    •What component or system failed or malfunctioned, and is it available for inspection upon request? oApparent seat cushion/leather-type material. Possible others. •How was your safety or the safety of others put at risk? oThe seat melted stuck to a gym (drawstring) bag I had placed on it. When removed, the hot, sticky parts of the seat stuck to my bag and hands. I was put at risk for potential burns. oA person could get stuck on the seat potentially putting them at risk when not being able to exit the vehicle if needed in a safety situation. oThe heat controls could be shorted in the seat potentially putting anyone near at risk of a heat or possible fire incident. •Has the problem been reproduced or confirmed by a dealer or independent service center? oNot that I am aware of. UNKNOWN. •Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? oNo. The service center has refused to send someone to inspect. •Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? oNo. oAugust 10, 2024

    The contact owned a 2024 Tesla Model Y. The contact stated that while the contact's wife was driving at an undisclosed speed, the vehicle was involved in a T-bone crash. The contact stated that the driver's side of the vehicle crashed into it while the vehicle was stopped at a four-way intersection. The contact stated that none of the driver's side air bags deployed. The contact stated that only the passenger's side air bags were deployed. The fire department and Police department arrived on the scene. The fire department had to cut the driver's door off to remove the contact's wife from the vehicle. The contact's wife was transported to the local hospital. The contact's wife was severely injured. The contact's wife sustained injuries to the head and the neck along with other injuries. In addition, the contact's wife has continued treatment with a chiropractor, physical therapy, and continued medical appointments. The vehicle was condemned as a total loss. A Police report was filed. The manufacturer was not made aware of the failure. The failure mileage was approximately 1,000.

    While using adaptive cruise control the vehicle regularly decelerates for no reason and without warning "phantom braking". I experience this about once a week, but experienced it multiple times on a longer drive this weekend. The unexplained abrupt braking could result in being rear-ended by another vehicle.

    The contact owns a 2024 Tesla Model Y. The contact stated that while driving approximately 15 MPH the vehicle suddenly experienced unintended acceleration causing the driver to lose control of the steering and crash into a second vehicle. During the crash, the frontend of the vehicle was severely damaged but the air bag did not deploy. The contact sustained a right-hand injury that did not require medical treatment. Three occupants from the second vehicle sustained undisclosed injuries and were transported to the hospital. A police report was taken at the scene and the vehicle was towed away. The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure. The failure mileage was 7,608.

    On multiple ocassions when using adaptive cruise control the vehicles slams on the brakes without warning and for no reason and brings car to a near standstill in the middle of traffic unless I slam the accelerator down and fight the car. Engineers from Tesla inspected the vehicle and said everything was working normally, but it continues to malfunction and almost causes accidents with cars behind in traffic having to slam on their brakes as well.

    I was driving on a street at the speed limit there were no cars in front of me and behind me thankfully the car just braked very hard suddenly for no reason and I lost complete control over the vehicle after the car came to a complete stop I was able to continue driving. Thankfully there were no cars behind me because if there were I don’t even want to imagine the accident I could’ve caused aside from this the breaking was so hard that my whole body hurts and I am afraid to drive the vehicle as I do not know when to expect this breaking since it can happen so suddenly at any time.

    The contact owns a 2024 Tesla Model Y equipped with Continental Tires, Tire Line: Pro Contact, Tire Size: 255/45/R20, DOT Number: (N/A). The contact stated that several weeks after purchasing the vehicle, a neighbor informed the contact that a tire was flat. The contact attempted to pump air into the tire; however, the air immediately seeped out. Due to the failure, the vehicle was towed to an independent mechanic where the tire was replaced with the same brand tire. The contact stated that almost a month later, the tire pressure indicator light appeared on the infotainment screen while the contact's wife was driving the vehicle. The contact was called to the scene and attempted to inflate the rear passenger tire; however, the air seeped out of the tire. Upon inspection, the contact discovered multiple cracks along the inner sidewall which caused the tire to peel. The vehicle was towed back to the independent mechanic where the vehicle remained. The vehicle manufacturer was notified of the failure and referred the contact to the tire manufacturer. The tire manufacturer was notified and informed the contact that a tire representative would need to inspect the tire. The vehicle was not repaired. The tire and vehicle failure mileage was approximately 550.
