2022 FORD F-150- Camden, NC
The daytime running lights are out. Vehicle is available for inspection. This puts my safety and the safety of others at risk by not being o by reducing visibility of the vehicle during low light scenarios where the headlights are off. Example I live in an area with lots of fog and drive a gray vehicle that is not clearly visible if headlights are off due to light exposure. Problem is consistent and able to be reproduced for inspection. Vehicle has not been inspected by any agent to this point but it is available and will be noticed upon inspection by direction. There are no warning lamps or messages alerting driver of malfunction which poses another safety risk. Problem has been consistent for past 3 weeks upon every start.

2019 INFINITI QX50- Lewisville, TX
#11753348 - on 7/1/2026, Infiniti Dealer found metal shavings actively present in my oil pan during a recall inspection. They passed the vehicle as good condition stating nothing found per a vehicle inspection report given to me. On July 25, 2026, my engine red light came on while driving and my engine began to shake violently and making a loud clunking noise. I towed the vehicle back to the dealer for another recall inspection. On 8/30/2026, the dealer informed me that the engine was failing due to the recall and needing bearing replacement but due to it passing back in July I would be responsible for replacing the bearings. They invoiced me for excess of $15,000, the diagnostic inspection of $270 and demanded that I return the loaner vehicle. When I returned the loaner vehicle on 9/3/2026, they attempted to sell me a new/pre-owned vehicle instead of providing me a written denial refusal notice of not honoring the vehicle recall. On 9/5/2026 Infiniti Consumer Affairs representative verbally confirmed that they were informed by the dealer that there were metal shavings present in my oil pan but denied the repair because there were not "chunks" or "clusters" of metal present. The manufacturer verbally claimed that the metal debris did not meet an internal, undocumented threshold of "chunks" or "clusters," and explicitly stated that Infiniti considered my vehicle's mileage and vehicle age and sludge present on the latter inspection that was not present on July 1 nor reported in making this denial decision. The representative also referenced a photograph sent to them at the end of August as justification for the denial. This photograph was not taken nor sent to them during the actual July 1st inspection. The dealer had already informed me that they did not send the manufacturer a photograph on July 1. The manufacturer refused to provide me written notice refusing to honor the recall and said to get it from the dealer.

2025 HONDA RIDGELINE- Caguas , PR
I am reporting an urgent potential safety defect involving my 2025 Honda Ridgeline Sport, in Puerto Rico. Since the first oil/filter service, the vehicle has repeatedly developed a grinding/growling/knocking-type noise and physical sensation through the steering wheel and cabin, especially on rough, porous or uneven pavement and during minor road irregularities or braking/load changes. The condition has been reported to Honda-authorized service multiple times (about four visits over nearly a year), but the cause was initially not identified. On 08/20/2026 Flagship Honda advisor informed me it was rack and pinion broken. On 08/24/2026, Flagship Honda (Kennedy Ave., San Juan, PR 00920), RO #XXXXXX, at 12,556 miles, documented a steering-system inspection and found a “golpe” (knock) in the steering column. The written diagnosis states the steering column had internal wear in the motor and “cruje al moverse” (creaks/cracks when moving), requiring replacement. Honda authorized the repair. On 09/01/2026, under warranty, Honda replaced the steering column assembly (Part #XXXXXX) under RO #XXXXXX, at 12,870 miles. The dealer documented replacement, alignment, and a 2-mile road test, stating the condition was corrected. However, the exact same grinding/growling noise and physical sensation remain after replacement. I reported this again to the dealer on 09/07/2026 and was told another appointment with a Honda inspector is required for further diagnosis. I am very concerned because this recurring steering-related condition remains unresolved in a nearly new vehicle, and the true root cause is unknown. I am concerned about possible loss of steering control or another safety consequence, especially with my family in the vehicle. I respectfully request that NHTSA review this complaint and determine whether this may represent a safety-related defect affecting other 2025 Ridgelines. I have repair orders and warranty documentation supporting these facts.

2019 ACURA RDX- Santa Fe Springs, CA
The factory-installed LED Daytime Running Light (DRL) system on my 2019 Acura RDX has failed. The passenger-side DRL is completely inoperative, and an authorized Acura dealership documented that the driver's-side DRL is beginning to fail as well. The affected components are available for inspection upon request dealership has declined to provide further documents and American Honda Moto Daytime Running Lights are a safety-related exterior lighting system designed to improve vehicle visibility to other motorists. The failure of one DRL and the developing failure of the second DRL may reduce vehicle visibility and create a potential safety concern. On July 3, 2026, Cerritos Acura inspected the vehicle and confirmed the condition. The dealership's written diagnosis states: "Recommend passenger headlight assembly due to daytime running light out. Recommend driver's headlight assembly due to daytime running light going out as well." The dealership recommended replacement of both headlight assemblies. The dealership found no evidence of collision damage, misuse, unauthorized modifications, or water intrusion. The DRL is an internal LED component that is not serviceable separately and requires replacement of the entire headlight assembly. The vehicle has been inspected by Cerritos Acura, an authorized Acura dealership. To my knowledge, the vehicle has not been inspected by police, insurance representatives, or any government agency there is no record of any only with the dealership. There were no warning lights, warning messages, dashboard indicators, or other symptoms before the failure occurred. The DRL failure happened without warning and was later confirmed by dealership inspection. I am concerned that the failure of both factory-installed LED DRL assemblies may indicate a defect in the lighting system rather than normal wear and tear. The condition was confirmed by an authorized Acura dealership and was not attributed to accident damage or other external causes.

2024 TOYOTA RAV4- Louisa, KY
On 9/06/2026 my 2024 Toyota RAV4 Hybrid was involved in a severe collision where the impact occured on the drivers side at the passenger door. The vehicle spun several times before coming to a stop. During the spins the front passenger door flew opening. My wife was sitting in the passenger seat her seatbelt successfully restrianted her preventing ejection and injury. However the unlatching of a non impacted door under rotational forces represents a catastrophic structural or mechanical latch failure that poses an imminent risk occupant ejection.

2016 HONDA PILOT- Kitty Hawk, NC
The 6-speed automatic transmission on my 2016 Honda Pilot EX-L (103,200 miles) has experienced a severe mechanical failure casing dangerously hard, violent gear shifts during both acceleration and deceleration. Critically, no dashboard warning lights or malfunction indicator lamps illuminated while this dangerous driving condition persisted, leaving me with no advance warning of a safety hazard. A subsequent diagnostic scan at an Authorized Honda dealership revealed hidden diagnostic trouble code P0776 (pressure control solenoid "B" performance or stuck off) and a failing torque converted. The severe, unpredictable jerking and hesitation during shifting creates an immediate traffic hazard during acceleration in traffic. Approximately 45 days prior, an out-of-town Honda dealership performed a prescribed triple fluid flush when the issue first arose. Then, 5 days later, my local Honda dealership performed the prescribed software update for shifting issues, which failed to remedy the underlying defect again. The dealership has now quoted $11,000 + for a full replacement of the transmission and torque converted. American Honda corporate has opened Case Number: 16606142 to track this defect.

2026 SUBARU OUTBACK- SAN JOSE, CA
Incident Summary: On August 23, 2026, a multi-occupant injury event occurred during routine city braking due to an inherent occupant packaging design defect in the vehicle’s front seating and sun visor layout. The vehicle exhibits a static seat-to-visor clearance of 79 cm versus a standard median adult sitting height of 95 cm, result of negative 16cm headroom clearance. During normal city deceleration (0.2g–0.4g), seatbelt inertia reels remain unlatched under FMVSS 209 tolerances, allowing normal forward upper-torso pitch. This causes adult occupants' foreheads to strike the lower edge of the stowed sun visor, resulting in head impacts, neck strain, and concussion symptoms. A service representative at Capitol Subaru personally observed the hazard during intake. Subaru Quality Assurance inspected the vehicle but closed the file by relate it to quality issue as claiming no "part manufacturing defects" exist, while ignoring the negative physical clearance envelope as saftey issue. Subaru's recommended workaround—sliding the seat back and make seat lean backward—does not eliminate visor contact during forward torso pitch and severely compromises rear-cabin legroom, causing physical leg entrapment and impact injuries to rear passengers during this same deceleration event. On the driver side, sliding rearward prevents safe operational reach to the brake and accelerator pedals, even with these saftey issue highlighted to Subaru, they keep insist to slide seat back to clear the sunvisor even with been notified multiple times that doing so, the driver cannot press the pedal and sterring wheel or reach to the control panel.

2020 TESLA MODEL 3- Reston, VA
I own a 2020 Tesla Model 3. Over approximately the past 2–3 weeks, the vehicle has developed a noticeable rattle/creaking noise coming from the left-front suspension area. The noise was not previously present and appears to be getting my attention as a potential suspension-related safety issue. I am concerned because my vehicle falls within the model-year range of Tesla Model 3 vehicles associated with reported front suspension/lateral-link issues. I understand NHTSA is investigating reports involving front suspension component failures that could potentially affect vehicle control. I contacted Tesla through the Tesla app and requested that the vehicle be inspected because of the suspension noise and my safety concern. Tesla provided an estimate requiring approximately $240 for diagnosis/inspection before determining the cause. At this time, I have not experienced a suspension separation, crash, injury, or loss of vehicle control. However, because the vehicle is producing a new and persistent noise from the left-front suspension area, I am concerned that this could be an early indication of a suspension component problem. I am submitting this complaint so NHTSA has a record of the symptoms on my vehicle and can determine whether they may be related to other reported front suspension failures. I am also requesting that this complaint be considered in connection with any ongoing investigation involving front suspension/lateral-link failures in 2020 Tesla Model 3 vehicles. I will retain Tesla service records and any inspection findings and can provide additional information if requested.

2020 HONDA PASSPORT- Fall Branch, TN
Adaptive Cruise Control Automatic Emergency Braking Forward Collision Warning Lane Departure Warning Lane Keeping Assistance Auto -high beams Every time I start up my car all the above error messages come up stating there is a problem. My cruise control is not able to be used at all. This appears to be a very common issue amongst Honda Sensing from the research I have done. Honda was even taken to court over this issue.

2021 VOLKSWAGEN ID.4- Davenport, FL
Faulty sensor under passenger seat will cause the airbags to go off while someone is in the passenger seat, buckled. Will go on and off intermittently. Alert on dash will illuminate and overhead notification of passenger airbag shutting off flashes.
