On July 25, 2026, at approximately 7:10 PM, I was a passenger in a fully autonomous Waymo vehicle traveling southbound near 24th Street and Southern Avenue in Phoenix, Arizona. The vehicle entered the intersection to make a left turn and, while still in the active intersection, unexpectedly reversed. I considered this a serious safety concern because of the potential risk to surrounding traffic. Fortunately, there was no collision or injury. I reported the incident directly to Waymo and subsequently followed up, asking them to review the vehicle’s video and driving data. Approximately six weeks have passed, and Waymo has not responded to me regarding the incident. I am reporting this to NHTSA so the automated-driving behavior can be documented and reviewed as a potential vehicle safety issue.

I am concerned that a potentially serious engine/timing-system defect may exist in my 2019 Jaguar XE R-Sport 2.0L Ingenium gasoline engine (64,820 miles). Jaguar Service Action H299 addresses VCT solenoids on certain 2018–2020 XE models, and my VIN falls within the identified VIN range. In August 2026, my vehicle developed a VCT/camshaft-related fault. An independent Jaguar/Land Rover repair facility replaced the camshaft adjuster solenoids, but the problem persisted as indicated by the engine service light. The vehicle then began running noticeably rougher and developed a rattling noise every time the engine is started. The repair facility recommended approximately upper timing-chain-related repairs, which I declined. Jaguar North America referred me to an authorized Jaguar retailer, which has advised me not to drive the vehicle because of the rough running and rattling. The earliest diagnostic appointment is September 21, 2026, and I have been advised repairs could potentially cost approximately $10,000 or more. I request that NHTSA document and investigate whether these VCT/timing-system problems represent a broader safety-related defect in affected Jaguar XE vehicles.

The contact witnessed a near-crash involving a 2024 Jaguar I-PACE Waymo Autonomous Vehicle. The contact was positioned approximately 5 feet away from the vehicle at an intersection. The contact observed a father and daughter walking across the crosswalk when the daughter fell to the ground. The Waymo vehicle started approaching the fallen child, and the father threw a stroller in front of the vehicle. Simultaneously, the contact positioned herself against the side of the vehicle, prompting the autonomous system to come to a stop and preventing the child from being struck. Once the pedestrians had cleared the crosswalk, the Waymo vehicle abruptly accelerated and sped away from the scene. The contact was document the license plate information. The contact stated the information was reported to the local news authority; however, the news authority was providing incorrect information.

Failure of the Electric Power Assisted Steering (EPAS) rack on 2019 Jaguar Pace causing an immediate safety hazard when driven in the rain. Factory EPAS motor housings suffer from inherent design vulnerability. Power steering to the vehicle was locked up with dashboard warning. Jaguar Monroeville has confirmed need for new steering rack, tie rods, etc costing $8900 including labor. There were no warnings prior to failure as the issue arose when driving in the rain. There is an active community of over 1800 JLR drivers called JLR Power Steering Alliance Group on Facebook fighting this issue with proposed class action lawsuits. Jaguar is inconsistently applying some goodwill assistance. Attempts for assistance were denied. Please call Jaguar CRC [XXX] and Jaguar Monroeville service manager [XXX]. Engine fuel rail leak and Engine feed line leak. Starting the car has a raw fuel smell inside the cabin of the car. Took the car to jaguar and they said nothing was wrong. Took the car to a jaguar specialty shop and they revealed the fuel rail needed a "recall" similar to the 2017+ Jaguar F-Pace for the fuel rail, and 2 engine feed lines. Safety is put at risk by introducing fuel fumes into the cabin of the car, and leaking fuel with the possibility of detonation. The problem has been inspected by a Jaguar specialist, there aren't any warning lights or indicators outside of the fuel smell in the car. This is the same thing that happened in my 2017 Jaguar F-Pace. Same fuel smell, same issues in the fuel rail and the engine feed lines. This is over 3k in repairs and jaguar is saying there is no issues.

While driving at 65 mph on the highway, the coolant system failed catastrophically without warning. This sprayed boiling coolant onto the hot engine block, filling the cabin with smoke, blinding visibility, and causing the engine to shut down suddenly in active traffic.

Pirelli Cinturato P7 All Season tires, size 255/35R20. I am requesting an immediate safety review and technical investigation regarding repeated failures of the Pirelli Cinturato P7 All Season tires installed on my Jaguar XF. The vehicle is equipped with Pirelli Cinturato P7 All Season tires, size 255/35R20. Three of the four tires from this set have now suffered serious failures involving sudden rapid air loss/blowouts and apparent tread or belt separation. On 8/24/26 this happened spontanously on I-35 going 70 mph causing an almost deadly crash. These have not been ordinary slow leaks. The failures have occurred suddenly and have created a serious safety concern. I am now extremely concerned about operating the vehicle on the fourth remaining matching tire. I have photographs of the remaining tire and can provide photographs via the service rep Joe@Jaguar and documentation regarding the previous failures, dealer repair/replacement records, mileage, tire pressures, and DOT/TIN information. Because three out of four tires have experienced serious failures, I am requesting that this matter be treated as an urgent potential safety-defect issue rather than simply as an individual road-hazard or routine warranty claim. The second failure was treated by Jaguar Dallas Darrick my service rep on July 14th , 2026(approx. 5 weeks earlier) with the first tire instance June 1, 2026. You can imagine how upsetting this is considering this happened twice while on I-35 going 70 mph. TWC is my tire warranty company and replaced the damaged wheel as well.

I own a 2020 Jaguar I-PACE with approximately 102,000 miles that is currently at a Jaguar dealership following a sudden failure to accept Level 2 charging in mid-June 2026 and has been there since. The vehicle initially showed fault code P0AA6-00, and the dealership subsequently replaced the 12V battery and charging module (440123457); after communication with the vehicle's systems was restored, they reported that 31 of the 36 modules in the high-voltage battery were failed or failing and that the entire battery requires replacement. My concern is that this may be related to the H570/H572 high-voltage battery recalls. My vehicle had not yet received the recall-related charging limitation software, and it had previously experienced a significant battery/electrical issue in June 2025 involving Restricted Performance, an isolation fault, and BECM reconfiguration/software that apparently resolved the issue at that time. The dealership is now attributing the 2026 failure to normal age-related battery degradation, but I find it difficult to reconcile that explanation with 31 of 36 modules reportedly failing simultaneously, particularly given the vehicle's prior isolation fault and the current recall's stated concerns involving internal battery faults, short circuits, and thermal overload. The dealership has told me they did not perform a thorough investigation specifically to determine whether the current battery failure is related to H570/H572, yet they have concluded that it is unrelated. I am requesting that NHTSA review this situation because I am concerned that a potentially serious high-voltage battery defect covered by the recall may have caused or contributed to this failure. In the meantime, they have quoted me $50,000 to perform a full HV battery replacement, as a potential option/resolution.
