2024 MAZDA CX-90 PLUG-IN HYBRID- Omaha, NE
The seatbelt reminder system is broken and someone almost took my door off this morning because I had to open my door to get it to stop. Every day when I drop my kids off at school I have to make sure the vehicle is in park so the seatbelt reminder doesn't go off. If they open it a second before I put the vehicle in park it will keep beeping and beeping until I turn the car off, open my door, and turn the car on. I had to do that today and at a stop sign. It's busy by the school so everyone is in a hurry. So it was beeping non stop because my son unbuckled early and the car will keep beeping even if noone is in the seat. I stopped at the stop sign, turned off the car (and it still kept beeping), opened my door to get it to stop, It was at this point the person behind me thought my car was disabled and went around me, almost taking out my door and possibly my arm at the moment. I also have to do this when someone in the back seat moves seats (and we have to put a seatbelt on an empty seat) or even puts some pressure on a seat. If they sit inbetween seats we then have to put a seatbelt on an empty seat. Dealer says this is the way its designed.

While vehicle was in motion, warning light came on stating “engine malfunction, have the vehicle inspected”. Engine light stays illuminated. The message and light now appear at startup

2024 MAZDA CX-90 PLUG-IN HYBRID- East Stroudsburg, PA
At approximately 16:50 engine malfunction warning light appeared and dealership was notified. Appointment scheduled for 4 days after. At 17:21 car began swerving uncontrollably along I80 with warning “engine malfunction, have the vehicle inspected.” The car was shaking violently and veered off the road into the ditch between the eastbound and westbound highway. The car was totaled. Review of the vehicle app indicated that just before the crash at 17:19 several critical vehicle malfunctions messages appeared. The vehicle crashed at 17:21.

This is a 2024 Mazda CX-90 PEHV where I took it in for all recalls on January 17th, 2025 related to loss of power and hybrid system malfunction. Today, Jan 31st, 2024 (2 weeks and a few 100 miles later), I now have engine malfunction warnings lights. This car has serious issues that nearly caused a bad accident where POST recall, the issues continue

2024 MAZDA CX-90 PLUG-IN HYBRID- Eden, UT
I am submitted this report for the 12/6/2024 event. The form I submitted previously was for the safety incident that occured last night (1/9/2025). On four different occasions, the most recent being last night (1/9/2025), the vehicle has suddenly lost drive power while driving. The ability to accelerate is suddenly gone. When you press the gas pedal the engine revs, as if it is in neutral, but it does not accelerate. When this happens, the vehicle is still in drive as indicated by light on the shifter showing drive and the fact the vehicle, just moments before, is driving. During all four instances, I had to bring the vehicle to a complete, put the car in park and then back into drive. On three of these occasions there was no pull-out lane and I had to bring the vehicle to a complete stop in a moving lane of traffic. The incident last night occurred mere seconds after making a left-hand turn with traffic coming up behind me at a higher speed then I was travelling. My safety, the safety all passengers in my car and the safety of other drivers are all at risk when this occurs. When this happens there are no warning lamps or messages that appear. After the third time it happened (on 12/6/2024), we took the vehicle to the Mazda dealership we leased it from and it was inspected and we were told they found nothing wrong with it. They noted they did some updates. They kept the vehicle from Monday 12/9 - Thursday 12/12. Less than a month after getting the vehicle back from the dealership this issue occurred a 4th time (on 1/9/2025 at about 8pm). We will be calling the dealership this morning and setting up an appointment for them to again inspect the vehicle for this safety concern. The vehicle is available for inspection.

1. Gear Shift Issue in EV Mode: The transition from 1st to 2nd gear is not shifting properly in EV mode. At times, the gear jerks during critical situations, which compromises control. 2. Power Output on Highway: While driving on the highway, there are instances where there is no power output when flooring the accelerator to overtake. 3. Engine Malfunction Light: The engine malfunction light has come on and off three times. 4. Radar Malfunction: The radar system is intermittently not working or working improperly. The radar malfunction light also came on and off once (i-ACTIVSENSE system malfunction). The last safety recall was completed a few weeks ago, but the issues are still ongoing. Since then, I have visited the Mazda dealership twice, but they let me go, dismissing my claims and saying they couldn’t duplicate the problems. I have a case with Mazda corporation. Nothing done yet (Case Number: [XXX]). The name of the Representative is Candy. I would appreciate it if you could investigate these issues at the earliest convenience. The contact owns a 2024 Mazda CX-90. The contact stated while driving at approximately 3 MPH, the vehicle unintendedly accelerated; however, the accelerator pedal was not being depressed during the failure. No warning light was illuminated. The vehicle was taken to a dealer and was diagnosed that a software update was needed. The vehicle was repaired; however, the failure persisted. The manufacturer was notified of the failure. The failure mileage was approximately 3,000.

steering wheel locks into the direction of turning (sometimes more than needed for the corner) and causes over-steering at all speed (from 20mph to 45mph). at low speed the steering can be forced to turn back, but at higher speed such as onramp or offramp, after fighting the oversteering, the steering disengages for a brief second while fighting oversteering which I think can/will cause inexperience driver to turn the steering wheel too much in the opposite direction. also the lane departure/centering feature also causes the steering wheel to jiggle while driving straight (incorrectly trying to move the car while going stright). this is going to cause accident.

with lane-keep assist /lane-centering enabled, the steering will try to straighten the car while turning. experienced this during on-ramp and off-ramp of highway. this is super dangerous. even experienced it at low speed in the neighborhood. steering wheel will fight me during turn and disengage for brief second - this is very disconcerting while making a turn. even with lane-keep assist disabled, I can still feel the wheel (very minor feedback) trying to change course while turning. this issue began immediately after dealer performed all active recalls.
