NHSTA SAFETY COMPLAINT TEXT Safety defect and Incident Chronology: I am reporting a catastrophic, recurring sudden unintended acceleration (SUA) defect originating strictly from a forward idle state, culminating in a severe bidirectional runaway and property damage crash. This vehicle utilizes an Electronic Throttle System-intelligent (ETCS) that failed to trigger a Malfunction Indicator Lamp (MIL) , Check Engine Light or fail safe limp mode during all three events. INCIDENT 1 (MAY 15, 2026): While maneuvering at low-speed idle into my residential garage, the engine un commandingly surged . I was tapping the brake to go cautiously. Simultaneously, the brake pedal became rigid and physically impossible to depress. The vehicle crashed forward into my tool benches and drywall. INCIDENT 2 ( on or about May 22,2026): Under the identical conditions, the vehicle without command surged forward with total brake rigidity. Fortunately, this time it caused no damage. INCIDENT 3 (June 10, 2026) During a slow speed idle parking maneuver , the vehicle violently surged forward with total brake rigidity and struck shelving. To cut propulsion I immediately shifted out of Drive. Instead of neutralizing the event the car the car underwent a severe bidirectional runaway, violently accelerating in REVERSE for approximately 35 feet across my driveway crashing into my woodpile . I ended up moving forward and stopped somehow. TECHNICAL EVALUATION for INVESTIGATORS: The recurring rock hard brake pedal serves as empirical pneumatic proof that the throttle body valve uncommandedly opened wide while my foot was actively modulating the brakes, instantly destroying the engine manifold vacuum reserve. Furthermore, the vehicle accelerating forward and backwards proves system failure. The vehicle was transported via flatbed to PAPPAS Toyota. Toyota Case # TA 261680161. I request NHSTA safety Egrs secure and audit this transient ETCS-I failure mode.

The contact owns a 2006 Toyota Solara. The contact stated that while his wife was driving approximately 55-60 MPH, the TPMS warning light illuminated. The vehicle was taken to the local dealer and the contact was informed that the ABS monitors the rotation of the tires. The contact was informed that a tire was losing air and was rotating slower than the other tires. The dealer recommended changing the tires. The tires were replaced but the failure recurred. The manufacturer was contacted and filed a claim. The failure mileage was approximately 51,000.

The contact received injuries to their arms and medical attention was required. The "VSC" continues to come on which in turn causes the ck engine light to come on. There needs to be a recall on this issue. I have been researching it, and finding it a problem for so so many customers.

TL* THE CONTACT OWNS A 2006 TOYOTA CAMRY SOLARA. THE CONTACT STATED THAT THE DASHBOARD WAS CRACKING. THE VEHICLE WAS TAKEN TO GETTEL TOYOTA OF CHARLOTTE COUNTY (1801 TAMIAMI TRAIL, PUNTA GORDA, FL 33950) WHERE THE CONTACT WAS INFORMED THAT THE VEHICLE WAS UNABLE TO BE REPAIRED UNDER WARRANTY. THE CONTACT STATED THAT THE VEHICLE WAS NOT REPAIRED DUE TO THE REPAIR COST. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 63,390.

ON MARCH 26,2020 I WENT TO DRIVE MY CAR. THE DASHBOARD WAS CRACKED, STICKY, AND MELTED THROUGHOUT. THE NEXT DAY I CALLED PRIORITY TOYOTA AND WAS TOLD (JOHN SERVICE MANAGER) THAT THEY WOULD NOT FIX IT DUE TO THE WARRANTY ENHANCEMENT NOTIFICATION-ZE6 WHICH ENDED MAY 31, 2017. ALSO, I HAD OWNED THE CAR FOR MORE THAN 10 YEARS. HE WOULD REPLACE IT AT MY COST OF APPROX. $2200 BUT HE WOULD PROVIDE AN EXACT AMOUNT WHEN I DECIDED TO FIX IT. THEN I EXPLORED THE INTERNET TO SEE THE SCOPE OF THE PROBLEM. IT CAN LEAD TO VISIBILITY PROBLEMS (GLARE) AND PROBLEMS WITH THE DASHBOARD AIRBAGS. YESTERDAY, APRIL 7, 2020 I CALLED THE TOYOTA CUSTOMER ASSISTANCE CENTER 1-888-2709371 AND SPOKE TO RANDALL WHO REPEATED WHAT JOHN HAD SAID, BUT ADDED THAT MY CAR WAS OLD AND NOT WORTH MUCH AND I HAD GOTTEN MY MONIES WORTH OUT OF IT. I ASKED WHO I COULD CALL NEXT AND HE SAID HE WAS THE END OF THE LINE. I WAS ASSIGNED A CASE #[XXX] BUT CAN'T SEE TO READ IT FOR MYSELF. THE CAR HAS REMAINED IN MY GARAGE SINCE I SAW THE PROBLEM ON MARCH 26. IT IS GETTING WORSE. IT WAS LAST DRIVEN ON MARCH 5 WITH NO PROBLEMS. I HAVE A TOYOTA RAV 2017 FROM PRIORITY TOYOTA, HAMPTON, VA 757 838-5000.
