  • 2022 CHEVROLET CAMARO
    - RICHMOND, TX

After 3 weeks of no using the car, the radio system stopped working. This included the backup camera, the charging ports, the GPS, and other screen features. Due to the nature of the size of the car and the blindspots, this made it really hard to see other cars/people when reversing. The car was seen by the dealership and I was told I would need a new screen. After replacement of the screen, the problem still wasn't fixed and I was told I would need a new radio. Changing the radio fixed the problem. Prior to this incident, there was no warning signs. Upon discussing with the dealership, this problem has existed with other Chevy models for unknown reasons.

  • 2022 HYUNDAI PALISADE
    - Carlisle, PA

Component Affected: FUEL SYSTEM, GASOLINE: DELIVERY: HIGH PRESSURE PUMP Description of the Safety Hazard: My 2022 Hyundai Palisade has developed a catastrophic failure of the engine-mounted High-Pressure Fuel Pump (HPFP). The insulating foam cover is completely saturated and leaking raw, liquid gasoline directly on top of the engine block. The fuel system is unable to maintain rail pressure, causing severe pre-detonation engine knock ("pinging") and mechanical stumbling during acceleration. Furthermore, there are visible electrical flashes/short-circuit arcs occurring in the engine bay directly adjacent to the high-pressure liquid fuel leak, presenting an imminent vehicle fire hazard. Active dark fluid puddles contaminated with fuel are dripping past the splash shields onto the ground. Dealer/Manufacturer Actions: Despite clear photographic evidence of a pressurized fuel leak on a powertrain-covered component, multiple authorized Hyundai dealerships (Fred Beans, Harrisburg, and Chambersburg) have refused service or grossly misdiagnosed the life-threatening safety issue as a 'gas cap' or 'water hose.' Hyundai Motor America Corporate Consumer Assistance has failed to intervene or provide emergency towing remediation for this severe fire risk.

    - Spokane, WA

The timing chain and camshaft broke the car froze while driving 70 mh. No warning. With about 67000 mi on it. Yes it can be inspected. At risk on the freeway going 70 mph. Was able to coast to the right side before engine locked the wheels. Not able to restart. Towed to an independent garage. They gave a diagnostic test and then opened it up to find timing chain broke and caused camshaft as well as other damages. No warning! Taking it to Moses Lake Honda dealership tomorrow for another evaluation and replace the engine if needed. Low mileage and well maintained! Should have never happened.

  • 2024 TOYOTA TUNDRA
    - Broken Arrow, OK

Subject: NHTSA Safety Complaint: Sudden Engine Seizure and Loss of Motive Power – 2024 Toyota Tundra (Recall 25V767 Alignment) To Whom It May Concern, I am writing to formally lodge a critical safety complaint regarding a life-threatening mechanical failure. On XXX, while driving at highway cruising speeds under normal operating conditions (no towing or payload), my vehicle suffered an abrupt, complete internal engine seizure and a total loss of motive power. Simultaneously, the instrument cluster displayed a "Pull over to level ground, oil hot" thermal alert, indicating an acute, systemic mechanical breakdown. This failure sequence directly aligns with NHTSA Recall 25V767 (Toyota Recall 25TA14 / 25TB14), which addresses permanent main bearing destruction caused by manufacturing debris left inside the V35A engine block. My local dealer, [XXX] Toyota, intends to process this repair under an extended warranty. I strongly reject this classification. Because this catastrophic failure perfectly matches the technical parameters and safety risks of an active federal campaign, it must be handled strictly under the purview of the federal recall framework. I request that NHTSA document this event to ensure Toyota is held accountable under its recall mandate. Sincerely, [XXX] [XXX] [XXX] | [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

  • 2016 HYUNDAI ELANTRA
    - Overland Park, KS

I am reporting a recall remedy failure involving my 2016 Hyundai Elantra under Hyundai Recall 256 / NHTSA Recall 24V-146 (trunk latch pawl defect). The McCarthy Olathe Hyundai dealership in Olathe, KS marked the recall as “completed,” but the trunk latch still does not open and the defect persists exactly as described in the recall notice. After the recall was marked complete, I returned to the dealership because the trunk still would not open. The dealership told me the issue was “trunk lid damage” from a prior accident and advised me to replace the trunk lid at my own expense. I paid for the trunk lid replacement, but the trunk still did not function. When I returned again, the dealership changed their explanation and said the problem was actually “rear frame damage,” and that they had been wrong to tell me to replace the trunk lid. Which I have audio recordings of but am unable to upload due to format restrictions. Despite these conflicting explanations, the recall defect remains unresolved, and I have been without trunk access for over a year. I contacted Hyundai Corporate (MyHyundai USA), but they responded that this was “not a part issue covered under warranty.” This appears to be a misunderstanding, because this is not a warranty claim — it is a recall remedy failure. The recall was marked complete even though the defect persists, and the dealership’s misdiagnosis led me to pay for repairs that did not address the recall defect. This situation has caused financial loss, safety concerns, and prolonged inability to access the trunk, spare tire, emergency equipment, and cargo space. I am requesting NHTSA to document this recall remedy failure and dealership misdiagnosis.

  • 2018 HONDA ACCORD
    - Plymouth, NC

The car ACC turns orange, you can’t control your speed, the car starts to slow down like it’s losing power. I got a new battery. Still will cut off during drive and you can’t accelerate. This could cause a major accident. I was turning into a parking lot and lost power. Had to cut the engine off, and hurry up and restart it just to get out the way.

  • 2023 HYUNDAI TUCSON
    - Hudson, MA

2023 Hyundai Tucson Limited, 2.5L GDI, Alabama-built (VIN 5NMXXXXXXXXXXXXXX), built [XXX]. On [XXX], at about 37,550 miles, the check engine light came on flashing and the engine ran rough while I was traveling in Canada. My scan tool showed P0300, P0302 and P0304 (misfires on cylinders 2 and 4). On July 6, 2026, Experience Hyundai in Charlottetown, PE (service order 92XXX) replaced one ignition coil under warranty. No fuel injector leak test was performed. The misfires returned. On [XXX], at 39,409 miles, my scan showed P0302 active and P0304 pending. The car also had extended cranking after sitting and the check engine light was on. I was told I could keep driving the car unless it started to jerk or engine light started to flash, then immediately stop driving it and have it towed. On September 25, 2026, the car was towed to McGovern Hyundai Route 2, Leominster, MA (invoice 148XXX). The dealer found all 4 fuel injectors leaking and replaced them under warranty with injector service kit 35399-2S133QQH. My vehicle is outside the production range of Hyundai TSBs 23-FL-002H-1 and 26-FL-001H, which cover this injector failure only for Alabama-built Tucsons produced through July 11, 2022. Yet it failed in the same way. I am concerned the defect extends beyond the bulletin dates and that a sudden misfire could cause loss of power in traffic. There is no recall for this condition. The dealer found all 4 fuel injectors leaking, even though only cylinders 2 and 4 had set misfire codes and replaced them under warranty with injector service kit 35399-2S133QQH. • 2020 KIA TELLURIDE
    - Burlington, NC

Purchased vehicle 8/31/2021 - 32k miles 10/18/21 - first mention of oil consumption (opened case with Kia) 2023,2025,2026. 3 more cases filed and DENIED with Kia customer care. Filed BBB claim, filed BBB anutoline claim, also hired lemon lawyer. Kia offered $6k to go away (I declined). *July 18,2026 Kia finally diagnosis and recommends engine replacement. Bore scope was used to discover scoring on cylinders from significant oil consumption. Vehicle is now smoking at take off (112k miles), current open case with Kia customer care again. Requesting buyback or settlement for the 3rd time in 5 years. Still paying $600 a month Safety concern: on a trip out of state vehicle dash lit up every icon and went dead in the middle of a packed 4 lane highway , started with power steering, no lights, no horn , couldn't roll window down.(video avail) Vehicle towed to dealership. Had replaced battery 2 times already. This has happened twice and my family was in the vehicle once on the large highway, very scary no one knew we were broke down bc we had no way to show it. People blowing and zooming by mad. Low oil light doesn't exist on this vehicle, I've been bone dry with no indication EVER. I now ride around with a 5 quart jug of oil and funnel in the back of my car since 72,000 miles. This should be illegal. Rear suspension: 4 times the rear suspension has been replaced (7 times in the shop to be fixed one time over 30 days) and has never been fixed , now making the same knocking noise it's always made. The car is a lemon and Kia knows it and will not stand behind their product. I DO NOT FEEL SAFE IN THIS VEHICLE, kia has been notified numerous times in every avenue and every way I could communicate, they say "we will call you with an update when we have one" that call NEVER happens.

  • 2021 ACURA TLX
    - Perris, CA

I am submitting this complaint regarding a major drivetrain failure in my 2021 Acura TLX Type S SH-AWD with approximately 86,643 miles. My vehicle experienced a complete loss of AWD. The front two wheels are receiving power, but the rear wheels are no longer receiving power. An independent mechanic initially diagnosed a transfer-case/drivetrain failure and advised that the transmission may also be involved. I then took the vehicle to OH Acura of Temecula, where the Acura dealership independently diagnosed the vehicle with transmission and transfer-case failure. The service manager confirmed that my independent mechanic’s diagnosis was accurate. The dealership provided a repair estimate of approximately $16,000. I am concerned because NHTSA previously opened investigation DP26-001 regarding reported failures involving the transmission and transfer case in 2021–2023 Acura TLX SH-AWD vehicles. My vehicle is now experiencing a complete loss of AWD consistent with the issue described in that investigation. I also experienced an unusual grinding noise on September 21, 2026, which lasted approximately five minutes before stopping. I was subsequently told the noise was caused by worn spindles. The noise has not returned, and I am not claiming that issue was related to the drivetrain failure. Acura declined goodwill assistance, citing lack of documentation for transmission maintenance. The vehicle is outside its original warranty. I am submitting this complaint so NHTSA has a record of my vehicle’s actual failure and complete loss of AWD and can consider it alongside other reports involving this vehicle and drivetrain configuration. I can provide the VIN, dealership diagnosis, repair estimate, and supporting documentation if requested.
