    The contact owns a 2021 Lexus RX350. The contact received notification of NHTSA Campaign Number: 23V865000 (Air Bags); however, the part to do the recall repair was unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue. The contact had not experienced a failure.     The seatbelt install will not stay tight, the seatbelt slips easily out of the lockoff. This happens in all seating positions in this vehicle but does not happen in our other vehicles.

  • 2024 LEXUS RX

    While driving on a local highway recently, my forearm brushed the automatic shift knob while reaching to place my phone on the wireless charging pad. The transmission shifted into neutral, resulting in an unexpected loss of speed. It took me a few seconds to realize what had happened - I shifted back into Drive, and resumed my trip. No accident resulted from this, but it has happened twice in the month that I have owned the car. I reported the issue to the dealership thinking my car had a problem - they said that this is the way the car is designed to function. I found a section in the Owner's Manual which confirms this - shifting from Drive to Neutral while stationary requires the knob to be held to the left for a couple seconds; for some reason, doing this while moving requires only a tap (see attached photo of the owner's manual). This seems like a design issue - the unexpected loss of power from bumping the shift knob to the left (no forward or backward motion is needed) could easily result in an accident. I don't understand why they specifically created a system where a "push and hold" is not required to shift into neutral when the vehicle is moving. I reported this issue to corporate Lexus today. They said they will consider this feedback in subsequent model designs, but I wonder whether a recall is appropriate. That's why I'm reporting the issue to you, as well.

  • 2019 LEXUS NX

    I have attempted informing Lexus of this situation. To my knowledge, they have not pursued the case. On October 31, 2023, we brought our NX300 [XXX] ) in at 44,806 miles. The left parking brake actuator was inoperative (also rendering the cruise control inoperative). To get to the dealership from Ormond Beach, I removed the appropriate fuses to avoid any undesired operation from the unit. I was hoping any problems would be covered by warranty—it had under 50,000 miles. The Basic Warranty had expired. The Powertrain Warranty was still in effect. Strangely, brakes are not listed in either warranty. This was definitely NOT a wear and tear item. After inspection by Lexus mechanic, the failed actuator would not and could not be covered by warranty. The ‘out-the-door’ cost was $1371.43 (RO# [XXX]). I asked for the old part, which I still have. There is an extreme manufacturing defect I found. Lexus charged $997.63 for the part. The same EXACT part is available online for $37.70---OEM is $189.00. I understand price markup is required for any business, but over 500% may be a bit excessive. I was truly shocked by what I had found within the sealed actuator. The why and how the actuator actually failed is blatantly obvious and could be a serious safety hazard for anyone driving a car with this type actuator that was manufactured in and around the same time as this one. The motor within the unit is held in place by 3 torx head screws which (all three) were completely unscrewed and were rattling above their appropriate threaded hole. Each screw has little or no threadlocker (Loctite) on the threads. The compression ring on top of each threaded hole is not compressed. This indicates the screws were never torqued to the proper specifications during manufacture. I believe there is a possibility of a brake malfunction due to manufacturer's defect in the brake actuator part.   • 2024 LEXUS TX
    - Canton, MS

    The contact owned a 2024 Lexus TX350. The contact stated while driving 15-20 MPH from a complete stop and making a left turn, another vehicle who was driving at an undisclosed speed, crashed into the front passenger's side end of the vehicle, causing the front end of the vehicle to separate. The contact shifted into park(P), and the vehicle stopped on the road. The other vehicle continued to accelerate and veered off into a ditch. The contact did not recall whether there were warning lights illuminated after the incident. The contact stated that several messages indicating the brake system, the Advance Autosonic detection, the Intelligence alert, the ABS, the parking brake, the electric power steering, the electronic stability control systems malfunctions, and other malfunctions were displayed on the vehicle mobile app. The contact stated that the front driver's side air bag and the knee air bag failed to deploy as intended and partially deployed. The contact sustained whiplash and hit her knees against the dashboard during the incident. The contact sustained neck, hips and legs pain. The ambulance transported the contact to the hospital, where a CT scan and X-Rays were performed, and the contact was informed that an internal neck injury was found. Additionally, the contact's legs had become swollen between the knees and the ankles. The contact stated that she was in the process of receiving further medical assistance. A police report was filed but the contact did not have the police report information available. The vehicle was towed and totaled by the insurance company. The dealer was notified of the failure and referred the contact to the manufacturer for assistance. The manufacturer was notified of the failure and a case was opened. The manufacturer referred the contact to the NHTSA Hotline for further assistance. The failure mileage was approximately 10,750.

  • 2021 LEXUS RX
    - NEWARK, DE

    Hit a deer that impacted the passenger side of the vehicle. Passenger seat was occupied, and door had to be pried open by tow truck driver to get passenger out. Entire right side, from the "L" logo to the front fender was destroyed. The passenger airbag did not deploy, nor did emergency braking or collision avoidance warning go off as deer was crossing from the left (driver) side to passenger side where it impacted. No injuries, but car had to be towed due to front damage. Lexus has no fix for the airbag sensor recall for almost a year now.

  • 2011 LEXUS ES

    I purchased the car on Feb 9, 2014 at the above Lexus Dealership. $26,596.   My dashboard is melting and it’s definitely a safety hazard. When materials used in the dashboard are prone to melting or degrading in extreme heat, potentially obstructing the driver's view and impacting visibility.  At times I couldn't see and I almost had an accident when the sun reflected in my eyes. It’s a defect that Lexus is aware of because they offered to fix it under the Customer Support Program (Takata Economic Loss Class Action Settlement).   Furthermore, by way of legal agreement, the ZLZ warranty enhancement coverage extends to my vehicle until January 8, 2026.  However, I should not have to rely on this legal agreement to get Lexus to take care of this repair which has been determined as a defective product/material.   I contacted Lexus HQ 716-941-2295 on November 21, 2024. Lexus said they would not be responsible.   The law recognizes concepts of negligence and strict liability to hold sellers liable for defective goods even when no contract existed.   Furthermore, regarding this dashboard defect, there is liability based on the breach of a warranty for the sale of this defective product. Regarding negligence and strict liability, the law has expanded the scope of warranty protections for consumers. This is true both of liability for express promises made by the sellers regarding the quality of products, and the protections the law presumes when sellers offer goods for sale. Lexus knows that when a seller makes a representation at the time of sale regarding the quality of the product, that product is said to be under warranty. Lexus should not continue to avoid responsibility using the excuse of time limitations on the warranty item.  Lexus should authorize the repair.

  • 2020 LEXUS ES

    The contact owns a 2020 Lexus ES 350. The contact stated that while his father was driving at 35-37 MPH down a hill, he applied the brake pedal, but the brakes failed to respond. The contact continued to pump the brake pedal. The driver moved the gear shifter, attempting to slow down the vehicle. The driver crashed into a 2016 Mazda 3, which crashed into a 2017 Honda Civic. There was an unknown brake indicator light displayed. The passenger occupant sustained extreme chest and neck pain. The driver and passenger were provided medical attention. Additionally, the two occupants of the Mazda sustained leg and neck pain but no medical attention was provided. The unknown occupant (s) of the Honda sustained back, feet, neck, and head injuries, and medical attention was provided. The passenger occupant confirmed that the driver had pumped the brake pedal. The vehicle was towed to a lot. The air bags deployed. There was no reported fire. The contact tested the brakes and showed resistance. The contact had an independent mechanic test the vehicle, and the OBD reading showed no results. There was an indication of a low battery. A police report was filed. The police tested the vehicle and noted the brakes were functioning. The contact stated that a witness traveling behind the driver did not see any brake lights illuminate. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 30,000.

  • 2021 LEXUS RX

    Component or System That Failed: The rear passenger seatbelt on the right side of a 2021 Lexus RX 450h malfunctioned during a collision. It is still in the vehicle, which is available for inspection upon request. How Safety Was Put at Risk: The seatbelt failed to restrain my son, who was seated in a booster seat and weighs approximately 78 lbs. During the collision, he flew forward and struck the front passenger seat and center console with his chest and upper abdomen. This malfunction could have caused severe injuries, and while his injuries appear minor, it posed a significant risk to his safety. Problem Reproduced or Confirmed: The seatbelt’s failure has not been reproduced or confirmed by a dealer or independent service center. However, the seatbelt remains fully extended and does not retract, indicating a potential defect. Inspections Conducted: The issue has not yet been inspected by the manufacturer or a service center. Police were present at the accident scene but did not specifically inspect the seatbelt. Warning Lamps or Messages: No warning lamps, messages, or other symptoms indicated a problem with the seatbelt prior to the collision. Best Assessment: Based on observations and my son’s account of the incident, it appears the seatbelt retractor mechanism failed to lock during the collision, which is a critical safety feature. I am submitting this report to ensure the potential defect is investigated to prevent similar occurrences in other vehicles.
