Accident Description: 2025 Kenworth T380 Model: 2025 Kenworth T380 VIN: [XXX] Date of Incident: [XXX] Location: Ontario, Canada On [XXX], while operating a 2025 Kenworth T380 on a straight section of highway in Ontario, Canada, the steering system suddenly became unresponsive while traveling at approximately 64 mph. I believe this failure is directly related to the USK tie rod ends, which are associated with two known open recalls concerning USK ball studs. At the time of the incident, no early warning indicators or mechanical symptoms were present to suggest imminent failure. Without warning, the vehicle veered sharply to the left, causing a complete loss of control. The truck subsequently rolled over multiple times, resulting in a serious multi-unit accident that blocked the highway. My drive truck, along with two additional trucks I was transporting, were all involved in the crash. I was entrapped inside the tractor cab and had to be extricated by emergency responders. I sustained significant injuries, including nerve damage and multiple cervical spine injuries, due to the violent nature of the rollover. After the incident, I observed indications that the vehicle may have been tampered with. I am uncertain about the current location of the vehicle or the whereabouts of critical front-end components. However, photographic evidence shows that the ball studs were removed post-accident. It is my belief that Paccar is not fully disclosing accurate information related to this incident and the condition of the steering components. This incident raises serious concerns regarding the vehicle’s safety and the handling of related recall information. When I was driving on the highway the transmission fault error came on and my truck came to a stop on on the highway. It changed gears weirdly and kept the rpm’s high. Once it came to a stop I turned the truck off and on and it work fine. This has happened multiple times and a nationwide failure of the MTM and transmission is the cause of this. Many of my fellow drivers are having this same issues and are having to replace the whole transmission due to the MTM failing and destroying the transmission.

The Eaton 12 speeds that these trucks come equipped with are failing just after the warranty expires. Lots of drivers with the same problem.

Right-hand shift stalk between the shifter and the transmission is causing the vehicle to become disabled while stopped and unable to go from forward to reverse. Also getting SPN 639 pop-up when communication has been lost between the engine control module (ECM) and SAE J1939 data link. The check engine light illuminates, and the ECM immediately logs the fault code. Have to depress brake pedal while stopped & cycle gear selector to get fault to clear to start engine . Message also displayed Transmission Fault Repair Required Find Service Center . Also had new spring clutch replaced in May of 2024. Truck has 525k miles on it .

The clutch Spring on these trucks have been failing heavily. We have a couple more of these trucks and they all have the same issue. the housing for the clutch springs are cracked or damaged at less than 500,000 miles on a heavy duty truck. These clutches are supposed to last over 1MM miles. Its not a coincidence that every truck we have is failing at the same mileage. These clutches are made defective from the manufacturer. Its also a coincidence that the replacement clutches are not the original design and are of a newer design and brand. THIS is RED FLAGS!!!!

Tread of the tire delaminated, similar to what a recap would do. This happened at 43,476 miles +or- the first time. This incident occurred In Anton, Texas. My driver got stopped safely and the tire was still inflated. Moved the truck about a 100 yards to be in safer spot and the tire then went flat. Had tire replaced with Identical tire by Loves of Lubbock, TX. Had driver bring the bad tire in, as had never seen in 18 years in business a tire do this. I have tire in my possession. At 53, 456 miles, right at 10,000 miles the newly replaced tire, did the basic same thing. A 3" piece came out of the center of tire for the complete circumference of the tire. Driver got stopped safely. recovered the piece which was still in one piece. experiencing what we did before, I had him take pictures of the tire with it still inflated and had him check the air pressure with a gauge. Tire was at 130 lbs warm with our normal cold setting being 122-123. This incident occurred near Taft, CA so I learned of Tyack Tires and Had 2 new Michelins put on and had driver bring both tires off the front back along with the center piece. I have tire in my possession. My dealer sent tires and center piece to Goodyear for damage claims, as both times it destroyed the entire fender linings and headlight. Goodyear declined damages stating "Over Deflection", which is chronic underinflation, but that is not the case. I had request in writing for my tires to be returned along with the piece. They shipped tires back without the piece, I requested the piece again and received an email stating there was no piece. My dealer says it was sent to them that it was placed inside the tire that it came off of. I've been doing business with this dealer over 16 years, maybe closer to 18. I have pictures of the tires and piece and all damages. I had the same steer tires replaced on the twin to this truck, VIN ending in [XXX] , with new Michelins as well. I have 3 undamaged tires as well. All can be Inspected. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF IN

Driver seat back is defective and will not support driver properly. The seat back will bend and twist if pressure is applied to it. Putting the driver in an unsafe driving position where the torso is contorted. Four out of four trucks have the same issue with this OE seat. There is no advance warning of when the seat will fail. The issue has been discussed with Kenworth and no resolution has been found as of this time. We have four tractors that are having this same issue, the corresponding VIN numbers are: [XXX] , [XXX] , [XXX] , [XXX] . With the issue occurring on all four of our trucks, we believe this is not an isolated issue and is common with this OE seat. The defect has occurred under light use with no involvement in an accident, which leads to the belief that in an actual emergency it would fail completely.
