While driving on the highway, something (likely a small rock) from oncoming traffic flew into the windshield. Upon contact, the windshield cracked and shattered in a manner I have not seen a windshield break before, immediately spraying glass shards across the interior of the vehicle (to include dashboard, seats, and floors) and the driver. From the outside of the windshield, the area of damage is soft to the touch such that it is likely applying any pressure would collapse that area and/or enable you to punch straight through to the other side, so for safety concerns it is undrivable until the glass is replaced. I am concerned the glass could have been defective in some way, as I don't believe this is a typical response to common road hazards like small rocks/other projectiles. The windshield was the original factory installed windshield. The vehicle has not yet been inspected by the manufacturer but the incident has been reported and owner is awaiting contact from the manufacturer regarding inspection. It can be available for inspection until the glass is replaced, but owner will have glass replaced as soon as possible. Additionally, because it is not drivable, it would require inspection at residence or towing. The incident was reported to insurance. There have never been any previous warning lamps/other indicators that the glass was defective.

My vehicle has slowed while driving four times within the last 12 months. I have taken it to the Hendrick Lexus dealership in Charlotte as well as the Scott Clark Toyota dealership in Charlotte. I was told by the Lexus dealership they could not find anything wrong (this was back in July 2025). The Toyota dealership stated (March 2026) that there may be a problem with the fuel pump, but they were unable to confirm this issue and after getting the vehicle back the same thing happened while driving. This has been concerning. It is very dangerous for us to drive the vehicle at this point. I see that there has been a safety recall introduced in the state of Georgia at a Smyrna Lexus dealership, but I have yet to see any other information. I would greatly appreciate an investigation. 3 unresolved safety defects on 2024 Lexus NX 450h+ plug-in hybrid leased May 2023: ELECTRICAL/BATTERY: Rear seat occupant sensor produced continuous false alerts since weeks after lease began, preventing vehicle from locking. High-voltage components including on-board charger located beneath/near the rear seat area per dealer representation at time of sale. Service manager acknowledged liability in writing July 2023. Lexus TSB L-SB-0035-24 addressing battery monitor module failure (DTC P162B87) never applied. Recall 25LD01 covering high-voltage floor wire corrosion unverified for this vehicle. Not charged since approximately April 2025 — over one year — due to fear of battery malfunction, fire, or explosion. Operating exclusively on combustion engine, eliminating primary plug-in feature. Reported in writing for 2 years 8 months. Never repaired. GM confirmed in writing February 2026 defect never submitted to manufacturer. DRIVETRAIN/TIRES: Severe premature tire wear from approximately 2,252 miles. At 7,800 miles dealer declared vehicle unsafe for freeway driving. Owner used alternative transportation for court appearance due to safety warning. Dealer's own alignment printout January 2026 shows multiple out-of-specification values. All 4 tires replaced after 72 consecutive days dealer possession, classified as goodwill, never submitted as warranty claim per GM written admission February 2026. HVAC/BIOHAZARD: Persistent mold/mildew odor from HVAC since early in lease. Service advisor initially attributed cabin odor to rat feces in engine bay entering cabin through air circulation — admitting biohazard exposure to occupants. Rodent nesting caused by manufacturer's soy-based wiring insulation. Rat feces constitute hantavirus and salmonella risk. Same defect across three consecutive vehicles from same dealer. Engine bay sanitized and evaporator cleaned during 72 days, classified as goodwill only, never submitted to manufacturer. Documentation available for inspection.

Vehicle was parked, engine off, and unattended. An internal electrical fire originated in the overhead/sunroof wiring area. Damage includes melted wiring harness near the sunroof, burned center console, and soot throughout the interior. The fire originated in a constant-power electrical circuit and spread downward into the cabin. There were no aftermarket modifications, no external heat sources, and no objects in the vehicle that could have caused ignition. Ambient temperature conditions were normal and insufficient to cause this damage. This represents a serious safety defect because the fire originated spontaneously within the vehicle’s electrical system and could have resulted in a full vehicle fire or injury if occupants were present. The failure appears to involve factory-installed wiring/components and is not consistent with normal wear or external factors.

The contact owns a 2025 Lexus RX Hybrid. The contact stated that while driving approximately 20 MPH, there was an abnormally loud sound coming from the vehicle. The vehicle was parked; however, upon inspecting the exterior of the vehicle, the contact became aware that the panoramic sunroof had unexpectedly fractured. The contact stated that there were no nearby vehicles nor object to impact the sunroof to cause it to fracture. The vehicle was taken to the dealer, where the contact was informed that the panoramic sunroof needed to be replaced. The contact was informed that the panoramic sunroof had fractured from an impact. The vehicle was in the process of being repaired. The contact stated that the failure was a known failure with the vehicle. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 10,200. The VIN was unavailable.

The hatchback window shattered without any known cause. It appeared to have been hit from inside the vehicle but no one was in or around the vehicle when it occurred. It was 82 degrees outside and the vehicle was parked in the shade. Lexus told me there were no known issues regarding the window on other Lexus vehicles.

Car stopped running during turn. Then started creeping along ,then after a bit ,started working normally. Happened a few times after. Then later car wouldn’t start in garage, had to get jumpstart. Took to Lexus dealer and found out the 12 volt battery was shot.they replaced it. Very frightening experience. Could have had an accident.

In heavy rain situations I get an error message stating all front active safety systems (forward collision warning, etc.) do not work. The error states to clean or clear the front sensors of the vehicle. There is nothing blocking the sensors but in heavy weather these situations become much more important. Lexus states this is normal when heavy water situations come into play. I understand for going through flood waters but I don't believe it should be in heavy rain. I feel this is a major safety hazard that should come with warnings.

The contact owns a 2025 Lexus GX 550. The contact stated that while driving at various speeds, the hood of the vehicle was fluttering up and down. The failure was more noticeable while driving 55 MPH or faster. The vehicle was taken to the dealer, who determined that the issue was normal and was known to occur with other 2025 models. The vehicle was not repaired. The manufacturer was informed of the failure, and a case was opened. The failure mileage was approximately 5.

While driving normally one day, all the brake lights on the dash came on and my stopping power was diminished. I pulled the trouble codes and the only code was C1391 which indicates an "Abnormal Leakage in the Accumulator". This is a major safety concern and a costly repair if left to the customer.
