    - Belmont, CA

Vehicle was stopped at a stop light with brake pedal pressed. Car started rolling with brake pedal pushed and repeatedly pushing the brake pedal did nothing! Car jerked three times and responded to pressing the park button. Once in park, I noticed that all warning lights came including brake warnings but the car would not turn off and it was stuck in park. Car would not come out of park but the radio and hazards worked. Police responded and advised not to take my foot off the brake until the tow truck got there. Initially, the tow truck driver could not put the car in drive or neutral or turn off vehicle. Tow truck driver was able to turn off the car and get it started again. Brakes should not just stop working in the middle of traffic in a 2 year old car with only 17k miles on it. Reporting because it appears this is a repeated Volvo issue. Per the Volvo app, all software downloads were updated and there was no error message before this happened.

Vehicle received recall 023G (TSB 24-01-076G) on 4/30/2025 at 42,910 miles for P0088 fuel rail pressure issue, involving ECU update to high-pressure fuel pump control logic. No codes were present at that time and no physical inspection of the pump was performed — only a code scan. Vehicle had no check engine light prior to this recall. On 9/16/2026 at 46,471 miles, P0088 code returned, diagnosed at an independent repair shop. Concerned the recall fix did not address the underlying defect

    1) left rear trailing arm broke while driving, causing high risk of injury. ( a known problem with this model. A recall issued in other salt belt states, but not North Carolina. Living in western NC, We have extreme amounts of salt due to (3) ski slopes in our area. Yes. 2) unknowing of the recall my husband and I were traveling home from out of town. We made it to our town when the wheel arm broke, causing the vehicle to stop unexpectedly. Had this happened while driving on the interstate like we were, we would not have survived it, and who knows how may others would have been killed or injured. 3) yes, when seeking repairs for the vehicle, we were told that no one would do repairs for the vehicle. It was then we found out about the recall for the other states. 4)No, manufacturer has been contacted. 5) No, there were none that we were aware of, except a crunch sound when making a sharp turn. It was heard the weekend we were out of town, but only a few times.

  • 2019 ACURA RDX
    - Seabrook, TX

The front sunroof glass on my vehicle shattered without warning while the vehicle was in motion and without any impact, collision, or contact from an outside object. I had the vehicle inspected at AutoNation Acura [XXX]. The dealership's own service advisor confirmed in writing that the remaining glass "seems to be facing upward, which is an indication that it may have shattered due to pressure, as opposed to any outside influence," and stated explicitly that there was "no evidence of mistreatment/accident found during inspection." I am enclosing a copy of that email. Despite this documented finding, Acura denied my warranty/goodwill claim for the repair telling me the glass had been broken by an outside force — a conclusion that directly contradicts the dealership's own written inspection. This is a known, documented issue Acura Technical Service Bulletin 23-004 acknowledges spontaneous glass breakage as a known issue on this platform. My incident is consistent with that bulletin: no external impact, and physical evidence consistent with breakage from internal pressure rather than an outside force. I believe this points to a defect that Acura has already identified internally but is not consistently honoring at the claims level. This could have easily caused a wreck when it shattered or had the covering not be closed the glass could have injured anyone in the car and I travel with a [XXX] baby often. • 2016 TESLA MODEL X
    - Hayward, CA

The car was making a grinding and knocking sound which varied with the engine speed. This noise was confirmed to be coming from the rear drive unit by the Tesla technician. The vehicle has approximately 105,000 miles. An internet search revealed it was a common problem. The repair costed $6,502.20. It appears the problem was related to the original part which had a high failure rate and this was corrected with an updated part (ASY, DRIVE, UNIT, 3.0-150, REAR (1037000-00-F)). I understand this the drive unit that would address the coolant-ingress/seal failure of the Model X Large Drive Unit

  • 2022 FOREST RIVER PUMA
    - Baytown, TX

The vehicle exhibits severe manufacturing safety non-compliance within the primary electrical chassis and interior matrix, creating an active fire and life-safety hazard. Due to chronic, un-remediated pre-sale factory roof structural defects, catastrophic water intrusion entered the upper ceiling channels. This moisture traveled directly along the internal wire tracks that feed the ceiling lights, main appliances, and slide-room mechanisms. As a direct result, the vehicle suffers from frequent burning odors, recurring short circuits, localized electrical arcing near the structural frame, and melted/burned wiring components. A certified mobile RV technician has formally flagged the electrical system as an active fire risk. Because the vehicle has been repossessed and is currently stored in a commercial lot in Baytown, Texas, this severe, documented electrical arcing hazard presents an immediate threat of fire to the storage facility and any future consumers if the asset is liquidated at auction without total electrical remediation.

  • 2025 ACURA MDX
    - CYPRESS, TX

I own a 2025 Acura MDX A-Spec purchased in XXX from John Eagle Acura in [XXX]. The vehicle has less than 10,000 miles. Component/system: The Auto Idle Stop/start system has malfunctioned. During the past week, the engine unexpectedly shut off while stopped at red lights and stop signs approximately 4–5 times. When it was supposed to restart, the vehicle sometimes failed to restart normally and required multiple attempts. The vehicle and components are available for inspection upon request. Safety risk: These incidents occurred at intersections while I was stopped in traffic. When the engine failed to restart, I was unable to immediately proceed while vehicles were behind me, creating a potential collision and traffic hazard. I regularly have my two young children in the vehicle, making this especially concerning. Dealer inspection: I brought the vehicle to John Eagle Acura after experiencing the problem. The service advisor indicated that this is a known issue, and the vehicle was serviced for the concern. The failure occurred repeatedly in real-world driving, but I do not know whether the dealer was able to reproduce it during inspection. Manufacturer/other inspection: The vehicle was inspected by an authorized Acura dealership. It has not been inspected by police or insurance representatives. Warnings/symptoms: I received no prior notification or warning from Acura regarding this issue. The first incidents occurred during the week before my dealership visit. The engine shut off while stopped and then did not restart normally. I did not observe a warning lamp or message specifically indicating that the vehicle would fail to restart. After experiencing the problem, I learned that NHTSA has an ongoing investigation, EA25-004, involving Auto Idle Stop failures on Honda and Acura vehicles, including 2023–2025 Acura MDX models. I am submitting this complaint so these repeated incidents can be documented as part of NHTSA's review. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 5

    - Sacramento, CA

Safety Complaint and Incident Report: Severe Injury Caused by Unprotected Sharp Metal Edge. I am writing to formally lodge a safety complaint regarding a serious design/manufacturing hazard on my Tesla vehicle. While washing the vehicle and wiping down the area near the liftgate/trunk strut, my finger was severely cut by an exposed, unshielded metal sheet edge. This area is extremely sharp and lacks any protective trim, warning labels, or factory finishing, presenting an unacceptable safety hazard during routine vehicle care. This poses a severe risk of injury to any owner or user. I am submitting this formal complaint to document the safety defect and request that Tesla address this manufacturing oversight immediately to prevent further injuries to consumers.
