Many people have have issues with the SRS warning light coming on and not going off. This issue seems to stem from harness connection issues that Lexus is aware of and have acknowledged. This needs to be fixed asap!

I am detailing an alarming and persistent issue with unexpected brake activation on my 2013 Lexus LS 460. Over a span of time, the vehicle's brakes have been activated spontaneously on four different occasions, putting me and my passengers in potential danger. Incident Descriptions: 1. Location: I-69 southbound, near Huntington. - Scenario: Alone in the car. Noted a shiny truck about 2,000 ft ahead under a bridge with sun reflection. - Brakes activated hard for approx. 3-5 seconds. - Active cruise and lane-keep assist were on. 2. Location: Westbound on Interstate 70, just past the Indiana line. - Scenario: My wife, [XXX] , was with me. A semi-truck with wind skirts was being overtaken when the rear wheels locked up. - This caused a sideways skid toward the truck, causing tire smoke and chaos among trailing vehicles. - Managed to drive down to the median at high speeds, stopping just before a bridge. Multiple dashboard lights illuminated. 3. Location: 69 south of Anderson IN - Scenario: - On the way to the dealership, the brakes activated similarly as in the first scenario. 4. Location: Ohio on interstate 30 just before Indiana line... Not sure how far... - Scenario: After dealership repairs and 10,000 miles of incident-free driving, the brakes suddenly activated. - I was with my engineer, [XXX] . Reflective tapes on road barrels might have triggered the brakes. The car came to a full stop despite full gas pedal depression. Fortunately the closest vehicle behind us was approximately 2,000 ft back so I was able to get off the road at a slower speed of approximately 5-10 mph with full control all the way to stop. Once the vehicle stopped we were able to take right back off again with no further incidents to date. The contact owns a 2006 Lexus IS. The contact stated that the dashboard was melted and was causing vision impairment while driving. The vehicle was taken to the dealer to be inspected. The vehicle was inspected, and the diagnosis was confirmed. The contact was informed that the warranty had expired. The vehicle was not repaired. The manufacturer was made aware of the failure but provided no assistance. The failure mileage was approximately 92,000. The VIN was unavailable.

Dear manager: I have this 2006 LEXUS RX400H which has CHECK VSC SYSTEM and CHECK HYBRID SYSTEM problems. When I was driving on the highway, the cruise control button is unable to turn on, when at speed of 65 MPH it will suddenly stall in the middle of the road and the READY light will be off and the vehicle has to be stopped by the emergency lane which is REALLY DANGEROUS. I have searched online and found out there were many complains about this issue and LEXUS issued a recall for this type of car and the dealer can change the inverter and ICM free of charge. On Friday, September 15, 2023 3:00PM, I had a “horrible” experience with my Lexus this weekend and sudden unintended acceleration. I was sitting at a red light, with my foot on the brake. Without warning the engine began to rev, the tires squealed and the car lurched forward. I through the car into park & turned of the car as quickly as I could react and “thank god” no one was injured and there was no accident. I was in the car with my wife and she reminded me she had a similar experience a month ago. I didn’t know what to think when she told me. Until I experienced it myself I didn’t really understand how dangerous this problem is. She experience this sudden unintended acceleration when the car was in park after she got in the car and started it. I’ve read of this sudden unintended acceleration but never experienced it until this weekend. It was exactly this.. an unintended, unexpected, uncontrolled acceleration of a vehicle, accompanied by an apparent loss of braking effectiveness. This was not driver error. It is some kind of mechanical, software problem, electrical problem, or some combination of these factors. There was no driver error. This is not an issue with the floor mats. I drove the car home into the garage. I took it to the dealership the first available appointment on Monday. I had another incident on Wednesday, August 30, 2023. I was at the gym. My wife went to move my car as my parking time limit in La Jolla had expired. She experience sudden unintended acceleration when the car was in park after she got in the car and started it. The car had high engine revs and the car lurched forward while in park. The Lexus Dealership kept the car for a week. Mileage 67,249 Vehicle Report - [XXX]. Reviewed and analyzed by Lexus Factory. No recommended repairs are needed or required at this time. I opened TAS Case# [XXX] . I contacted Lexus Product Liability Team Case # [XXX] but, no one from corporate will look at the car. The contact owns a 2009 Lexus RX350. The contact stated that the dashboard had started to crack, causing the dashboard to become sticky. Over time, there was a glare coming from the dashboard which obstruct the contact’s vision while driving. The contact stated that there was condensation in the headlight housings. In addition, there was a puddle of oil was discovered leaking from the vehicle while parked in a parking garage. Due to the failure, the vehicle was towed to an independent mechanic where the engine oil cooler pipe was replaced. The manufacturer was notified of the failure and the contact was informed that each of the failures had been covered under warranty; however, the warranties had expired. The vehicle was not repaired. The engine cooler hose had been replaced. The failure mileage was approximately 125,000.

I purchased this vehicle in 2015 - it has been dealer serviced and has never been in an accident. At just under 90,000 miles on the odometer, a major fluid leak started. It turns out that the rear main (crankshaft) seal had failed, causing a leak of transmission oil. No warning lamps or messages appeared. This failure resulted in an extraordinarily expensive repair, over $2,500.00 at the dealer, Lexus of Greenwood Village (Colorado). It was not covered on warranty. This is a major safety issue - had I not noticed the leak, which occurred without warning, the engine could have seized on a highway, leading to a crash. This failed seal should be designed to last far longer than 90,000 miles. Premature failure of this kind is highly likely secondary to a defect in design or materials. I have complained to Lexus, which is unwilling to take any responsibility for this. Please investigate as to whether a recall is warranted, and whether the manufacturer should be responsible for the repair of this obvious defect. Thanks.

I purchased a new Lexus NX 350 on Oct 15, 2022. On Aug 7, 2023, with 6300 miles, my tire and wheel flew off while I was exiting a 5 lane highway. The tire continued rolling and went through the windshield of another vehicle. I was able to get the car off to the side of the exit ramp while riding on the rotor. The dealer claimed no responsibility even though the car had been in for its initial 5K service that included a tire rotation 1300 miles earlier. I asked Lexus Corporate to inspect the vehicle to protect others from a potential serious hazard. Lexus said the lug bolts were shorter in style and incompatible with the standard lug bolts. In addition the jack was missing. Why would I or anyone change lug bolts when the wheels and tires are still original? In addition, this SUV has 20” standard run flat tires which does not include a jack..

This is a brand new car and I was driving on the highway on a Sunday weekend on 9/24/23 while it was raining outside. All of sudden I started hearing noise from outside as if my windows were open. Then yellow triangle showed up on my head up display and on dash screen, it asked me to shift to parking mode. Minutes later dash screen shows my driving-side door was opened. There was no shoulder to park at the time while I was driving on the highway, so i stopped on the right hand side of the highway, shifted to parking mode, open the door and closed, and the warning sound and signal finally went away. I think it was the rain drop that has activated the Electronic door lock release on a rainy day, during this time, i heard wind noise, wind coming into the car, and warning alarm from car safety system.

The dashboard on my car is melting/shiny/sticky. I have tried many things to get it to stop reflecting on my windshield, but nothing helps. I contacted Lexus and this is an identified known concern for this vehicle and a bulletin has been issued, but the bulletin has expired. The reflection impairs visibility anytime the sun is out or lights at night reflect on the dashboard. I am concerned for my safety and my children’s safety in a vehicle that is known for reliability and longevity.
