    The contact rented a 2024 Mazda CX-50. The contact stated that while driving on a main street at 5 MPH, the contact attempted to slow down the vehicle by depressing the brake pedal, and in the process, the brakes locked and the vehicle proceeded to accelerate without the contact's consent. The contact was involved in a front-end collision with the vehicle in front of them. No property damage was reported. The vehicle came to a stop upon impact. The vehicle was towed back to the Enterprise rental center, where the vehicle was originally rented. The airbags did not deploy. No warning lights were illuminated. A police report was not filed. The vehicle was not destroyed. Three people were reported to be injured: the contact seated in the driver's seat, the contact's coworker seated in the front passenger seat, and the contact's wife seated in the rear passenger seat. No injuries were reported outside of the contact's vehicle. The contact sustained injuries to his shoulders and lower back, and the contact's wife sustained injuries to her neck and back; the contact's coworker's injuries were unknown. Medical treatment was not needed for the injuries. The local dealer was not contacted, and the vehicle was not diagnosed. The vehicle was not repaired. The manufacturer was not notified of the failure. The approximate mileage at failure was 26,839.

    The contact owns a 2024 Mazda CX-50. The contact stated that while opening the doors, the door stop detent had failed, causing the door to rapidly close and swing back, and making contact with the occupants. Both the contact and the wife sustained leg bruises, and the wife sustained a head contusion due to the failure. No medical treatment was required. The contact indicated that the failure was present on both the driver's side front and rear doors and the passenger side front and rear doors. The cause of the failure was not determined. The vehicle was taken to the local dealer, who examined the vehicle, and no failure was found. The contact was informed that the door operated as intended. The manufacturer was notified of the failure, but no assistance was offered. The failure mileage was 100.

    Straight to the point, I backed the vehicle up, pushed the brake peddle in and shifted into park. The engine begun to increase in RPMs and I could hear the engine load increase, maybe about 2,000 to 5,000 rpms higher than idle or first start building vacuum idle, as if I was pushing on the accelerator. I literally looked down at the peddles, as I was just doing a quick spot change in my driveway, the accelerator did not appear to be stuck or in contact with the aftermarket winter weather plastic floor mat. I decompressed the brake peddle again and shifted the car from P to D and the engine went to its normal baseline idling RPMs and sound. I put it back in P and turned it off, no issue. I have noticed this problem before, but I only noticed it during this past winter season and blamed it on my winter boots and/or myself instead of the vehicle. I am going to guess this has happened in total about 4 times, including the most recent incident that I just wrote about. The CX-5 is connected to Mazda's network/ data service or whatever it is called, because I like my cars clock to maintain correct time, which it won't or can't do if I prefer my privacy from Mazda. So Mazda should be able to see what happened on their end. It happened in the late afternoon to early evening. I do also use 'auto hold' daily and cruise control w/out radar active.

    When filling up with gas, the gas filler tube would overflow spilling gas out of the filler tube. This happened multiple times. The dealer replicated the issue and found that the back flow sensor, valve and back flow door in the gas tank were broken and faulty. They replaced the gas tank with all of the above components replaced as part of the gas tank.

    My daughter was driving on the freeway at 75-80mph on the morning of April 19th, 2025. The rear passenger tire blew out without any prior symptoms or warnings. Surrounding cars slowed down, or got out of the way because the other drivers saw her struggles to maintain her lane and direct the car to the shoulder of the freeway. AAA was called to change the tire to the safety spare. The tire remains with us, and the NHTSA is the initial entity that I am filing a report to.

A new crack has occurred on [XXX], with cracks consistently radiating from the driver's A-pillar side. This pattern suggests stress fractures rather than impact damage, as initially assumed. The vehicle was taken to a Mazda dealer in Seekonk, MA, but the Mazda Service Manager denied warranty coverage without an in-person inspection, relying solely on photos taken by a trainee service advisor. These images clearly show a recurring pattern of cracks in the same area, which is concerning. Research into Mazda forums reveals numerous reports from other owners experiencing similar windshield failures, including unexpected cracking, chipping, waviness, visibility issues, and delamination. Many report quick denials of warranty assistance, similar to this case. The frequency and similarity of these reports suggest a potentially systemic issue that may be more widespread than the recent hitch recall. The manufacturer's reluctance to investigate raises serious concerns about windshield safety. After three cracks and two replacements, Mazda has not physically inspected the issue. This situation warrants a thorough investigation by NHTSA, including major glass providers and installers, to ensure occupant safety and address potential systemic failures. The lack of proper reporting and inspection by Mazda underscores the need for NHTSA intervention to protect consumers and ensure that safety standards are met. Furthermore, the consistent denial of warranty claims without adequate inspection may indicate a broader problem in how these issues are handled, potentially affecting many more vehicles than reported. This pattern of failures and lack of action by Mazda necessitates a comprehensive review to safeguard public safety.     Collision Prevention System is dangerous. I was nearly rear ended going 75 mph on the PA Turnpike. I was passing a truck that was in the right hand lane when all of a sudden the brakes automatically engaged hard and I nearly was rear ended by another vehicle that was right behind me. It braked so hard it made the tires squeal. I contacted two dealerships and told them what happened and both told me there haven't been any problems with the system that they were aware of and maybe a software update would help. Neither dealership checked if a update in fact was available. I turned the system off and will never turn it back on because it is so dangerous!

    Fuel issues P0460 fuel level sensor circuit. Engine malfunction light comes on with fuel sensor issue. The car is full of fuel or not even close to be empty but the sensor reads off triggering a chain of event. Killing car mid of the highway when it got enough fuel. Average full tank ranges 280mi, this instance it only drove 202mi and the sensor reads 14mi to go with quarter Guage left in the tank. Then the needles drops by a notch. I can't upload the video but attached at the screenshot. Contacted dealer and they won't diagnos till 90days out. This is safety hazard and the car comes to halt in the mid of the drive. Where it's highway, parking lot, or anywhere; as if it's out of gas but the tank still good. Wait for few minutes and restart the car the garage read normal. The dealer doesnt want to listen or even consider this as safety issue. Multiple dealer reject to even make an appointment and request that I go to the dealer I purchased the car from.
