At approximately 4,588 miles on my 2024 Lexus LX 600 (VIN: [XXX] , built May 2024), the orange brake system warning light (exclamation mark in a circle) illuminated on the dashboard while the vehicle was parked and running. No prior braking issues, unusual noises, or loss of pedal feel noticed. The light stayed on steadily after restart. This matches symptoms in Lexus TSB L-SB-0022-22 (DTC C1391: Abnormal leak in brake accumulator/accumulator assembly). It disables ABS, VSC (Vehicle Stability Control), brake hold, and other safety features, posing a potential risk in emergencies or low-traction conditions (e.g., off-road use). No crash or injury occurred. Vehicle is under warranty; planning dealer service for actuator replacement. Dashboard photo available if needed. Experienced unintended acceleration when pulling into a parking space. My vehicle went over the curb and into a ditch. The bushes and embankment of the ditch stopped the vehicle. There were no warning lamps, messages or other symptoms of a problem.

I was driving at 45 mph during a sunny day with normal traffic flow. The right rear tire had suddenly exploded. It almost caused a major accident. Upon inspection, the tire was totally blew out. Only the rim was left. The tire expert at Northside Lexus dealer in Houston said he had seen hundreds of blow out tires on Lexus like that. He didn't want to alert NHTSA. The tires were made by Dunlop in 2023. My car had only 20,000 miles on it. Please consider to recall this kind of Dunlop Sport Maxx tires before another accident or fatality happens. Thanks.

VIN: [XXX] Current Mileage: 70,440 Vehicle: 2018 Lexus LS500 In November 2021, I purchased this vehicle with approximately 19,000 miles. Routine maintenance was performed every 7,500 miles. At approximately 44,000 miles (in 2023), the vehicle experienced severe engine failure following warning lights for Parking Support Brake Malfunction, Secondary Collision Brake System Malfunction, Check Engine, Reduced Engine Power, and Traction Control Off. The vehicle lost power and stalled. The engine began emitting smoke and was replaced under manufacturer warranty in March 2024 at a Lexus dealership. The repair exceeded $50,000 and included a new engine block and related components. Shortly after receiving the vehicle back, additional major failures occurred: Fuel pump failure resulting in “Reduced Engine Power” and restricted speed (~20 mph). The pump was replaced under warranty (approx. $4,600 cost). Brake system failure occurring three days after that repair (January 2024). As of October 28, 2025, the vehicle has again failed and is currently at Christian Brothers Automotive with the following findings: “Fuel pressure sensor located under the intake manifold is actively leaking fuel when the engine is running. This creates a fire hazard and can affect engine performance and fuel pressure regulation. Immediate attention required.” This repeated pattern of engine and fuel-system failures within a short period after a full engine replacement raises serious concerns about component reliability, workmanship, and safety. Fuel leakage near the intake manifold poses a significant fire risk. There are currently no open recalls addressing this defect. I request that NHTSA investigate potential defects or systemic issues in the engine or fuel-delivery systems of the 2018 Lexus LS500, as these failures appear premature, hazardous, and recurrent despite dealer and manufacturer repairs. On Monday, Oct. 27, 2025 at approximately 11:50 am, my vehicle experienced unintended acceleration. I was pulling into a parking spot and suddenly accelerated and jump the barrier into a ditch. Luckly there were bushes to stop me. I was standing on the brake. There were no prior indications there was an issue.

The electronic screen on dashboard that displays map, HVAC, etc. turns pink then black and disappears for several minutes. One then has no control over the various functions displayed on the screen. I've done a hard reset by disconnecting the battery for several minutes...to no avail. Thank you.

I have a stick dashboard and it’s impairing my vision to see out of front windshield. Lexus did have a recall, but it expired. However, I’m still in danger while driving because of glare.

The contact owns a 2007 Lexus LS460. The contact stated that while driving at various speeds, the vehicle hesitated while responding. The vehicle was taken to an independent mechanic, but no diagnostic trouble codes were retrieved; however, the contact was informed that the failure was related to the knock sensors. The contact related the failure to TSB: L-SB-0122-17, which the contact stated was to be performed after the completion of NHTSA Campaign Number: 10V309000 (Engine and Engine Cooling) to prevent the engine hesitation while driving. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was approximately 157,000.

Driver's seat is painful after 15 minutes. It is available for inspection. Causing shifting of position and pinching of nerves reduced feeling to foot. Problem has not been confirmed by dealer. Vehicle has been inspected by manufactures dealer. Problem first appeared 7/26/2025

I own a 2018 Lexus NX F Sport and have had repeated fuel pump failures since purchasing the vehicle in October 2022. We have replaced the fuel pump twice — once at the dealership where we bought it and again less than a year ago. Despite these repairs, the same issue continues to occur. There were no warning lights before the failure, but once it happens, the check engine light turns on, the traction control light comes on, and the vehicle goes into limp mode. When this happens, the car will not accelerate when pressing the gas pedal. It slows down rapidly and eventually stalls completely. This has happened multiple times, including once in the middle of Dallas traffic and another time while driving to Midland, TX. Each time, my children were in the car with me, putting all of us at serious risk. I have read about the Lexus fuel pump recall affecting similar models and years, and my vehicle is showing the exact same symptoms, yet my VIN is not included in that recall. I believe my vehicle should be reviewed for inclusion in the existing Lexus fuel pump recall, as this ongoing defect poses a major safety hazard to me and others on the road. Recall number: 20LA01 for Lexus (low-pressure fuel pump) — for certain 2018–2019 Nexuses and other Lexus models. Description: “Low-pressure fuel pump inside the fuel tank may fail. If it fails, warning lights and messages may display and the engine may stall while driving.” Additional support: 23LE01 (a Customer Support Program, not a direct recall) covers certain model years with low-pressure fuel pump defects, repairing under this program until July 15 2036 or 150 k miles. I believe my vehicle may fall under recall 20LA01 (Lexus low-pressure fuel pump) or the subsequent support program 23LE01, because the symptoms and repeated fuel pump failures match exactly, yet my VIN returns no open recall. My car currently has 144,400 miles on it. I’d like to get it fixed as swiftly as possible. Diagnostic tests are too costly.
