My vehicle has been diagnosed with a cracked cylinder head. The problem began when the vehicle had less than 28,000 miles on it. Mazda is repairing it and has agreed to cover the repair under Mazda Customer Care Assistance.

When I checked for recall information 2 recalls were listed under my car's vin #: Recall #1: 3219D with Closed status. Recall #2: 6023G is listed as Active. But, when I tried to schedule a repair for the active recall, the manufacturing website ONLY listed the "closed" recall (3219D) was listed as available for repair, which is bizarre. Please see the attached screenshot of the website. Thanks.

As I was driving, a burning smell emanated from the car. There were no check engine lights on. When I pulled over, I saw a lot of smoke coming from the engine. I checked the oil level and it was above the minimum line bit just barely, even though my husband had just done an oil change on it. I refilled the oil and continued on my trip. The problem repeated for several dqays until I got home and took it to the Mazda dealership. They reported the following: "Cylinder Head Assembly: oil leaking out of back of head, recommend replacing cylinder head and related gaskets. The cylinder head sits above the cylinders on top of the cylinder block. It closes in the top of the cylinder, forming the combustion chamber. This joint is sealed by a head gasket. In most engines, the head also provides space for the passages that feed air and fuel to the cylinder, and that allow the exhaust to escape." This service is $5600! My car only has 80K miles on it. This is a safety issue because there was no warning. No check engine light appeared and there was no warning.

While driving on the highway on a posted 65mph, the fuel remaining on the vehicle was posted at 1/4, approx. 75miles remaining. The vehicle abruptly stalled and I managed to pull over on the side of the road. The vehicle then refused to start again. Had to bring it to a fuel station and refueled leading to the vehicle starting again. Dealership diagnosed a bad fuel pump and fuel rail.

I own a 2019 Mazda cx 5 and it has 78,000 miles. I noticed smoke coming from the engine and a lot of oil leaking on the ground. I brought the car into my independent service center and they confirmed that there was a crack in the engine cylinder head. There is a TSA # SA-031-21 on this problem. I feel that my safety was at risk, since the engine could have seized while driving. This vehicle is available for inspection upon request. No warning lamps or messages were displayed, however there was a strong odor of burning oil. This problem occurred on 1/9/25 and it has been in the service center since.

The contact owned 2021 Mazda CX-30. The contact stated while attempting to park the vehicle in the parking garage at her workplace, the contact reversed and then straightened the steering wheel; however, after shifting to drive and attempting to drive 6-10 feet forward into the parking space, the vehicle unintendedly accelerated and crashed into a pillar before coming to a stop. The contact stated that the engine had revved up, and the vehicle made an abnormally loud sound before the vehicle unintendedly accelerated. The contact stated that the brake pedal was depressed but the vehicle failed to respond, and the vehicle failed to stop as needed. There was no warning light illuminated. The contact stated that the air bags deployed as intended; however, a warning light indicating an air bag malfunction illuminated afterwards. The contact stated that the front driver's side and knee air bags had deployed as designed. The contact sustained pain and bruises on the left side of the chest, on the upper part of the left breast, on the left leg, and on the right wrist and right arm. The vehicle automatically called 9-1-1 for Emergency Assistance, but the contact declined to receive medical assistance. The contact was able to walk away from the scene. No Police report was filed. The contact was unsure whether there was any property damage. The vehicle was towed from the scene and totaled by the Insurance Company. The contact stated that the failure had occurred once before while driving at approximately 60-65 MPH, and the vehicle unintendedly accelerated with an abnormal sound coming from the vehicle, but the contact was able to depress the brake pedal and regained control of the vehicle. The dealer and the manufacturer were not yet notified of the failure. The failure mileage was approximately 31,000.

Good afternoon. I am reporting an incident that occurred on December 29, 2024. I was driving my 2019 Mazda Cx-5 and I was about to make a left turn. However, as I was making the left turn, I received a notification on my dashboard about a "Forward Smart City Brake Support Malfunction" (a photo of the notification will be included). My car instantly lost control and it did not stop until it collided with the street curb of the road I was attempting to turn on. Fortunately for those around me, my car did not collide with any other vehicles on the road. However, this collision caused damage to the right side of the vehicle, as the right airbag of the vehicle had deployed. I believe that this is a very significant problem that should be looked into as soon as possible. If my car were to hit other drivers on the road, it could have led to a very tragic incident. As far as repairs go, I wanted to wait and see how this complaint would carry out. I do not believe that it would be fair for me to pay repairs for an event that was out of my control. Thank you.

Engine light came on. Immediately brought to dealer. Always serviced at dealership. One previous occurance of illuminated light reading major engine malfunction. Pulled car to side of road, called dealership who advised turn off and back on. Didn't tell me about ALL the problems that Mazda was aware of. Now I'm told my new car with 22k miles has a blown engine. No notification from Mazda about a known problem. Asked when buying the car if there were any major problems with this car. Mazda apparently lies.

I am submitted this report for the 12/6/2024 event. The form I submitted previously was for the safety incident that occured last night (1/9/2025). On four different occasions, the most recent being last night (1/9/2025), the vehicle has suddenly lost drive power while driving. The ability to accelerate is suddenly gone. When you press the gas pedal the engine revs, as if it is in neutral, but it does not accelerate. When this happens, the vehicle is still in drive as indicated by light on the shifter showing drive and the fact the vehicle, just moments before, is driving. During all four instances, I had to bring the vehicle to a complete, put the car in park and then back into drive. On three of these occasions there was no pull-out lane and I had to bring the vehicle to a complete stop in a moving lane of traffic. The incident last night occurred mere seconds after making a left-hand turn with traffic coming up behind me at a higher speed then I was travelling. My safety, the safety all passengers in my car and the safety of other drivers are all at risk when this occurs. When this happens there are no warning lamps or messages that appear. After the third time it happened (on 12/6/2024), we took the vehicle to the Mazda dealership we leased it from and it was inspected and we were told they found nothing wrong with it. They noted they did some updates. They kept the vehicle from Monday 12/9 - Thursday 12/12. Less than a month after getting the vehicle back from the dealership this issue occurred a 4th time (on 1/9/2025 at about 8pm). We will be calling the dealership this morning and setting up an appointment for them to again inspect the vehicle for this safety concern. The vehicle is available for inspection.
