My family and I was I involved in a hit and run accident. My minor child is still suffering from shoulder/rotator cuff injuries. Hewas in the process to start conditioning for football. The seatbelt on the day of the accident did not lock. GM sent a criminal investigator not a trained engineer for the vehicle. The seatbelts needs to be inspected. The motor/transmission is making a ticking sound.

2019 CHEVROLET EQUINOX- Sicklerville, NJ
This issue presents a serious safety concern. During the initial occurrence, the vehicle was unable to sustain highway speeds for extended periods (approximately 10 minutes or more). When coming to a stop, the vehicle would shake, rev unexpectedly, lose power, and shut off completely. During these events, the vehicle would not accelerate, creating a hazardous situation, especially in traffic. The condition has since worsened. The same symptoms now occur upon starting the vehicle and during the warm-up period, increasing the risk of sudden engine shutdown without warning. Additionally, a distinct “gulping” sound is coming from the rear of the vehicle, raising concern that the fuel system may be compromised. No diagnostic or pending fault codes are present, and the local dealership has been unable to identify the cause. The vehicle has fewer than 100,000 miles. Due to the unpredictable loss of power and engine shutdown, this issue poses an immediate safety risk to the driver, passengers, and surrounding motorists.

2021 CHEVROLET SILVERADO 1500- Glendale, AZ
Check engine turned on and at the same time the vehicle turned into safety mode abruptly, it slowed down to 10 miles an hour while on the high way and receiving a message stating that the "engine power reduced", the issue was presented to the dealership and they believe to be fixed with a sensor issue, the light came back on a few days after the initial repair and once again the engine lost power while driving the vehicle, the vehicle was then taken back to the dealership for repair once again, both times I mentioned the service bulletin, which they did not consider in their repair, stating that the issue was specific for a code (which all codes present are in the service bulletin) the 2nd repair was done on a actuator, which they stated should fix the issue, and did not, a 3rd time the vehicle lost power while hauling a trailer, this time I brought up the concern of them not reviewing the service bulletin, this time they confirmed that the service bulletin was the issue causing the check engine light (GM service bulletin no: 21-NA-149) now they are looking to slowly start repairing the issue and wait for the check engine light to turn on again to then go back and forth and review step by step the instructions in the bulletin.

Transmission Failures. The issue initially presented as intermittent trasmision shuddering, hesitation during accelaration, harsh and delayed shifting, and noticeable vibration, particularly at low speeds and during gear changes. These symptms progressively worsened over time and affected drivability and safty. Prior the failure, there were no warning lights or dashboard messages displayed. Based on professional diagnosis, they are consistent with internal transmission defects commonly reported for this model.(GM's-8 speed automatic transmition 8L45/8L90)Technical Service Bulletins TSB18-NA-335 and TSB16-NA-175. Chevrolet has denied reimbursement. The repairs performed to date have only provided temporary remedy, the issue remains ongoing and consistent with the known defects associated with the transmission.

2015 CHEVROLET SILVERADO 1500- Southborough, MA
The failure occurred in the rear sliding window defroster electrical system, specifically at the left-side electrical connection of the power sliding rear window. The component overheated and ignited, causing fire damage and shattering of the rear left portion of the window. The vehicle and damaged components are currently available for inspection upon request. The defect resulted in an active electrical fire during normal vehicle operation (remote start while warming up). This posed a serious risk of vehicle fire, injury to occupants, and potential spread to nearby vehicles or structures. As a firefighter, I recognized the danger and immediately shut off the vehicle and ventilated it; without trained action, the incident could have resulted in a total vehicle loss or more severe consequences. Yes. Copeland Chevrolet in Hudson, Massachusetts inspected the vehicle and confirmed that the damage was consistent with the known defect in the rear sliding window defroster system, consistent with GM Customer Satisfaction Program XXX. The vehicle has been inspected by an authorized GM dealership (Copeland Chevrolet). No police or insurance inspection has occurred to date. No warning lamps, messages, or prior symptoms were present. The failure occurred suddenly and without warning during normal operation while the vehicle was warming up using the factory-installed remote start. This is a known defect acknowledged by GM under Customer Satisfaction Program XXX (which expired in 2022), which described overheating and fire risk in the rear sliding window defroster electrical connection. That program was limited by state and excluded Massachusetts, so I was never notified despite identical risk. This same defect is currently the subject of an active safety recall in Canada, where it is treated as a fire hazard requiring corrective action. While driving to work on [XXX] in Huntsville AL at approximately 55MPH. I was using Chevrolet SuperCruise. I tried turning off super cruise with the super cruise button. It did not turn off, tried pressing cruise control button, nothing, tried the cancel button, nothing, pressed brakes and would not turn off. Truck started slowing in very heavy traffic, it turned hazards on. I was finally able to get it turned off. I have reported to dealer, no surprise they didn't answer, left message at 9 am this morning, no call back. I have made a report to GM. I no longer feel safe putting my family in this truck. This truck has been in shop several times for "Service driver assist", no on-star, no cellular data, extremely slow infotainment system and no supercruise available. The truck at last visit for the service driver assist took more than 2 week. I was told a camera and a software update were performed. The issues have persisted. The super cruise event yesterday was the first occurrence of this type. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

2019 CHEVROLET TRAVERSE- Gary, IN
When placing the vehicle in park, whether it’s on a flat surface or an incline, the SHIFT TO PARK notification displays. In addition, the nonfiction chime rings consistently until the gear shift is realigned or until it stops on its own. During this ringing period the headlights won’t go off and the ringing last anywhere from 5 to 20 minutes. The longest time recorded was 20 minutes. Once the ringing finalizes, there is still a possibility that the start/stop button has to be pressed manually so the headlights will completely turn off.

2015 Chevrolet Silverado complete engine failure. At 143,000 miles the engine failed because of the displacement on demand fuel injection system/lifters. Lifter failed without warning, destroyed the camshaft, cam bearing and crank and crank bearings. Chevrolet/GM were no help at all and took on responsibility for building defective engines they knew would fail after the warranty expired. I ended up spending nearly $17,000 to have a remanufactured engine installed.
