2008 LEXUS LS- Melbourne, FL
The contact owns a 2008 Lexus LS460. The contact stated that while driving out of the driveway, the brakes became inoperable. There was a loud audible sound detected. The message to replace actuator was displayed. The vehicle was towed to the local dealer, where it was diagnosed that the brake actuator needed to be replaced. The vehicle was not repaired. The vehicle was taken to an independent mechanic, who replaced the actuator. The manufacturer was contacted, and opened a case, but no assistance was provided. The failure mileage was approximately 57,000.

2024 LEXUS RX HYBRID- Woodbury, NY
The contact owns a 2024 Lexus RX Hybrid. The contact stated that after the husband parked the vehicle on the street while visiting the hospital, the husband later discovered that the vehicle had caught on fire and burned and the fire department was called to the scene and extinguished the flames. The origin of the fire was not yet determined. A fire report was not yet available. The vehicle was not yet towed away. The manufacturer and local dealer were not contacted regarding the incident. The failure mileage was 3,000.

2015 LEXUS CT- carrollton, TX
I noticed that the brakes felt a bit weird when engaged. I felt a touch on the brakes and heard as if the abs pump was pumping. Thinking it was something of a fluke, I didnt pay any further attention to it. After not driving the car for couple of days, when I turned on the car a day ago the warning (!, brake, ABS, and traction control lights) are all lit up on my 2015 Lexus CT200h. After doing some research, I have found an old recall for Lexus CT200 H for (2011-2014) see (L-SB-0009-210) from May 11,2021 letter online. A code scan revealed a C1391 error, indicating an abnormal leak in the accumulator. This ABS module is the same as the recalled Toyota ABS module. Please this could be dangerous to me and my family.

2014 LEXUS CT- Houston, TX
Defective Brake Actuator and Safety Concerns in 2014 Lexus CT200h I am writing to express my deep concern and disappointment regarding a critical safety issue with my 2014 Lexus CT200h. I have recently discovered that the brake actuator in my vehicle is defective, potentially compromising my safety and the safety of others on the road. Details of the Issue: I have experienced all of my brake warning lights are all lit up. I would have to step on my brake 5 times as hard to stop my car. This issue poses a significant safety risk. From what I understand from this problem, the brake system can stop working at any moment. Upon further research, I have discovered that this problem appears to be a known issue with the Lexus CT200h and similar Prius models, as they share the same brake actuator components. Numerous complaints and reports have been documented online and with the National Highway Traffic Safety Administration (NHTSA). Request for Action: I urge you to take immediate action to address this critical safety concern. I request that you: Thoroughly investigate the issue with the brake actuator in my vehicle and perform any necessary repairs to ensure its proper functioning. Cover the cost of all repairs associated with this defect, as it appears to be a systemic issue rather than an isolated incident. Provide a clear explanation for why this known defect was not addressed proactively and what steps are being taken to prevent similar issues in the future. I am deeply concerned about the potential safety implications of this malfunction and believe that Lexus has a responsibility to prioritize the safety of its customers. 2007 LEXUS IS- Pocomoke City, MD
The contact's son owned a 2007 Lexus IS250. The contact stated while her son was driving at 15 MPH through an intersection, another speeding vehicle that did not have the right-of-way, crashed into the passenger's side of the vehicle, causing the front end of the vehicle to crash into another vehicle. The contact's son was knocked unconscious. The air bags failed to deploy. There was no warning light illuminated. The emergency services escorted the contact's son to the nearest hospital, where he passed away a few days later. The vehicle was towed and totaled. A police report was filed. The contact stated that the vehicle had previously been serviced and had passed State inspection. The local dealer was not notified of the failure. The manufacturer was made aware of the failure, but no assistance was provided. The failure mileage was approximately 135,000. The contact is requesting a phone call in regards to this case.

The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The dealer was made aware of the issue and confirmed that parts were not yet available. 2020 LEXUS RC- Wytheville, VA
Car was sold to me with wrong size front tires . It was a certified car. Had collision and was fix . Drove till mileage service and they reject for wrong tires. But did not fix.. I drove home and waited for repo. Now car is in repo yard with wrong tires and they want to auction it that way. It was certified by 3 dealerships. I have pictures to show I was sold certified car with wrong tires. I am disabled veteran. Not safe per Michelin. Car has 235.it is a 255 only to be used

Urgent Safety Complaint Regarding Brake System Failure and Manufacturer's Extended Warranty Acknowledgment To Whom It May Concern, I am writing to formally file a complaint concerning a critical safety issue with my vehicle, which has significantly impacted its braking system. This issue not only presents a severe risk to my safety and that of other road users but has also been acknowledged by the manufacturer through the issuance of an extended warranty for this specific malfunction. Approximately one month prior to the complete failure, my vehicle began emitting an unusual hissing noise from the brake pedal area, leading to a near-total loss of braking functionality. The incident was characterized by the activation of the Anti-lock Braking System (ABS), traction control, and brake warning lights, accompanied by a repetitive buzzing noise every 5-10 seconds related to the brake system's charging mechanism. The brake pedal became exceedingly soft, rendering the vehicle's brakes nearly non-existent. Despite the vehicle being only a year and a half old and previously showing no major issues, this braking system failure occurred suddenly and without warning. However, in light of the manufacturer's recent communication regarding the extended warranty for this specific problem, it is clear that this is a known issue that poses a significant safety risk. Given the manufacturer's acknowledgment of the defect through the extended warranty, I urge the National Highway Traffic Safety Administration to investigate this matter with heightened urgency. The potential for harm not only to myself but to other individuals on the road necessitates prompt and decisive action to ensure the safety and reliability of affected vehicles.
