Multiple vehicle systems have malfunctioned, including the transmission, infotainment/display screen, and backup camera. Most recently, the backup camera has stopped functioning, preventing a rear view while reversing. The vehicle is available for inspection upon request, along with any service records documenting prior repair attempts. The transmission issues created an unsafe driving condition due to unpredictable shifting and hesitation. In addition, the backup camera is no longer functioning, significantly reducing rear visibility while reversing and increasing the risk of striking a pedestrian, child, vehicle, or object. These defects pose a safety risk to both the driver and others around the vehicle. The vehicle has been presented to an authorized Toyota dealership on multiple occasions for repair of various issues. Most recently, I sought service for the inoperative backup camera but was refused service. Previous concerns have been documented through dealership service visits. The vehicle has been inspected by an authorized Toyota dealership during previous repair attempts. It has not been inspected by police or insurance representatives. The vehicle experienced ongoing drivability and infotainment issues before the backup camera failed. The backup camera eventually stopped functioning entirely.

Engine seized up just after completing a pass on 2 lane road with oncoming traffic approaching Truck started shaking, warning lights illuminated and I lost all ability to accelerate. I was only able to crest a slight incline and safely get to the shoulder of the road due to momentum gained during passing. Toyota states hybrid models have locomotive ability after engine seizing, this is clearly not true. Had the seizing occurred while passing, it would have likely resulted in a head on collision. Dealer inspection confirmed engine seizing and noted metal shavings were observed on the dipstick.

We have dropped off our vehicle in January of 2026 for an engine recall with Toyota. We are about to go into the 8th month of no repair. We have a Toyota Brand Engagement Center Case number 260615000517. Still no engine ID'd by corporate nor any updates towards eta. We have a loaner, but our truck is sitting with brand new tires and loosing value daily.

Forward facing Camera's: After vehicle was recalled for a refacing camera issue and repaired for that recall, now the forward facing camera does not work properly. When coming to stop or while stopped the forward facing camera's no longer work. They only come on when I get close to something. They also no longer read speed limit signs. When stopped at a cross walk if a child runs in front of the vehicle I do not get a warning until the child is directly in front of the vehicle. This could cause me to hit a child based on the height of the child and being blocked by the hood of the vehicle. I need the forward camera's to activate at 5 miles per hour or slower and stay on when stopped until going over 5 miles per hour like they did before the backup camara recall was installed on my truck. This is an on going issue starting June 18,2026. I took it to the dealership July 14,2026 and they did not correct the issue. This issue could case a child to be run over!

was waiting for the traffic light to turn green. Vehicle in drive with foot on brake. the traffic light turned green, I took my foot off the brakes and pushed on the accelerator. Nothin happened. I saw another vehicle approaching at a high rate of speed behind me so I continued to mash on the accelerator. right before being hit by the car behind me my vehicle lurched forward a few feet then stopped again. I looked at the dash display and saw a "Hybrid System Malfunction" warning. Because of the traffic situation/location it was not safe to just sit in the road so I continued to pump on the accelerator. each pumped moved the vehicle forward a couple of feet. I did not turn of the engine because I was unsure what would happen so I continued to do that for about a mile or so until I could get to a safe place to pull over. during this time, I almost got rear-ended and side-swiped several times. Once stopped I attempted to start the vehicle several times and got the "Hybrid System Malfunction" display and the vehicle would not start. I waited for about an hour with the vehicle off and restarted the engine. I stayed off the freeway and took backroads to the closest dealer service center where it is currently located. When I dropped it off the service rep said he hasn't seen anything like this. Prior to this incident I had no issues with the truck and it was last serviced by the dealership on March 4, 2026.
