    Dear NHTSA Vehicle Safety Team, I am submitting a safety complaint regarding my 2010 Lexus ES350 (VIN: [XXX] ) concerning a dashboard melting issue. Several years ago, Lexus issued a recall for melting dashboards, including my vehicle. At that time, I did not request a replacement. However, after the recall period ended, my dashboard began to melt. The material becomes sticky, shiny, and unsafe under sunlight. During daytime driving, the melted dashboard reflects sunlight onto the windshield, making it difficult and dangerous to see the road clearly. I have contacted my local Lexus dealer and Lexus corporate headquarters, but both refused assistance, citing the expired recall period. Despite this, the defect continues to exist and presents a serious safety concern. I request that NHTSA record this issue as a safety defect and consider it for further investigation.   • 2010 LEXUS IS

    My car experienced sudden unintended acceleration. I was at a stop light preparing to turn right when my car accelerated of its own accord crashing into the vehicle in front of me. My foot was firmly on the brake, but I had lost control over the vehicle. Whatever electrical defect I experienced, it was sufficient to rev the engine by itself and override the braking system enough that the vehicle advanced forward in an out-of-control manner. It was unintended, unexpected and beyond my ability to control. Failing to recognize the braking system, my vehicle collided into the vehicle a second time. Because my foot was on the brake and my vehicle still advanced forward, I initially thought I had been hit from behind. However, the erratic RPM behavior and the vehicle's continued acceleration while braking confirmed I was no longer in control of the vehicle. Acting promptly, I immediately took the gear shifter out of drive and put into neutral. It was then and only then that I regained control of the vehicle: it stopped accelerating, stopped revving of its own accord and began responding to the braking system. Please note: my foot was not on the gas pedal, for as soon as I took it out of drive, the RPM returned to normal (i.e. the erratic behavior indicated by the tachometer's RPM, stopped). Had my foot been on the gas pedal instead of the brake pedal, the revving would have continued to behave as before; but it did not. (I do not have an all-weather floor mat. It is neither here nor there, because in the event that I did have one, you would expect the RPM behavior to continue when put into neutral; but again, all behavior returned to normal when the vehicle was put in neutral.) Though I am sympathetic to those that panic in a moment of distress, I was of a present enough mind to know what was happening. An electrical engineer with intimate knowledge of the computer system for this vehicle would be the best to assess what went wrong from a technical perspective.

  • 2025 LEXUS TX

    Air Conditioner Condenser failed after only 12,000 miles on a brand new 2025 Lexus TX 350 Vehicle. Leak caused AC to only blow hot air. After having the vehicle serviced, the Service Advisor indicated that the condenser had a leak which was cause by road debris and would not be covered under the warranty or purchased additional extended warranty. The vehicle is exclusively driven on highway and paved roads. Researching the symptoms, many other now frustrated Lexus Owners are complaining about the same issues and lack of Lexus Support. The placement of the condenser behind the wide- open grill, make the condenser prone to failure. Repair cost estimated at over $2,000 for a $70,000 car, less than 6 months old.

    - Atlanta, GA

    The contact owns a 2015 Lexus LS 460. The contact stated that while driving at an undisclosed speed, the ABS, and the Pre-Collision System (PCS) warning lights illuminated. The contact was able to pull over to the shoulder of the roadway, where the failure persisted. The vehicle was taken to the dealer, who diagnosed failures with the brake booster and brake booster pump. The vehicle was not repaired. After investigating the failure, the contact related the failure to Customer Support Program: 24LE01 (Coverage for Brake Booster and Brake Booster Pump Assemblies). The manufacturer was notified of the failure, a case was opened, and the contact was referred to the NHTSA Hotline for assistance. The failure mileage was 96,000.

    Door actuator motor failed and door doesn’t open. Known in forums that it’s a bad design where water from washing or rain leaks and corrodes the actuators leaving failed doors from opening.

  • 2025 LEXUS RZ
    - DENVER, CO

    While operating my 2025 Lexus RZ 300e at normal road speeds, the vehicle experienced a sudden and dangerous propulsion loss when the traction battery reached 0% state of charge. The only advance notice was a single soft audible alert. There were no escalating warnings (visual or audible) and no meaningful low-battery reserve to allow the driver to safely exit traffic or reach a charging location. Within seconds of the single alert, the vehicle entered reduced-power “limp mode,” creating an immediate traffic hazard. This abrupt reduction in speed occurred in active traffic among large trucks, putting my safety and the safety of other road users at serious risk. The lack of progressive alerts and absence of a usable reserve buffer prevented any safe corrective action and increased the risk of a collision or injury. Lexus marketing materials, sales claims, and the owner’s manual indicate that the RZ’s battery management system provides intelligent, progressive warnings and adequate reserve energy to prevent drivers from being stranded. My vehicle did not perform in accordance with these stated safety features. This represents a potential defect in the warning system and energy reserve calibration. The vehicle is currently in the custody of the authorized dealership. I requested that the service department attempt to recreate the incident to determine whether the system functions as designed. They refused, citing an open claim with Lexus corporate. Lexus corporate closed my initial claim without investigation six days after it was filed and has not inspected the vehicle. This failure mode—no escalating alerts, no sufficient reserve, and sudden loss of propulsion—is a clear safety concern that should be investigated for possible noncompliance with FMVSS requirements for driver warnings and vehicle control.

    The vehicle looses power and hesitates to accelerate from a stop start. This happens when The HV Battery State of Charge (SOC) is below 30%. •The vehicle is in catalyst warm-up mode (cold start). The Motor Generator (MG) is in charge mode. As a result of this scenario I have almost been T boned 3 times while attempting a left turn from my home across a dual carriage lane. And again nearly rear ended because of the lack of power or hesitation. To me this is a big safety concern that Lexus do not have a credible fix for.

    common problem with these Lexus ct200h, as well as Toyota Prius V models, are that the brake accumulators begin to leak internally, as they share the same part numbers. Therefore, the actuator will buzz every few seconds trying to keep pressure in the braking system to a safe operating level of gas pressure to no avail due to this leak in the actuator. After it quits buzzing from shutting the car off, you can hear the air leak hissing the pressure away. There are tons and tons of articles on this issue online as this is a major safety concern. Many have had Toyota replace the part free of charge as well due to the extended 10 year, or 150k miles, in whichever comes first. warranty. This may be perhaps because there are several more prius on the road than this rare Lexus CT200H. Just look up “c1391 danger” and see what comes up. In my opinion, as well as many, it is pathetic that an issue in which endangers the lives of others due this brake failure is not further addressed. There is Not much stopping distances when and after this suddenly occurs. In the process, the abs, traction control, brake, a yellow exclamation mark, lights suddenly all come on the dash at once and the brake feels like a sponge very suddenly, as it did me as I was driving today. There were no engine lights as I started my drive. There are no external brake fluid leaks involved for this actuator. Toyota covers it under extended warranty, although it appears as though Lexus does not, this is a major safety hazard and concern. The trouble code is.: C1391. Also, I went to Sewell Lexus of Dallas today and was told there are no recalls, after looking my vin #. The service program had expired and there is nothing they can do for me. Please extend the program or issue a recall for this dangerous failure. The repair cost is substantial and should be covered for a known flaw.

    GOOD DAY GENTLEMEN: THIS REPORT IS CONCERNING A KNOWN, ONGOING AND POTENTIALLY DANGEROUS PROBLEM WITH LEXUS LC500 CARS FROM 2018 UP TO AND INCLUDING 2025 COUPES AND CONVERTIBLES. THE PROBLEM CONCERNS THE DOOR HANDLES/ACTUATORS. APPARENTLY WATER SEEMS TO BE THE CULPRIT. THE MECHANICAL AND ELECTRONIC EQUIPMENT INSIDE BOTH DOORS FAIL AFTER A WHILE INVARIABLY ON ALL THESE CARS DUE TO WATER GETTING INSIDE. LEXUS KNOWS ABOUT THIS ONGOING VERY COMMON PROBLEM AND SUPPOSEDLY IS LOOKING INTO IT. THE REASON I AM NOTIFYING YOU IS GOD FORBID A DRIVER GETS INVOLVED IN A SERIOUS ACCIDENT FIRST RESPONDERS MAY NOT BE ABLE TO EXTRICATE THE OCCUPANTS EASILY VIA THE DOORS. THE DOOR/ACTUATORS FAIL AND OWNERS REPORT THAT THEY CANNOT OPEN THEIR DOORS AT ALL WITH OR WITHOUT THEIR KEY FOBS FROM THE OUTSIDE. FORTUNATELY I HAVE A 2024 COUPE AND THIS HAS NOT HAPPENED TO ME AS OF YET BUT I DO EXPECT IT TO HAPPEN EVENTUALLY BECAUSE I HAVE MULTIPLE INTERACTIONS WITH MANY LC500 OWNERS AND THIS IS REPORTED CONSTANTLY. I DO FEEL THAT THIS A SERIOUS SAFETY CONCERN AND AS A FEDERAL AGENCY I ASK YOU TO PLEASE CHECK INTO THIS TO SEE IF LEXUS CAN TAKE STEPS TO MITIGATE THIS SERIOUS PROBLEM. I WILL say that other than this isolated problem Lexus does in fact manufacture an excellent vehicle and I am very happy with mine. APPARENTLY EVERY VEHICLE DOES HAVE SOME TYPE OF "ACHILLES HEEL" AND THIS DOOR/ACTUATOR PROBLEM IS THE LC500'S. THANK YOU FOR YOUR ATTENTION TO THIS MATTER.
