2020 MAZDA CX-5- Los Angeles, CA
-Passenger side air bags deployed for no reason on May 19, 2025 at 1945h. -Driver safety compromised due to shock, momentary loss of hearing, disorientation while driving. -Other drivers safety compromised due to driver having to react, re-orient themselves and drive to safety. -Culver City Mazda dealership ran diagnostic. Conclusion: air bags deployed due to pressure. NO OTHER conclusions regarding system error, software error, ignitor, sensors or vehicle impact. -Police inspected vehicle at time of incident. No damage was observed or reported. -NO warning lamps or messages appeared prior to passenger air bag deployment.

2014 MAZDA MAZDA6- Fairfield, CA
The car will not go into Park right away. As previously reported, I have to start the car multiple times to get it into Park--can take up to 10 plus minutes before the car will go into Parkand shut off--only then I can lock the car! Very frustrating since it's a known issue and still has not been recalled

2019 MAZDA MAZDA3- IRAPUATO, NH
The seat belt stopped retracting and no longer works, it was reported directly to the Mazda agency, but the warranty was not valid. The dealer said that the tensioners no longer work and the seat belt needs to be replaced.

2025 MAZDA CX-70- Riverview, FL
Vehicle has been squealing countlessly when braking about 1 to 2 days after leaving the dealership in September 2024 up until current. The vehicle has been out of commission for more than 15 days while attempts to repair the issue at local Mazda dealerships. On the last attempt to correct the issue, Ferman Mazda dealership in Brandon FL performed some sort of shaving of brake component to ensure this would not continue; however, the noise continues and this also may increase the safety risk now.

2019 MAZDA CX-5- Lathrop, CA
I’m reaching out regarding a persistent issue with my Mazda 2019 cx5 infotainment system. The screen is experiencing frequent “ghost touch” or phantom button presses, where functions are activated without any input. This makes the screen erratic and often unusable, creating a distraction and raising safety concerns while driving. I’ve already restarted the system and attempted basic troubleshooting, but the issue continues. I’ve learned that this is a known issue in various Mazda models, and I believe this should be covered for repair, especially since it impacts both functionality and driver focus. I’ve already contacted Mazda customer service and my local dealership; however, they advised that the repair would need to be paid out of pocket. Given the nature of the issue and the potential safety concerns it presents, I strongly believe this is something that should be covered by Mazda. Please let me know how I can escalate this issue for formal documentation and create a new recall. 2020 MAZDA CX-30- Ellenwood, GA
I am reporting a repeated and potentially dangerous transmission failure in my 2020 Mazda CX30. At 6 months old, the original transmission failed. Mazda confirmed it was defective and replaced it with a refurbished unit with 10,000 miles on it. That replacement is now also failing—within 4 years of the first failure. This means the car has experienced two transmission failures before reaching 5 years of service. This raises concerns about: •The reliability of Mazda’s factory parts •The quality and safety of refurbished transmissions Mazda uses in warranty repairs •A potential pattern of premature transmission failures on this model Repeated transmission failure can cause loss of power and control, posing serious safety risks to drivers and passengers. Mazda has all service records confirming both incidents. I am submitting this to alert NHTSA to a possible defect pattern that may warrant investigation.

2024 MAZDA CX-50- Spokane, WA
The contact owns a 2024 Mazda CX-50. The contact stated that while opening the doors, the door stop detent had failed, causing the door to rapidly close and swing back, and making contact with the occupants. Both the contact and the wife sustained leg bruises, and the wife sustained a head contusion due to the failure. No medical treatment was required. The contact indicated that the failure was present on both the driver's side front and rear doors and the passenger side front and rear doors. The cause of the failure was not determined. The vehicle was taken to the local dealer, who examined the vehicle, and no failure was found. The contact was informed that the door operated as intended. The manufacturer was notified of the failure, but no assistance was offered. The failure mileage was 100.

2019 MAZDA CX-5- Peckville, PA
The contact owns a 2019 Mazda CX-5. The contact stated that while operating the vehicle, the message that the engine oil level was low was displayed. The contact stated that oil was added to the engine; however, the next day the message reoccurred. The contact stated that later while the vehicle was parked, the contact discovered an oil leak underneath the vehicle. The vehicle was driven to the local mechanic who confirmed oil leaking from the engine, but the cause of the oil leak was not yet determined. The local dealer was contacted, and a service appointment was scheduled. The cause of the failure was not yet determined, and the vehicle was not yet repaired. The manufacturer was not contacted about the failure. The failure mileage was 96,600.

2023 MAZDA CX-9- Spokane, WA
Starting [XXX], through [XXX], we began receiving multiple "Critical Vehicle Malfunction" alerts (46 alert messages)via the Mazda app, including "Parking Brake Malfunction" and "Electric Parking Brake Malfunction." During this time, the vehicle experienced delayed starts and eventually required a jump-start. We brought it to the dealership multiple times to show the messages, before our 3-year/36,000-mile B2B warranty expired, but the issues were dismissed until the battery was finally replaced. At just over 36,000 miles (currently 39,646), a new issue emerged a “thud” sound from the steering wheel. The dealer diagnosed it as a failed u-joint shaft, which was deemed non-serviceable and not covered under warranty. I inquired about potential causes or prevention—no clear answer was known. I found relevant NHTSA TSBs (06-001/20 & 06-001/24) related to steering issues and escalated to Mazda USA. Mazda agreed to cover 50% as a goodwill gesture, leaving me to pay about $500. Two weeks later, the part arrived and was installed—but it did not fix the issue. The dealership now says it’s a failed bearing within the Electric Power Steering (EPS) unit, which requires complete replacement at a cost of over $2,000. Only one part is available in the U.S., with another two-week wait. The original (misdiagnosed) part was already sent back to Mazda. My out-of-pocket cost is now $1,151 and rising. Given the early warning alerts, misdiagnoses, and the fact that EPS controls several safety-critical functions (steering, cruise, lane assist, etc.), I believe this is a fundamental system failure that should be fully covered under warranty or as a safety-related goodwill repair. I respectfully request that Mazda assume full financial responsibility for the replacement and any related work. Thank you for your consideration.
