2023 LEXUS LX- Fredricksburg, VA
The contact owns a 2023 Lexus LS 600. The contact received notifications of NHTSA Campaign Numbers: 25V767000 (ENGINE AND ENGINE COOLING), 26V18000 (SEATS, AIR BAGS); however, the parts to do the recall repairs were not yet available. The local dealer was contacted and confirmed that parts were not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repairs. The manufacturer was made aware of the issue. The contact had not experienced a failure.

2013 LEXUS GX- Idyllwild, CA
This issue began after I brought unrepaired recall concerns to the dealership’s General Manager and was told I would need to sign an NDA and failed to provide the recall part numbers as agreed. When I received my vehicle back, I immediately noticed physical changes to my instrument panel. I then reviewed four years of photos and videos stored on my original devices and discovered approximately twenty date-stamped images showing drill holes, pry marks, tool damage, and components of the instrument cluster visibly moving, changing, or disappearing over time. I am submitting information regarding possible unauthorized access, alteration, and replacement of my vehicle’s instrument cluster by the same Lexus dealership during multiple service visits. My photographic evidence shows the cluster’s condition changing repeatedly while the vehicle was in the dealer’s custody, including unexplained damage, a puncture in the bezel, tool marks, and ultimately a different cluster faceplate. None of these alterations were ever disclosed or documented in any repair order. I can provide a chronological photo record showing each stage of cluster alteration, service records showing no mention of cluster removal or replacement, and a timeline correlating each change with periods when the vehicle was at the dealership. I also have evidence of additional undocumented or suspicious service actions occurring during the same timeframe. Based on the physical evidence and the lack of disclosure, I believe this may involve unauthorized instrument-cluster access and potential tampering. I request that this information be forwarded to the appropriate Department of Justice component for review, consistent with the DOJ’s process for submissions involving potential violations of law.

2020 LEXUS NX- Chesapeake , VA
My car is going into limp mode/ reduced power and I am having misfires on cylinders 3 and 4. I’ve replaced my fuel injectors, spark plugs and catalytic converter. Everything down to the electrical has checked out good. The last issue is the fuel pump. My mechanic says there is not enough fuel going to the injectors . I have a faulty fuel pump.

2024 LEXUS RX- Albuquerque, NM
The sunroof exploded spontaneously while driving on major road at 65 MPH, nothing hit it and no other vehicle around. No abnormal weather conditions. No warning just a burst into air. Remaining glass fell into vehicle when I opened shade.

2011 LEXUS ES- Mount Dora, FL
I AM GETTING A PARASITIC ELECTRICAL DRAW OF 12% OVERNIGHT. ON A NEW INTERSTATE BATTERY. I HAVE CHECKED LIGHTING AND ALLSTATE MILEWISE MONITOR AND NEITHER APPEAR TO BE THE SOURCE. IS THERE A COMMON ISSUE ON A 2011 ES350?

2025 LEXUS TX- Monroe Township, NJ
The electronic throttle control, vehicle speed control, and powertrain/transmission control system failed. When I press the pedal to accelerate there was no power. However I did not lose power to the vehicle as I can see the vehicle was in Drive mode but was not moving forward. I was on a decline hill and the decline helped me go downwards before a truck could hit me. This total loss of acceleration has occurred exactly 2 times within a 4-week period. The vehicle is currently located at the Lexus dealership service center and is available for immediate inspection upon request. My wife and small children were in the vehicle when the accelerator pedal went completely dead while driving. The sudden drop in speed in active traffic almost caused us to be catastrophically rear-ended by an 18-wheeler semi-truck. After one minute of zero throttle, the vehicle violently jerked, locked us into a highly dangerous 5 MPH limp mode on the road, jerked a second time, and then suddenly resumed normal speed. My family's lives were put at extreme risk. No. The vehicle is currently at the authorized Lexus dealership for inspection, but the service department states they cannot find any diagnostic trouble codes (DTCs) and have not yet reproduced the intermittent failure on a test drive. The vehicle is currently under inspection by the authorized Lexus dealership service department. It has not yet been inspected by the manufacturer's regional corporate specialists, police, or insurance. No. There were absolutely no warning lamps, check engine lights, dashboard messages, or symptoms prior to either of the two failures. The loss of acceleration happened instantly without warning. The vehicle's computer system failed to store any error codes despite the severe mechanical jerking and the sudden 5 MPH speed restriction.

2018 LEXUS GX- Manorville, NY
The back up camera would go black when reversing, creating a dangerous situation. It’s a known issue with the GX 460 model that when the radio gets corrupted and the infotainment screen goes black it will cause the back up camera to not function properly! I reached out to Lexus and they confirmed the issue is a bad logic (mother ) board that causes this and since it’s out of warranty they will not honor the repair or replacement of this highly dangerous situation. Your assistance is much appreciated!

2018 LEXUS NX- Loganville, GA
Foam is breaking off of the shell. Reached out to Chicco and issue was not resolved nor replaced. This seems to be a reoccurring problem with this car seat. For it to have an 8 year lifespan, it hardly lasts 2 years before Chicco deems it unsafe and needs replaced on my dime. Highly frustrated about this product.

2012 LEXUS ES- Ballwin, MO
The contact owned a 2012 Lexus ES 350. The contact stated that while driving 40 MPH, the vehicle was rear-ended by another vehicle, causing the vehicle to rear-end the vehicle in front, resulting in a three-vehicle crash. The impact caused the hood to unlatch, and the hood flew up. The air bags did not deploy. The contact's grandson was a passenger in a rear-facing car seat, and the contact was unable to determine whether the child had sustained any injuries. The contact suffered a sore neck and body, and medical attention was received. A Police report was filed. The vehicle was towed to a body shop and declared a total loss by the insurance company. The manufacturer was made aware of the failure and requested that the vehicle be retrieved to access the Event Data Recorder (EDR). The failure mileage was approximately 82,000.

2016 LEXUS RX- San Diego, CA
I am reporting a safety issue with a 2016 Lexus RX350, VIN [XXX], with about 60,000 miles. The vehicle is registered in [XXX] and was being driven to [XXX] when it suddenly overheated on XXX near [XXX]. The warning lights appeared while the vehicle was traveling at highway speed. The driver immediately took the first available exit within about one minute. The failed system appears to be the engine cooling system, specifically the water pump. The vehicle is currently at Kendall Lexus of Eugene and should be available for inspection. This failure created a safety risk because it happened suddenly on a major highway. The driver had to exit quickly and the vehicle became disabled far from home. A sudden coolant-loss or overheating event at highway speed can leave drivers stranded in unsafe locations with very little warning. Kendall Lexus of Eugene inspected the vehicle. Their Service Manager stated in writing that the technician found a leaking water pump, misfires in every cylinder, and a 95% leak in cylinder #6 through the exhaust valves. She stated that the water pump failure caused the vehicle to lose coolant, overheat, and suffer internal engine damage requiring engine replacement. Lexus Customer Relations has been notified under Case #XXX. Whether Lexus corporate has physically inspected the vehicle is unknown. Before the incident, there were no known warning signs. The warning lights appeared five hours into the road trip when the overheating occurred. After exiting, the driver let the vehicle cool for 30 minutes, checked for visible leaks, and tried to continue only briefly around the block. The warning returned almost immediately, so the vehicle was towed. This vehicle has extensive Lexus dealership maintenance history. Lexus quoted about $20,000 for engine replacement. Lexus has issued TSB L-SB-0039-20 / NHTSA ID 10183269 regarding water pump leak inspection and diagnosis, but there appears to be no rec INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
