2016 MAZDA CX-5- Novato, CA
The contact owns a 2016 Mazda Mazda CX5. The contact stated that on two separate occasions, the vehicle unintentionally accelerated without warning in an attempt to park the vehicle. On the first occasion, the vehicle drove in between two trees with the contact's foot on the brake pedal before the vehicle came to a stop. The air bags did not deploy upon impact and no injuries were reported. The vehicle was towed to an independent body shop where the vehicle was repaired for minor damage. The contact was also informed that no computer codes indicated any signs of a mechanical failure. On the second occasion, the vehicle crashed into a parked vehicle while again attempting to park the vehicle in a parking lot. The force of the collision caused the already hit vehicle to crash into yet another parked vehicle. The air bags did not deploy upon impact and no injuries were reported. A police report was filed. The vehicle was towed back to the same independent mechanic where the code reader indicated that the vehicle had a low battery; however, the battery did not need to be replaced. The dealer nor the manufacturer had yet to be notified of the failure. The vehicle remained in the possession of the independent mechanic and had yet to be repaired. The failure mileage was approximately 70,000

2016 MAZDA CX-5- Renton, WA
In September of 2023, while driving I attempted to use the windshield wipers, and they would not function. We live in a rainy part of the country, so having non working wipers is a major safety issue. Research for possible causes pointed to the body control module which had been recalled on Mazda 3s in recent years. I took the car to the dealership, and they traced the problem to an internal failure of the body control module. I do not have the component to inspect, and I do not know what the dealership did with it after removal. We filed a complaint with Mazda in October of 2023. They gave us case #001-463-33. They were not receptive to feedback, and simply stated many times that there was not a current recall for our vehicle. There were no warning signs/messages/ lights before the wipers stopped working, and they had worked normally the day before.

2016 MAZDA CX-5- Grand Ledge, MI
First symptom, no heat coming out of heater vent took to shop to investigate and found antifreeze in the oil . Cylinder head removed and found a nonreparable crack In the cylinder head. I'll maintenance up todaye, Engine never overheated ,no symptoms, just suddenly Blowing cold air from the heat vent. $7300 estimate 2 fix.

Headlights are exceptionally dim, especially low beams. Recall for other models within same year not considered valid for this particular car. Ongoing but has gotten worse.

Part 573 Safety Recall Report 20V-063 Manufacturer Name : Mazda North American Operations Submission Date : FEB 06, 2020 NHTSA Recall No. : 20V-063 Manufacturer Recall No. : 4320A [XXX] My vehicle was impacted by this yet I never received notice from Mazda and when asking the dealership and Mazda USA to honor the repair I was told it was not covered. Mazda has also issues two TSBs regarding the issue, I inquired while the vehicle was under warranty but they said the issue was not covered. TSB [XXX] and TSB [XXX]. 2016 MAZDA CX-5- BETHESDA, MD
11/19/2023 - About 8 pm. on November 17, 2023, I was parking at my condo parking lot and slowly drove the car into the parking space when all of a sudden the car accelerated on its own. I immediately pressed on the brake pedal and it stopped. I felt fortunate that nobody was on the sidewalk. The same thing happened about a month ago but I thought it was just something unusual but now that it happened again I am very concerned and I am going to call the Maxda dealership to see what they say.

The contact owns a 2016 Mazda CX-5. The contact stated while driving 25 MPH, the driver’s side headlight became inoperable. No warning light was illuminated. The vehicle had not been diagnosed or repaired. The manufacturer was made aware of the failure and determined that the VIN was not included in a recall. The failure mileage was approximately 62,000.

2nd time in 40k miles right rear caliper locked up mechanic suggest getting both left and right replaced figuring if one failed its only a matter of time, calipers should last 10 years and 2nd replacement in 4 years, this is a bad safety issue

The contact stated she was able to drive the vehicle to a safe area before the vehicle lost motive power. The check engine warning light was illuminated. The vehicle was towed to an independent mechanic, where it was diagnosed with an inoperable fuel pump. The contact was informed that the fuel pump needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 66,000.

What component or system failed or malfunctioned, and is it available for inspection upon request? The daytime running light (DRL) failed and stopped working. It is available for inspection upon request. How was your safety or the safety of others put at risk? Other cars won't be able to see my car during the daytime. Has the problem been reproduced or confirmed by a dealer or independent service center? There is a Mazda reference number for the recall is 4320A. There is also a NHTSA campaign number 20V063. NHTSA 20V063: [XXX] Mazda 4320A: [XXX] Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No, we can see the daytime running lights not working Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, there were no warnings, messages, or other symptoms prior to the failure. It happened a few weeks ago. I have called Mazda and they tell me it is not part of recall. My VIN number is in the range of the VIN numbers affected. Mazda refused to fix this recall issue because my VIN number is not associated with this recall.
