  • 2018 TOYOTA PRIUS
    - Orange Park, FL

    The contact owns a 2018 Toyota Prius. The contact stated that while in bumper-to-bumper traffic, the vehicle suddenly accelerated. The brake pedal was applied but failed to respond. The driver crashed into the front passenger side of the Lexus. The vehicle continuously moved forward. The contact was eventually able to pull over safely. The pre-collision malfunction warning light illuminated. The brake malfunction light was flashing. The air bags did not deploy. There was no reported fire or injury. A police report was filed. The vehicle was taken to the local dealer, who could not determine the failure. The vehicle was not diagnosed or repaired. The vehicle was then towed to an autobody center where it remained. The manufacturer was contacted, but no additional assistance was provided. The approximate failure mileage was 39,000.

    As I was driving on a local highway going approximately 50 mph, the hood of the vehicle released and hit the windshield with full force. Luckily there was no traffic on the local highway at the time of the incident. I did not release the hood at all, and the dealer reports verbally to me that the hood latch is damaged. The incident could have resulted in my death if it happened on the highway.

    The Airbag lights were lit, the person sitting with seatbelt on (flashing) and the person sitting with an inflated airbag in front of them. I ran an OBD2 scan on SRS and got code B1656. Driver seat belt buckle Left Hand. I Investigated the latch and found one wire was snapped off and a couple of inches shorter that the wire that was still connected. I have included 2 pictures of the back end of the seat belt latch. You can see the broken wire and where it use to be connected.

    - NAPERVILLE, IL

    My 2018 Toyota Prius Supreme was parked too far away from the curb in front of a house where we were invited, in a residential area in Naperville IL. I entered the car to park it properly. I had my foot on the brakes and pressed the starting button for the engine to turn on. I then put the car on reverse and suddently the car accelerated uncontollably. Went on the grass, hit the neighbor's mailbox and stopped by crashing on a tree. I was in shock but luckily not injured. Other guests were present and witnessed the incident. Police was called in and issued a report. The car was declared by Progressive Insurance Co as a total loss.

    Someone rear-ended my Prius and they were driving a Ford 250 truck pulling a large flatbed truck that had a huge box on it. My seat went forward and backwards - I slammed my knee under the dashboard and then slammed by head into the headrest and I could not reach the pedals after this. I have repeatedly asked the collision center to look at it because there is nothing about it on the report of the estimate of repairs. My adult son was a passenger in the front and his seat didn't move.

    Airbags deployed prematurely as I was parking my car in a doctor's office parking lot. The car was going less than 5 mph and I did not hit anything. The premature deployment of the airbag startled me and caused me to hit the accelerator instead of the brake and I ended up crashing my car into a nearby pole and another .automobile. The accident resulted in a soft tissue injury for my daughter and I'm experiencing lower back pains. The air bag caused burns on my nose and arms. My daughter has bruising on the arms from the air bag deployment

    - Los Angeles, CA

    I entered a large, strip mall parking area and was driving at the vastly reduced speed one would use in such a situation. My intention was to park my car to enter a store in the mall. I turned a corner to park on the side of the building (parking options were limited). As I entered a parking space (moving at the speed one would drive to do so; essentially INCREDIBLY slowly; moving forward into the space at almost 0 MPH and already applying the brake). Suddenly and with no warning, the car violently lurched forward at a vastly increased speed. I pressed down on the brake harder to try to stop the vehicle, but it propelled into and over the concrete curb stop and into the thick brick wall in front of me. There is considerable damage to the front end of my vehicle as a result. I was horrified at this dangerous situation. My safety was definitely at risk. I feel very fortunate that I wasn't seriously injured (and that no one, either a pedestrian or another driver, was involved in the accident). The airbags didn't deploy and I was of course, wearing my safety belt, which kept me from any severe injury. I do feel some physical injury effects this morning however (sore neck, shoulder, back; a slight headache). I'm not aware of any warning lamps, messages, etc. going off, but I must say everything happened very quickly. I've reported the accident to my insurance company & filed a claim. Given that the incident happened on the evening before the July 4th holiday, my insurance company scheduled vehicle towing (flatbed) for my car. It's my understanding that it's being stored at the tow company location & will be delivered tomorrow (Wednesday, 7/5/23) to the certified body repair facility recommended by my insurance company and chosen by me. An insurance adjuster will be doing their evaluation, likely in the next few days, at the earliest tomorrow morning. I've also advised the insurance company of these details & told them of my safety concerns and fears.

    Take foot of break, car accelerates under power. Sooner or later, this is going to result in killing a pedestrian. The question has been for the last year, is this a behavior that can be gotten used to. The answer is definitely NO! This resulted in a near collision with a pedestrian today. Toyota believes the behavior is intentional and will not confirm the safety issue. Farmers Insurance is willing to believe the issue exists without inspection but has no idea what to do. You have a problem on your hands. Biden is trying to transition almost everybody away from gasoline vehicles; thus those that drive manual transmissions will be driving electric vehicles, and their brain patterns have not changed and the procedure for getting rolling from a dead stop will be done the old way, and it's now dangerous. In a manual transmission it is necessary to transition off the break before completing looking all three ways (left, right, straight) because the transition time from break to power is so long that if the looking is done before, it is out of date by the time power is being applied. Applying the same procedure to an electric car has potentially lethal results; and people who have been driving a manual transmission for a long time will end up using its procedure in an electric car by habit. Perhaps you observed the reports over the last few decades of old men with new cars having unintended acceleration problems with no cause found. I now believe that a number of these have a very similar explanation. Car isn't off, take foot of break to get out of car, car accelerates. In an electric car, telling the car isn't off isn't very easy any more. This is easy to understand. An automatic transmission vehicle has an unremovable dumb behavior where take foot off break accelerates, whereas in a manual transmission this stalls, which is a safer result. There is no reason this behavior should carry over to computer drive, including electric or hybrid vehicles.

    The automatic high beams will occasionally fail to operate and are stuck in low beams until about 2 minutes have passed, or until the automatic high beams switch is turned off to disable the system. This seems to be a software issue, as it seems to occur when an approaching vehicle passes by with bright lights. When this happens, all high beams are disabled for a couple of minutes, even if the high beam lever is switched to low beams and back to high.

    Air bag did not deploy during a head on collision at 25-30 mph. No warning lamps. Car is totaled according to tow truck driver. Currently at the body shop, waiting for adjusters approval for body shop to take car apart to assess the damage.
