The contact stated that the vehicle experienced a no-start condition and was towed to a dealer, where it was diagnosed and determined that the high-pressure fuel pump had failed and needed to be replaced. However, the contact was informed that the repair was not covered under NHTSA Campaign Number: 24V957000 (Fuel System, Diesel); because there were not enough metal shavings found in the fuel system. The check engine warning light was illuminated. The contact used a vehicle scanner to retrieve DTC: P2291. The contact notified the manufacturer of the failure and was referred to the dealer for assistance. The failure mileage was approximately 134,000.

    I was traveling down the highway at approximately 55mph when truck suddenly lost power. I was able to pull over and had the vehicle towed as truck would not start. It was later determined that the cp4 high pressure fuel pump grenaded causing complete fuel system replacement costing over $11,000 in repairs. Shards of aluminium was sent throughout the fuel system.

    Subject: Request for Review of Recall Remedy Effectiveness – Ford Recall 24S78 / NHTSA Recall 24V-957 Vehicle: 2020 Ford F-250 Super Duty 6.7L Power Stroke VIN: [XXX] To Whom It May Concern: I am submitting this letter to document a significant safety event involving my 2020 Ford F-250 Super Duty and to request that NHTSA consider whether the current recall remedy associated with Ford Recall 24S78 / NHTSA Recall 24V-957 adequately addresses vehicles that are already exhibiting symptoms of CP4 fuel pump deterioration. I am not submitting this complaint to seek financial compensation. My purpose is to provide information that may assist NHTSA in evaluating whether the existing remedy sufficiently protects motorists from the safety risks associated with CP4 pump failure. Background My vehicle's VIN has been confirmed as included in the affected population covered by the CP4 fuel pump recall. Prior to the ultimate failure, the vehicle exhibited symptoms that, in hindsight, may have been indicators of fuel-system deterioration. In February 2026, I documented concerns regarding abnormal starting behavior and specifically questioned whether either vehicle battery was failing. No battery issue was identified. Believing the issue might be battery related, I replaced both batteries at my own expense. The starting concern continued. On [XXX], while covered under a Ford Extended Service Plan, Fuel Injector #4 failed and was replaced under warranty. During the same service visit, the recall software update was performed. Despite the recall remedy being completed, the vehicle subsequently suffered a catastrophic CP4 pump failure in August 2026. Safety Event The failure occurred while traveling between Utopia, Texas and Tarpley, Texas on a rural roadway with sharp curves, steep terrain, significant drop-offs, and little or no roadway shoulder. The contact stated that the vehicle was repaired under NHTSA Campaign Number: 24V957000 (Fuel System, Diesel); however, while driving 40 MPH approximately two months later, the vehicle was running rough and stalled. The check engine warning light was illuminated. The contact pulled over to the side of the road. The vehicle was towed to a dealer, where it was diagnosed with a high-pressure fuel pump failure, causing the fuel system to become contaminated. The dealer determined that the fuel system needed to be replaced. The dealer submitted a diagnostic report to the manufacturer. The contact notified the manufacturer of the failure, and a case was filed. The vehicle was not repaired and was pending the manufacturer's decision regarding the coverage of the repair. The failure mileage was approximately 103,000.

    10r140 paired with 6.7 powerstroke in a 2020 f250 build date 11/19 at the Kentucky plant. My truck specs and build date and location is in the timeframe of the planetary gear recall, ford has also issued tsb’s about the cdf bushing that should have been a recall because it is a design flaw on every 2020-2022 model truck. It’s not fair that we should have to pay for a tsb bc we don’t have warranty left. My truck has 84,000 miles but is out of the 5 years warranty period. Symptoms I am having with my transmission are: Harsh engaging to drive and reverse Flair shifts bad sometimes almost like it goes into neutral between changing gears Has shifted from 4th to 1st instead of 4th to 5th or 6th, then it started shifting normal right after. Had my factory 3250rpm limiter not been moved to 4500rpm the truck would have locked the rear wheels up and possible caused driveline damage or other damaged. Also had a car been behind me when this happened I would have been in a wreck because it slammed to a low speed and the car would have rear ended me. Slips in 4th and 7th gear the most but sometimes in other gears Bumps when it shifts a lot of the gears Sometimes shudders and shakes and will almost make like a pop noise Will clunk on downshift from 5-3 Sometimes clunks when upshifting 3-5 I have had the truck looked at by an independent diesel mechanic I use and he said there is a problem somewhere in the transmission, it’s either planetary gear or cdf bushing has moved or the drum itself has cracked. The component itself has not been inspected because the only was to inspect this is to tear the transmission down. The pan has been dropped and had a lot of metal material on the magnet. No codes on the dash but my mechanic said I had dtcs stored for the transmission. I have created a case claim with Ford trying to see if they will extend the recall for the known issue but I have had zero luck with them. I have videos of the issues but can’t upload them on here.

    The contact owns a 2020 Ford F-250. The contact received notification for NHTSA Campaign Number: 22V256000 (Power Train); however, the recall repair could not be completed. The vehicle was taken to a dealer where it was diagnosed that a thrust washer was missing from the output planetary assembly and had disintegrated, contaminating the transmission case, transfer case, torque converter, cooling lines, and radiator. The missing washer caused the planetary gear to shift and damage the transmission case. The manufacturer was notified of the failure, and a complaint was filed. The failure mileage was 80,256.

    Left rear seat belt buckle or system failed causing a fault in the Airbag/Restraint system. The fault initially happened in June of 2025 with about 50k miles on the vehicle. I took it to a dealer who told me the buckle was not available and the whole system needed to be replaced for over $1500. Vehicle seldom has anyone in the back seat but that is where my children sit. Looking at posts on the internet, this seems to be a common enough problem that this should be taken care of under recall or customer service warranty.
