    Front passenger wheel cracked, daily driving route does not have potholes or any other debris that could have cause the rim to crack randomly.

    8 speed transmission will not shift to 8th gear at highway speeds. It will shift at a lower speed which makes merging onto an interstate dangerous. The vehicle gets better fuel mileage if it will use 8th gear. For it to use 8th gear on the interstate you must merge onto the road at a much slower speed than posted speed limit and then very slowly accelerate to posted speed limit which puts driver at greater risk of an accident.

    The glare from the dashboard into the windshield blocked my view almost completely when driving on the San Francisco International Airport ramp, especially when the car was on a downhill ramp. I could not see anything in front of me for a couple seconds. This could have led to an accident should the car infront of me had breaked and stopped. The glare is also visible in normal driving conditions on sunny days and can distract my attention from the road.

    The onstar module installed originally from manufacturer was faulty, causing electrical issues. This ended up draining the battery unexpectedly. No warning that the battery was low. Only had .03 volts left when tested at dealership. The onstar button was experiencing issues such as blinking and loss of connection. Onstar Support was no help, and the car manufacturer ignored issues as my phones software problem, until new battery was drained and issues persisted while driving. Ultimately this left the OnStar not working for weeks leading up to the drained battery which was unsafe in case of accident as well.

    BRAKING FAILURE on 2025 Equinox EV LT model - my car has had two separate brake failures going down a steep dry rocky road in clear weather, once with one pedal driving on and once without, on July 21st, 2025 and August 11th, 2025 respectively. It doesn't happen every time going down this road and each time at no more than 5 - 10 miles per hour, with the brake pedal to the floor the car was not slowing down and the weight of the car on the steep track made it speed up considerably which made it dangerous due to the steepness of the hill and turns in the road. The first time I drove off the road onto flat grass and the car came to a stop, the last time it happened I pumped the brakes until the car stopped and the "ABS engaged" light came on the dashboard above the steering wheel. It happened twice on the road that time. There were no prior alerts or warning lights. The car has 5000 miles and is still under warranty, the Chevy dealership had the car for 3 days. The dealership was not able to reproduce or confirm the problem with the brakes. The dealership told me they did a series of resets. They have not given me the written service report yet. I have an appointment to take the car back to the dealer. The brake fluid level is good, there are no current alerts on the dashboard while driving. The car is available for inspection upon request. I am concerned that my wife and I may be injured or killed in a braking failure or that we may injure or kill others or damage property.

    Telematics module stopped working out of nowhere and drains my battery to my vehicle. Had to have the vehicle towed to nearby dealship. Huge safety hazard as it is essentially the brains to my computer. App doesn't work without it, battery doesn't work and can't even get inside vehicle with key fob. Please issue recall.

    NHTSA Safety Complaint (Loss of Emergency Connectivity) Subject: 2025 Chevrolet Equinox EV — OnStar/telematics failure disables Emergency Services & Automatic Crash Response; part on national backorder VIN: [XXX] Make/Model/Year: Chevrolet Equinox EV, 2025, RS trim Mileage at first occurrence: 4000 Date problem first occurred: [XXX] Frequency: Continuous (services remain unavailable) Injuries/Crashes: None to date Component(s) affected: Electrical system — telematics/communication (OnStar); emergency calling/SOS; crash notification (ACR). Description of the safety defect: The vehicle’s OnStar/Connected Services display “Services Unavailable.” The dealer diagnosed a failed telematics module. Replacement is on national backorder with no estimated timeframe. While in this condition, Emergency Services and Automatic Crash Response are disabled—the SOS button does not connect, and the vehicle will not automatically contact OnStar after a crash. Parts availability (scope/delay): My dealership and multiple GM/Chevrolet and OnStar owner forums indicate 3+ month waits for the telematics module with no ETA, leaving vehicles without emergency connectivity for extended periods. What has been done so far: Dealer confirmed module failure and opened a parts order; advised no ETA due to national backorder. Manufacturer has not provided an interim fix or loaner vehicle. Issue remains unresolved as of Aug 11, 2025. Why this is a safety concern: Loss of emergency calling and crash notification defeats safety features marketed as critical for post-crash response. Owners cannot rely on the vehicle to summon help after a collision or medical emergency. Requested NHTSA action : Track this failure and parts unavailability, assess scope, and determine whether a safety investigation or recall is warranted.     Discovered water underneath passenger footwell carpet, causing a musty and mildewy smell. Many reports of similar discoveries online prompted the initial search. Safety concern is mold growth and rust. Dealer was unable to replicate issue after two attempts (ran through car wash, hose over windshield, ran A/C for extended period, checked AC condensate drainage). Dealer dried up water and carpet and told owner to monitor. Owner pulled up carpet to do own investigation after the second failed attempt by the dealer. Confirmed dry before starting investigation on warm sunny day (no rain). Owner found leak in passenger footwell underneath black access plate that only appeared with A/C usage AND during/after driving, not while car was stationary. Owner brought vehicle back to dealer. Dealer removed black access plate and confirmed leak was coming from a compromised body seam weld. Dealer repaired the failed body seam and no water intrusion has been seen by owner since repair was completed. Owner still has concerns about underlying cause of where water was coming from - theory is that condensation is forming ON the the A/C lines and is pooling somewhere in the body then sloshing around while the car is moving, causing further potential for water related issues, including rust.

    There is a very dangerous glare-reflection of the dashboard on the windshield whenever it is sunny. It is difficult seeing pedestrians, bicyclists and oncoming cars. We have had this car for a few days.

    At certain times of the day and depending on the lighting, the dashboard of my vehicle, has a very significant reflection that interferes with my ability to see through the windshield clearly. This puts myself and those in my path at risk of injury or death. I have contacted the person who sold me the car but he has not responded. I have read numerous online accounts of the same concern. GM is aware of the issue and chooses to do nothing.
