    On 6/7 while driving 65 mph I hit a deer on the driver front. My car detected the crash and deployed every airbag inside the car. I received an injury to my legs from the driver's airbag under my steering wheel. If this airbag were not there, I would have had no injuries from this accident. I have missed work and was seen at the local ED for treatment.

    I am filing a safety complaint for my 2024 Toyota Corolla because the infotainment screen intermittently goes black while driving. This is most dangerous when I am using GPS/navigation while driving. The screen has gone black while driving on busy highways at 60+ mph, leaving me without functional navigation. This creates a distraction and safety concern because I suddenly lose directions and may need to look at my phone or troubleshoot the screen while driving. The car has also had recurring phone connectivity issues. Sometimes my phone does not connect through Bluetooth or even when plugged in directly. However, the black screen while driving is the bigger safety issue. I contacted Toyota multiple times. Toyota reference number: #[XXX]. Toyota told me to take the car to the dealership, which I did many times while the vehicle was still under warranty. I took it to two different Toyota dealerships multiple times. They repeatedly said there was no issue because they could not duplicate it. They also blamed Fitbit devices connected to my phone, which did not explain the screen going black while driving. I sent Toyota and the dealerships videos of the issue. However, it is difficult and unsafe to record the screen failure while actively driving. I also contacted Toyota about lemon law concerns, case [XXX]. Toyota denied the claim. The vehicle is now out of warranty, but the screen still occasionally goes black while driving.     I was driving my 2024 Toyota Corolla SE gas model (with the convenience package) on the interstate in [XXX] during stop-and-go traffic. Traffic had been moving slowly for about 15 minutes, and I had Brake Hold turned on, cruise control was off, Proactive Driving Assist was on. I was behind a semi truck and was moving very slowly, getting ready to stop. I had maybe 3 or 4 car lengths between me and the semi so they could see me in their mirrors. As I was about to stop, the brake pedal suddenly went all the way to the floor, the car jerked to a stop, and the electronic/emergency parking brake applied on its own. I put the car into Park and then back into Drive, and the problem did not happen again during that drive. There were no warning lights, dash lights, or warning messages before this happened, and there were also no warning lights or dash lights afterward. The issue happened suddenly with no prior symptoms that I noticed. I believe the brake system, Brake Hold system, electronic parking brake system, or a related brake/vehicle control system malfunctioned. This put my safety and the safety of others at risk because the car unexpectedly stopped abruptly in interstate traffic. Since I was in heavy stop-and-go traffic and other vehicles, there was a risk of being rear-ended, causing a crash, or becoming disabled in traffic. I took the vehicle to a Toyota dealership after the incident. They were not able to reproduce the problem, but they did find stored code C14FE14. They told me they checked the brake system, cleared the code, tested the steering angle sensor connector, and said everything tested within specification. They also performed a multipoint inspection and said the battery, brake pads, and other inspected items tested well. Driver Information Display Issue Type: Design Defect / Driver Distraction I am reporting a recurring issue with my 2024 Toyota Corolla Cross (gas-powered, not a hybrid). The vehicle frequently displays a “Battery Charging” message on the dashboard whenever I come to a stop at traffic lights or stop signs. This message appears even when the vehicle is operating normally and there is no actual charging system problem. According to Toyota documentation and owner discussions, this message is triggered when the auto start/stop system decides not to shut off the engine due to the 12-volt battery’s state of charge. However, the message is unnecessary, cannot be disabled, and provides no actionable information to the driver. It functions as a non-critical status notification but is presented with the same prominence as a warning. This creates a driver distraction, as the message repeatedly draws attention away from the road for a condition that is not a fault, not a safety issue, and not something the driver can address. The message appears frequently during normal driving and cannot be suppressed. I believe this is a design flaw in Toyota’s start/stop implementation and dashboard messaging. The system should not display a warning-style message for normal operating behavior, especially when it cannot be corrected by the driver. I am requesting that NHTSA log this issue as a potential driver-distraction concern and encourage Toyota to revise the software to eliminate or suppress this unnecessary message.

    Incredibly dangerous situation! Driving down I-15 in SLC when my Sunroof EXPLODED! 2024 Toyota Corolla Cross Hybrid, 22k miles, purchased new and sunroof came with the car. Sounded like a gunshot. Luckily, no one was injured by flying glass and kept the car stable. I could see how someone reacting to this sound might slam on their brakes or swerve the car or worse the driver could be cut by falling glass if the slider were open. My "damage" is having to pay for a new roof. Toyota dealer tried to say that something hit my car and it wasn't a manufacturer defect. Right. Tell me how a rock is going to get past my large Thule cargo box to drop a hole like this one! I filed a claim with State Farm but the part is special and must be ordered. UP until yesterday I have never had any issues with the sunroof. Such a racquet. Toyota gets to keep selling replacement sunroofs and the insurance companies and consumers are the ones paying for their poor workmanship.

    DEFECT: Steering wheel fails to fully return to center after turns, consistently stopping 1/2-1 inch off-center. Minor steering adjustments stick and don't self-correct. Pronounced binding zone near center prevents smooth return. SYMPTOMS: After curves, wheel returns 80% but stops visibly off-center requiring manual correction. Minor lane adjustments stick and require counter-correction. Constant manual steering correction causes driver fatigue. Progressive worsening since purchase. SAFETY: Incomplete return at highway speeds causes drift. Sticking divides attention from hazards. Emergency maneuvers require manual straightening during critical response time. SERVICE: February 2026 - McGovern Toyota technician documented "VEHICLE DID NOT RETURN OR SELF CENTER" but Toyota corporate refused repair claiming "normal characteristic." Had 6+ alignments with no improvement. Recent alignment caused binding to flip sides, proving internal mechanical binding not alignment issue. RELATED: Toyota issued CSP 22TE02/TSB T-SB-0028-22 (2022) for 2018 Camry with identical "reduced return to center" caused by intermediate shaft binding. My 2024 Corolla has same defect Toyota acknowledged in 2018 models but refuses to repair. INDUSTRY PATTERN: Honda HR-V TSB 24-094 (2024), Mazda CX-90 Recall 24V-022 (26+ failed fix complaints, 2 crashes), GM Bolt TSB 17-NA-148, Ford Explorer ST owner reports - all for steering not returning to center. REQUEST: Investigate if Toyota intermediate shaft defect extends to 2024 Corolla and whether industry-wide EPS binding represents systemic safety issue. Toyota confirmed defect in writing but denies repair.
