2017 HYUNDAI TUCSON- Indianapolis, IN
Vehicle is losing a lot of oil. But there is no oil leaking on the ground. Upon looking at the safety recall, it says it might be fire hazardous. I noticed smoke coming from under the hood. Vehicle run rough and also hear sounds.

2022 JEEP GRAND WAGONEER- Chanhassen, MN
My 2022 Jeep Grand Wagoneer has been subject to an open safety recall involving the rear passenger-side window trim since I purchased the vehicle in 2025. I scheduled and attended numerous appointments with the authorized Jeep dealership seeking completion of the recall repair, but the defect was never remedied. On September 7, 2026, my husband was driving the Grand Wagoneer while I followed behind in another vehicle. My two toddlers were occupants of my vehicle. Before the incident, I observed the recalled rear passenger window trim flapping and becoming loose. The trim then detached completely from the Grand Wagoneer while both vehicles were traveling at approximately 75 mph on the highway. The detached trim entered or moved toward my vehicle’s path, forcing me to swerve suddenly to avoid striking it. This created an immediate risk of losing control, colliding with another vehicle, or leaving the roadway. The incident endangered the occupants of both vehicles, including my two young children, as well as other motorists. Fortunately, no collision or injuries occurred. The flapping trim was the only warning immediately before it detached. The dealership had prior notice of the unrepaired recall because I had brought the vehicle in approximately four times beginning in November 2025. Despite those appointments, the recalled component remained on the vehicle and ultimately failed in the manner the recall was intended to prevent. The vehicle and photographs of the affected area are available for inspection. The detached trim was recovered. Dealership service records and recall communications documenting the unsuccessful repair attempts are also available.

2025 AUDI Q6 E-TRON- Cary, NC
Sudden complete loss of propulsion while driving, with no opportunity to safely leave the travel lane; failure repeated again seven miles later after vehicle restart. Once propulsion is lost the driver has 1 to 2 seconds before multiple loud banging noises and the vehicle shuddering to an immediate stop without the driver using the brake pedal. The warning on the dash was "High Voltage Electrical System Fault" all in red.

2024 JAGUAR I-PACE- Tempe, AZ
On July 25, 2026, at approximately 7:10 PM, I was a passenger in a fully autonomous Waymo vehicle traveling southbound near 24th Street and Southern Avenue in Phoenix, Arizona. The vehicle entered the intersection to make a left turn and, while still in the active intersection, unexpectedly reversed. I considered this a serious safety concern because of the potential risk to surrounding traffic. Fortunately, there was no collision or injury. I reported the incident directly to Waymo and subsequently followed up, asking them to review the vehicle’s video and driving data. Approximately six weeks have passed, and Waymo has not responded to me regarding the incident. I am reporting this to NHTSA so the automated-driving behavior can be documented and reviewed as a potential vehicle safety issue.

2025 TESLA CYBERTRUCK- Plano, TX
I own a 2025 Tesla Cybertruck AWD. The fixed lower rail beneath the front passenger seat has two exposed openings. On September 7, 2026, a metal key fell through one opening into an inaccessible enclosed area beneath the seat and floor structure. The key could not be seen or retrieved. While driving, I repeatedly heard it sliding as the vehicle accelerated, stopped and turned. I attempted to reach it by creating a hand-sized opening in the lower protective panel, damaging that panel, but I still could not locate the key. While parking in my garage, I suddenly heard the key drop into a deeper area instead of continuing to slide. The unexpected sound startled me and I immediately braked. During this abrupt reaction in the confined garage, the right exterior mirror struck the garage structure and was damaged. This presents a design and safety concern. Small everyday objects can fall through the exposed seat-rail openings into an inaccessible vehicle cavity, move unpredictably and distract the driver. There is no apparent cover, screen or plug, and no reasonable owner-accessible retrieval method. I request that Tesla explain the purpose of these openings and why they are unprotected; inspect the vehicle and confirm whether the key could affect wiring, seat components, the battery enclosure or other systems; remove the passenger seat and necessary trim to retrieve the key; repair the damaged lower panel; and repair or provide reimbursement or goodwill assistance for the damaged right mirror. Tesla should also protect the vehicle’s existing paint protection film during repair and escalate the openings to its engineering or product-safety team for a preventive solution. I can provide photographs of both openings, the damaged panel and mirror, and can provide my VIN privately.

2025 LAND ROVER RANGE ROVER SPORT- Manhattan Beach, CA
2025 Land Rover Range Rover Sport P400 Dynamic SE, VIN [XXX]. Leased new March 2025; 11,690 miles. The instrument cluster repeatedly displays "Please check engine coolant level" although the vehicle is not due for service. The cooling system is sealed and coolant should not be consumed or lost on a vehicle this new, indicating a leak or a defective level sensor. Land Rover has documented this exact symptom in manufacturer communications filed with NHTSA for the 2025 Range Rover Sport (July 2025: engine coolant leak, low coolant warning) and in a dealer bulletin for turbocharger coolant feed/return hose O-ring leaks on the Ingenium I6 3.0L petrol engine (bulletin D061). An undetected coolant leak can lead to engine overheating and sudden loss of power. I am requesting that this defect be investigated and recorded. 2018 HONDA ODYSSEY- Liberty Twp, OH
Tail gate problems Looks like there are a lot of tailgate problems with the Honda Odyssey, including water leaks that can affect the control module. I replaced the control module, but it looks like there’s a lot of information and research about this issue online.

2021 HYUNDAI PALISADE- Cedar valley , UT
My car started having the airbag light come off and on randomly the end of January 2026. I caught the first photo February. My car engine shut off the first time March 18. 2026. I was at a traffic light, auto off turned off. Car switched to park, dash lights included: oil, battery, engine, and immobilizer incon. When trying to hit drive it tells me the driving conditions aren't met. I shut the car off and back on, everything reset. Contacted hyundai and no codes were stored. My car has now done this same thing the following dates: 6.5, 7.3, 8.4(car went into the shop August 6th-September 3rd). They could not duplicate what I told them, they drove the car 100 miles being mainly freeway miles though I told them its city driving and mainly idle and stopping. I drove my car 3 days and on 9.6.26 it did it twice within 5 minutes. First time on a main road/traffic lights, next in the parking lot I pulled in. Same scenario as above.

2019 JEEP WRANGLER- Detroit, MI
2019 Jeep Wrangler JLU experienced intermittent CAN-C communication failure. Vehicle has repeatedly experienced simultaneous loss of multiple electronic systems. On September 7th 2026, while driving, vehicle unexpectedly lost electric power steering, windshield wipers activated without command, ABS/TPMS/ESC warning lights illuminated, and vehicle subsequently would not shift into gear after stopping at a traffic light. Hazard lights would not activate. After shutting vehicle off, vehicle would not enter ACC or restart normally. Opening driver’s door unexpectedly caused vehicle to enter ACC. Manipulating the CAN-bus wiring/STAR connector behind the glove box has historically temporarily restored normal operation. Vehicle has previously required repair to this wiring/connector, and the failure has recurred. This has been occurring for at least 3 years and since 60,000 miles. This presents a significant safety hazard because steering assistance, shifting/propulsion, and signaling were simultaneously affected while driving. It is a known Chrysler issue (FCA STAR Case S2108000146) and should be recalled as there is a CLEAR and present danger with this ongoing issue.

2019 HONDA RIDGELINE- CASTLE ROCK, CO
On [XXX], at [XXX] am MDT, the closed sunroof on our 2019 Honda Ridgeline RTL exploded, without warning, while we were driving northbound on [XXX] north of [XXX]. At the time of the sunroof exploding, there were no cars close to us, in front or behind, and no overpass overhead. The glass projectiles of the sunroof, at high velocity, entered the cabin, struck and cut the back of my wife's left hand. My wife and I were covered in shards of glass. I could see a cloud of glass shards fall to the surface of the freeway in my rearview mirror at the time of the explosion. The vehicle is currently being serviced and estimated for damages at Ralph Schomp Honda in Highlands Ranch, CO.
