    The contact stated that while driving at an undisclosed speed, the contact observed a 2026 Tesla Model Y Robotaxi that had unexpectedly stopped in active traffic. The contact stated that while driving in the right lane, the contact became aware that the vehicle in the left lane remained stopped at a green light. The contact expressed concern that the issue posed a serious safety hazard and could have resulted in a more severe situation with ongoing traffic. The contact was concerned that the issue could be especially dangerous in a School Zone with children nearby. The manufacturer was not made aware of the failure.

    Subject: Formal Safety Complaint – Tesla Robotaxi Operation in Austin, TX on [XXX] Dear NHTSA ODI Team, I am writing to formally report a serious safety concern involving a Tesla Robotaxi operating in Austin, Texas on [XXX]. The vehicle was documented drifting into the wrong lane, crossing a double-yellow line before realigning in the correct lane—posing a definite risk to oncoming traffic The same or a related Robotaxi was recorded exceeding posted speed limits, with speeds reaching approximately 39?mph in a 30?mph zone Visible in these clips were no adverse weather conditions or challenging intersections, contradicting claims that such factors influenced the mishaps Why this matters: The deployment is an early paid, driverless Robotaxi service—yet the performance demonstrated in these incidents raises immediate concerns about the system’s readiness for real-world, public-road operations The vehicles appear to be violating traffic laws, compromising the safety of passengers, other road users, and pedestrians. NHTSA’s own inquiry into these behaviors, and Tesla’s desire to keep responses confidential, underscore the importance of fully transparent oversight What I respectfully request: Confirm whether Tesla submitted its full responses by the deadline Ensure that all communications are reviewed under applicable standards, not withheld as proprietary. Promptly assess and address the potential defect in Tesla’s Robotaxi control software or hardware. Require Tesla to provide crash data, performance logs, and operational protocols for these and future incidents.     The supercharger cord has exposed wiring near the handle where people plug in the car to the charging station, and is a potential electrocution safety issue. I contacted Tesla a month ago, but they didnt do anything, and they also, don’t have any easy way to report Supercharger issues, even though they have easy ways to report car issues through the app.

    The contact owns a 2020 Tesla Model X. The contact stated that while the vehicle was plugged into the home charging station, an app message was received that indicated that the vehicle had no charging power. Upon inspection, it was discovered that the home charging station electrical cables connecting to the home had malfunctioned causing the electrical wiring to become burnt and charred. The home's electrical wiring was also damaged during the failure. The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure, but no assistance was offered. The failure mileage was 53,000.

    Hello, I was taking my car reverse and my car door was open slightly and it hit my garage door, and that was my mistake. My only problem here is, the driver side rear window broke and it broke into MILLIONS OF PIECES and shattered all over inside.the car. Not sure what the technology is, and it should not break into millions of tiny, tiny pieces. It's a brand new car and struggling to clean it because they are no minute. Thanks

    https://www.tesla.com/support/home-charging-installation/wall-connector The strain relief on the cable that is used to charge the vehicle became loose exposing dangerous high voltage 240v (40A) bare wires inside where the consumer holds it to charge the vehicle. If touched by anyone while charging, would likely cause electrocution and possible death. As the vehicle takes hours to charge outside, not only is the driver and passengers at risk but passerby's including young children. This could happen during clear, dry conditions and exacerbated by wet conditions. The manufacturer, Tesla, responded to my complaint by telling me the only remedy is for me to buy a new charging unit at a cost of $500 plus a few hours of electrician labor. There was no mention of a replacement part (strain relief/handle) other than purchasing the entire system and paying an electrician to replace it. There were no prior symptoms (no scratches, no cracks, no physical damage, etc..) which leads me to believe that this is a manufacturing defect and a serious live threatening one. Furthermore, the Tesla support team wrote on 7/16/2022: "Thank you for reaching out to the Tesla Wall Connector Support team. "he crack shouldn't stop the unit from charging. Your unit is out of warranty. If you would like to purchase the newest Wall Connector while it's on sale for $400 instead of $500, I have provided the link. We do recommend a certified electrician install the Gen 3. https://shop.tesla.com/product/wall-connector." This implies that the wall charger can be used to charge the vehicle and ignores the DANGEROUS ELECTRICAL HAZARD present which is totally unacceptable.

    The contact owns a 2013 Tesla Model S. The contact stated upon attempting to remove the charger from the vehicle charging port, the charger remained plugged in. The manufacturer was notified of the failure and sent a certified technician who assisted in releasing the charger. The certified technician diagnosed that the charging port and the charger cord had melted and needed to be replaced. The manufacturer informed the contact that he would be responsible for the cost of repair. The contact became aware of NHTSA Campaign Number: 14V006000 (Equipment). The vehicle was not repaired. The failure mileage was approximately 120,350.

    Only charges to 22 amps even though the Car is set to 45 amps. I had two electricians look at it already. The charger does not connect to tesla to update even though the wifi works
