The front timing chain cover on the engine was leaking at 53k miles. This is a 2GR-FKS engine. We have had the car for 19k miles, and bought it as a Certified Pre-Owned. We're looking at spending over 10% of the purchase price of the car on a repair, after barely having it for three years.

    - Syracuse , NY

On Juky 7, 2026 @ 10:30pm a catastrophic safety failure occurred involving a 2026 Lexus TX 350. The vehicle experienced a complete electrical and software failure while an occupant (my mother) was inside. All electronic door release buttons failed simultaneously, completely locking the occupant inside the vehicle. The emergency manual double-pull override mechanism on the interior door handles failed to mechanically release or open any of the doors. Concurrently, the climate control system locked on high heat, causing the cabin temperature to rise rapidly. Due to the total entrapment and extreme heat, the occupant began losing consciousness, and her blood pressure spiked to a dangerous level of 188/100. Family members outside the vehicle were unable to open the doors or bypass the electronic locks from the exterior. Emergency services were called to the scene. The local Fire Department had to perform an emergency extraction to rescue the occupant from the vehicle. The occupant required immediate medical attention for severe heat stress and a hypertensive crisis.

2012 Lexus IS 250 Complaint Category: Interior/Seat Belts/Air Bags ? Interior Component: Interior Moldings/Trim/Panels Narrative This is a safety issue. The dashboard on my 2012 Lexus IS 250 has developed significant cracking and is actively decomposing into a gelatin-like texture, despite the vehicle being garage-kept, low mileage, and well-maintained throughout my ownership. This is not the result of any accident, impact, or misuse — it appeared spontaneously and has progressively worsened. The degrading dashboard material is emitting fumes, which are causing a haze to form on the interior surface of the windshield. This directly impairs driver visibility, particularly at night, creating a glare and obstruction hazard while driving. Lexus acknowledged this as a known defect by establishing Customer Support Program ZLZ, which provided extended coverage specifically for dashboard, instrument panel, and interior trim panel cracking on certain Lexus models. The existence of this program confirms that Lexus was aware of a systemic manufacturing or materials defect affecting these vehicles. My vehicle fell outside the ZLZ coverage window by a matter of months. Despite my appeal to Lexus National Headquarters citing my long-term ownership, the vehicle's low mileage, and the timing of the condition's appearance, Lexus has declined to provide any assistance and has stated this is their final position. Supporting Witness Statement [XXX], a window tinting professional, inspected the windshield and dashboard on XXX, at my residence. He observed that the dashboard surface has deteriorated to a gelatin-like, decomposing texture, and assessed that the haze visible on the interior of the windshield is consistent with off-gassing from the degrading dashboard material, rather than from window tint film, cleaning residue, or normal glass wear. (Contact: [XXX]) INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The Component Affected: Electrical System / Drivetrain Propulsion. The Core Hazard: Systemic software/hardware defect causing a total failure of the 12V auxiliary system, resulting in immediate vehicle blackouts and leaving the vehicle stranded without warning. The Safety Risk: The vehicle leaves passengers and young children completely stranded in high-traffic or unsafe environments. Note that this platform is heavily affected by the active June 18, 2026 electrical system shutdown recall (NHTSA Campaign Number: 26V393), and the unresolved 12V parasitic battery loops present an ongoing safety risk of sudden power loss. The Timeline: State that the vehicle has suffered three consecutive electrical dropouts in less than 2,147 miles, with the first failure documented by a dealer battery replacement at just 12 miles.

While driving, my vehicle's check engine light came on and the vehicle entered limp mode without warning. It would not exceed 25 MPH and was stuck in 2WD, displaying the message "reduced engine power – take vehicle to nearest dealer." This created a serious safety hazard, as the vehicle could not maintain safe speed in traffic. The vehicle had to be towed to the dealership. The Lexus dealer diagnosed trouble codes P219C00, P160500, P030027, and P0301300 — cylinder #1 air/fuel ratio imbalance, rough idling, random misfire, and cylinder #1 misfire. Through active injector testing, the technician confirmed the cylinder #1 direct (high-pressure) fuel injector has failed, and recommended replacing all four direct injectors at a quoted cost of approximately $6,600. The dealer's inspection report also cited Lexus service bulletin L-SB-0007-21 for possible worn exhaust valve guides, a separate documented defect in this engine that produces these same misfire codes and limp mode, with a repair (cylinder head and valve replacement) that can exceed $10,000. Lexus has known about these failures in the 8AR-FTS engine since at least March 2021, when it issued L-SB-0007-21 covering 2018–2021 NX 300 vehicles. The bulletin acknowledges affected vehicles enter limp mode. Despite this, Lexus has not issued a recall, and owners whose vehicles fail shortly after the warranty period expires are left paying thousands of dollars for a known defect. My warranty coverage lapsed shortly before this failure occurred, and Lexus corporate declined meaningful assistance. A vehicle that suddenly cannot exceed 25 MPH on public roads is a crash risk to its occupants and surrounding traffic. I am requesting NHTSA investigate this defect for a potential recall.

On 2026-07-05, my car started except the digital instrument panel was blank/black. This meant the digital gas and battery gauges weren't visible, in addition to the speedometer, etc. I tried restarting the car to see if it was a fluke but it did not come back. On 2026-07-06, the car worked fine / as expected, but after a google search, it seems like this happens to other owners as well in the past with no clear reason why. Wanted to report in case it becomes a larger thing.

We were driving down the highway and the vehicle started shaking. I immediately slowed down as I felt we had a flat. There was notification on the screen. I moved to the right onto the shoulder. Only after we were completely stopped did I see a low tire warning come on display. I slowly got out as the traffic was heavy. I could see the outer section of the tire came apart from both sides. As we were in a dangerous location someone called the police. Two police cars pulled behind us and provided us some safety. We agreed to swap out bad tire for spare as we were in a very dangerous location and needed to get vehicle moved before someone ran into all of us. We did look at the highway to ensure no objects were in the road. I didn’t see any while driving, but we wanted to verify to protect other drivers. There also weren’t any pot-holes in highway. We inspected tire after taking it off and there were no screws, nails, objects, or damage to the center tread/sides. The tire seemed to separate fairly cleanly on both sides. Tires were Continental Cross-contact 255/45 R22 107V LX20 (Eco plus) To repeat, no Warning Lights came on vehicle before tire came apart. I have sent message to my insurance company. No one has inspected tire and it is in my possession.
