle would lose stability even with the backup cable as one side would hold and the other side would be broken. A simple fix is for the manufacturer to supply the correct bolt.

  • SARIS SUPERCLAMP BIKE RACK
    - MOOSE LAKE, MN

    On several occasions this Saris Superclamp bike rack has dropped from an upright position to a tilted position when there is a bump/dip in the road. A driver following has later reported to me that the rack/bikes falling quickly startled him and led to him braking. The bikes have not fallen from the rack when it drops down but I am concerned about the possibility. I have contacted the manufacturer and they will send me parts to repair if I can provide the original sales slip which I can not find. The part is available for purchase for $200 which is nearly half the cost of the rack. I have communicated to the manufacturer that this defect is a safety hazard and they should rectify the issue at no cost. They have refused.

  • RAIN-X WINDOW CLEANER
    - Granville, NY

    The contact owns a 2023 Honda Accord. The contact purchased a one-gallon container of Rain-X windshield washer fluid and added approximately two quarts to the reservoir. The contact stated that immediately upon application, the fluid coated the windshield with an oily film, severely degrading visibility. While driving in rain, the windshield wipers smeared the fluid across the glass, obstructing the driver's view and creating a safety hazard. The manufacturer was notified of the failure; however, assistance was denied.

  • ADAPTIVE EQUIPMENT

    I am a disabled Army veteran reporting a severe safety defect and dangerous commercial configuration of adaptive mobility equipment installed by Mobility Works ([XXX]). The business sold me a 2018 Chrysler Pacifica wheelchair van with a heated transfer seat sight-unseen. The dealership delivered a completely non-conforming transfer seat that did not match the photo I bought it from and listed on the window sticker or marketing materials. The installed seat was missing critical structural spacer component, causing the driver's seat to wobble and was especially bad while turning. Furthermore, the vehicle frame creaks loudly under the weight of a VA-issued 400lb power wheelchair, indicating underlying structural collapse from a hidden September 8, 2018, insurance total-loss salvage event. On XXX, the non-conforming transfer seat failed completely. It jammed and refused to rotate, trapping me inside the vehicle. Because of my physical disability, I was entirely helpless and stranded, requiring an emergency response from 911 personnel to extract me from the vehicle. The dealer's regional service manager has inspected the vehicle and admitted that a spacer was missing that cause the seat to wobble and drivers needing 3" cushion to sit on to see over the dash.   • FORD SERVICE FUEL TANK
    - Houston, TX

    The gas tank is leaking gas on the gas tank inlet plastic yellow retainer ring appears to be broken. This gas leak has put safety family members at risk. It appears to be a manufacturer defect built defective.

  • ROUGH COUNTRY LIFTED STEERING KNUCKLE
    - salem, OR

    While operating my vehicle at normal legal speeds on a public road, the aftermarket Rough Country lifted steering knuckle suffered a sudden, catastrophic structural breakage at the tie-rod mounting point after hitting a standard pothole. The complete fracturing of the steering knuckle resulted in an immediate, unpreventable loss of steering control on the driver's side wheel. This sudden structural failure forced the front wheel assembly out of alignment, completely disabling the vehicle's steering mechanics and bending the lower drop crossmember brackets. This is a severe life-safety hazard. Had this casting failure occurred at highway speeds or in heavy traffic, it easily could have resulted in a rollover, a head-on collision, or a fatal accident. Heavy-duty cast suspension components should never experience catastrophic metal breakage from everyday public road imperfections. The manufacturer, Rough Country, is refusing to recognize this as a structural defect and is denying liability. This failure needs to be investigated immediately to see if there is an underlying casting, metallurgical, or design defect in Rough Country's lifted steering knuckles before it causes a fatal crash on public roads.

  • EVERSTART MAXX 12V BATTERY H5/47, 650 CCA
    - Anderson, TX

    A new EverStart Maxx lead-acid battery (Group Size H5/47, 12V, 650 CCA) was bought at Walmart on 07/14/2026 and installed the same day in a 2014 Chevrolet Sonic. It was installed correctly: correct polarity, both terminals tight and free of corrosion, hold-down secured. The car ran normally for about eight weeks, with no warning lights or electrical symptoms. On 09/10/2026 at about 12:00 p.m., the car caught fire while parked. It had not been driven or started since about 5:00 p.m. the day before (about 19 hours), so the engine and charging system were off. The fire appears to have started at the battery. It stayed in the engine compartment but melted the underhood fuse box and the engine wiring harness. The local volunteer fire department responded, and a police report was filed. No one was hurt, but a battery fire in a parked, unattended car could easily spread to a home, garage, or other vehicles. The car had an earlier front-end repair after hitting a deer. It had been driven for more than a year since that repair with no electrical problems. The vehicle and the battery remains have been left in place and are available for inspection. The retailer and manufacturer have not inspected them yet.

  • GE AIR CONDITIONER

    My air conditioning failed producing cool air in late 2020. I took it to Honda Penske in Ontario. The said at that time the Evaporator core is leaking but it not covered by warranty. I had pay out of pocket to install freon and that would like me for 5-6 months however the same problem would return. The following year I took it to El Monte Honda dealership where I purchased the car. They reviewed and stated the same the problem and they couldnt do anything about it. I once again had to pay out of pocket to install freon myself. Any local shop installs freon for $500. I have had to do that every summer of the last 4-5 years. My most recent visit was to Penske Honda which told me there is a recall for the AC Condensor, which was performed along with installing of freon. The same problem arises after 3 months where the evaporator core doesnt hold in the cool air, it leaks and eventually doesnt produce cool air at all. Hot days can be miserable especially with my kids in the Car.

  • HENSLEY ARROW HITCH

    Well, doing regular maintenance to my trailer hitch for my RV. I found several welds that were cracked, which made the hitch unavailable to use and not safe to be on the road. I contacted Hensley hitch and they said the hitch is no longer safe to use.

  • VEVOR ELECTRIC TRAILER BRAKE ASSEMBLY
    - Gilroy, CA

    Complete brake failure, loss of trailer brakes. Near accident. TWICE! In 2024, I installed all new brakes on my enclosed car trailer from VEVOR. I experienced this exact brake failure less than 1000mi later (magnet arm snapped at spring hole) and reported this to VEVOR and requested contact with engineering so I could convey to them the stress marks in the other arms on my brakes that showed signs of potential failure. They responded by immediately replacing the brakes and ensuring me the situation was now fixed. A year later, on the same trip/route/event, TWO MORE failures on the brand new brakes they already replaced once before...in the exact same spot. So, now we have 3 failures in the exact location. I reported this again, and this time demanded a technical contact so I could ensure someone in the right position got the right information to prevent potential loss of life and/or property by this obvious design defect. VEVOR has repeatedly offered me a full refund, new brakes, etc but REFUSES to put me in contact with anyone that has the ability to learn/understand/resolve this design defect. I feel it is imperative to do so, have refused the refund for the product, and continue to insist they fix the defective product. I need help. They are not listening. I've written letters, and sent emails, and made phone calls. I can't get anyone over there to listen. I'm a [XXX] old Engineer that wants to FIX this problem.
