  • 2021 CHEVROLET BLAZER
    - Shelby Township, MI

My 2021 Chevrolet Blazer experienced a radiator failure at approximately 42,000 miles. The radiator and cooling system malfunctioned, causing coolant loss and triggering a warning light on the instrument panel. The failed radiator is available for inspection upon request. This defect put my safety and the safety of others at risk because a compromised radiator can lead to engine overheating, sudden loss of power, or even steering assist reduction while driving. These conditions increase the likelihood of an accident, particularly at highway speeds. The problem was confirmed by my Chevrolet dealer, who diagnosed the radiator as needing replacement. They initially quoted $1,800, later reducing the cost to $915.90 after acknowledging that approximately 20 customers per month at their dealership alone are experiencing the same issue. This indicates the defect is widespread, at least regionally, and may be systemic across the model. The vehicle has been inspected by the dealer service department, which is an authorized GM representative. No police or insurance inspection has occurred. Prior to the failure, a warning lamp illuminated on the instrument panel, alerting me to a cooling system issue. This was the first symptom I noticed before bringing the vehicle in for service. I believe this premature radiator failure represents a systemic defect in the 2021 Chevrolet Blazer’s cooling system. It occurred well within the 5-year/60,000-mile warranty period, yet the dealer claimed the radiator is not covered. I am requesting NHTSA investigate this issue, as it appears to affect many owners and poses a significant safety risk.

    - Sanford, ME

For the past 3 to 6 months I've been hearing a gear backlash sound while using the adaptive cruise control when it would slow down for a car in front of the vehicle then when it would start to speed up. Brought the vehicle to the dealership on October 20th, 2025. They said that the sound is normal and there's a service bulletin that says its normal in other Chevy models. Then on November 28th, 2025 I was driving home, turn right at a light, and started to accelerate. Then I heard the same backlash noise as before then the check engine light came on with the oil light. I pulled over immediately and turned off the engine just to see all the oil spilling out of the engine compartment onto the ground and white smoke. Got it towed to the dealership and they told me that "the engine had multiple components fail inside it and that I need a new engine assembly." Currently the engine is still in the vehicle awaiting a new engine assembly what is currently on backorder with no expected arrival time.

The upper radiator hose on some huge percentage of these trucks fail (mine has failed at just over 20K miles). You can check the Colorado Chevy forum, youtube, etc., et al, and find plenty of instances where this has failed. As this is the cooling system, it can cause the engine to overheat and potentially ruin the engine. The flaw seems to be an engineering issue with the rubber seal they use (instead of a high quality o-ring) to seal the upper hose to the radiator. Additionally, since this can cause engine failure it should be addressed immediately. Instead, my local Chevy dealer, since its not a recall item, said they could schedule a time next week for me to LEAVE the truck and then they would get to it in 7-10 days. This is extremely unfair to the consumer who depends on these vehicles. This should absolutely be a recall item. You can find 1000's of examples online and if you can pull GM repair data, I'm sure you'll find 1000's more.

I recently purchased this vehicle about five months ago. I took it to Riverside Chevrolet to get a diagnostic on it because my steering wheel is locked all the lights on the dashboard are on in the vehicle is not drivable almost a month and a half of Riverside Chevrolet and they could not get a diagnostic on it. Apparently the technicians there were not able to figure out what’s wrong with it. The vehicle is not drivable and it is a safety hazard

  • 2016 CHEVROLET VOLT
    - Fairfield, CA

Component/System Failed: The engine of my 2016 Chevrolet Volt experienced a sudden internal crack, leading to complete engine failure. The engine is available for inspection upon request. Safety Risk: The engine failure rendered the vehicle completely inoperable, posing potential risk if it had occurred while driving. Sudden loss of engine power could have caused a crash or left me stranded in traffic, endangering myself and others. Problem Reproduction / Confirmation: The failure was confirmed by a GM dealership, which performed a full inspection and documented the engine crack. The dealership verified that the crack was sudden and not caused by negligence or improper maintenance. Inspections by Others: The vehicle has been inspected by the dealership, and all reports are available. I have also submitted the issue to GM corporate for warranty review. No police or insurance representatives have reviewed this, as this was a mechanical failure with no collision. Warning Lamps or Symptoms: No warning lights, messages, or unusual symptoms appeared prior to the failure. The engine appeared to operate normally until the sudden failure. Upon failure, low oil and low propulsion error message appeared on dash. Current no dash lights are on in the vehicle. Assessment of Cause: Based on dealership inspection and documentation, the failure appears sudden and possibly related to a defect in emissions-related engine components. No external damage or user error contributed to the failure.
