2026 LEXUS TX- Fords, NJ
The vehicle is a brand new Lexus TX. The primary issue involves a brake system malfunction. While driving, a “BRAKE” warning message appeared on the dashboard. At the same time, the brakes were actively applied, and this occurred multiple times in succession during the same driving event. At the time of the incident, we were not aware that the vehicle should not be driven when this warning appears and drove the vehicle home. After reviewing the owner’s manual, we learned that the vehicle should not be driven under these conditions. The vehicle was then towed to the dealership for inspection. This event posed a significant safety risk. Repeated, unintended brake application while driving could result in sudden deceleration, loss of vehicle control, or increased risk of being struck by other vehicles, increasing the risk of a crash, injury, or death. Prior to this incident, the vehicle also experienced a complete failure of the camera/display system, where the screen went black, impacting driver visibility systems. The vehicle is currently at the dealership and under inspection by the manufacturer’s authorized service center. The issue has not yet been confirmed or reproduced at the time of this report. There were no prior warning messages before the brake warning appeared. The failure occurred unexpectedly. The component(s) responsible for the failure are currently unknown, pending diagnostic evaluation by the dealership. The vehicle is available for inspection upon request.

2006 LEXUS IS- Germantown, MD
My dashboard is melting and I understand that there was a campaign to resolve this that ended in 2016. I bought the vehicle in 2019 and it has only gotten worse to the point that the dashboard has a sheen to it that causes a glare on the interior of the windshield when the sunlight hits it, making visibility very difficult.

2007 LEXUS RX- Locust Grove, GA
I am reporting a serious and potentially dangerous braking issue with my 2007 Lexus RX400h. The vehicle currently has approximately 126,296 miles. While driving, the vehicle emits a continuous and intermittent warning beeping sound. At the same time, the braking system becomes unreliable. The brake pedal becomes extremely difficult to press, requiring excessive force, and the vehicle does not respond consistently when braking. In addition, there are instances where the vehicle appears to brake unpredictably or inconsistently, which creates a significant safety hazard for myself, passengers, and other drivers on the road. Based on my research, this issue appears to be related to a known defect involving the ABS brake actuator assembly. Lexus previously issued a warranty enhancement program for this component due to internal malfunctions that affect brake pressure and system performance. However, this issue was not issued as a formal recall tied to VIN eligibility. Despite my vehicle being under 150,000 miles, I have been informed that I may be outside the time-based warranty window, and no active recall is listed for my VIN. This defect directly impacts the braking system, which is a critical safety component. Failures in the brake actuator can lead to improper brake pressure, reduced stopping ability, and increased risk of a crash. Given the safety implications and known history of this defect, I believe this issue warrants further investigation and potential recall consideration. I am submitting this complaint to request that the NHTSA review this matter for safety risk and determine whether broader action is necessary to protect consumers.

2018 LEXUS LS- Ruffin, SC
The contact owns a 2018 Lexus LS 500. The contact stated that while driving 50 MPH, the vehicle stalled. The vehicle coasted and was steered over to the side of the road. The vehicle failed to restart. The vehicle was towed to the residence, where the oil was drained and the cap and filter were removed. The contact stated that there were metal shavings inside the oil. The vehicle was towed to a dealer to be diagnosed. The contact was informed that there was no oil in the engine and that the oil filter was not the correct oil filter. Additionally, the contact was informed that the engine had seized, and the engine needed to be replaced. The vehicle was not repaired. The vehicle was then towed to the residence. The manufacturer was notified of the failure, and a complaint was filed. The failure mileage was 36,000.

2020 LEXUS IS- Waldorf, MD
I am filing this updated complaint to supplement my prior complaint (NHTSA ID No. XXX, filed January 27, 2026) regarding a recurring, safety-critical engine defect in my 2020 Lexus IS300 RWD, 8AR-FTS engine. DEFECT: Three documented misfire events over seven weeks. The second (12/11/2025) and third (1/17/2026) each involved a temporary stall in active traffic requiring immediate evasive action. All events involved flashing MIL, reduced engine power, shaking, and abrupt deceleration. DIAGNOSIS: Authorized Lexus dealership—following 34-day retention with Lexus Technical Assistance (TAC Case XXX) and personal on-site inspection by a Lexus Field Technical Engineer—confirmed worn intake valve guide on Cylinder 3. Repair estimate: $10,355.61 subtotal per Service Bulletin L-SB-0007-21 ($10,671.72 total incl. shop supplies and tax). Estimate cites L-SB-0007-21 without mileage qualification, confirming technical applicability. TSB PATTERN — NO RECALL: Lexus has issued multiple TSBs on the 8AR-FTS engine: L-SB-0153-17 (September 2017) and L-SB-0007-21 (March 2021, revised April 17 and July 2, 2024), covering 2015-2024 model year vehicles—progressively widening applicability over three years. Both 2024 revisions occurred during the active warranty period. No recall has been issued and this consumer received no notification of any kind. A TSB is an internal dealer communication — not consumer notification. A CPO purchaser relying on Lexus's representations of comprehensive inspection and rigorous certification has a reasonable right to assume known defects were addressed, without searching technical databases. MANUFACTURER RESPONSE: Lexus declined to authorize repair under warranty or as goodwill and declined to confirm authorization in writing. REQUEST: Please investigate whether the pattern of multiple TSBs covering progressively wider applicability—with no recall despite safety-critical symptoms—warrants a formal investigation and escalation to a safety recall. While the car was idled at a stop light, the warning "reduce engine powere appeared". I took the car to the vehicle and learned this was linked to code P125d71 for fuel pump valve control- a known recall

2025 LEXUS RX- Comfort, TX
On April 1, 2026, while pulling into a parking space at our church, I was applying the brake when the vehicle suddenly and forcefully surged forward without warning. The unintended acceleration was intense and unexpected, occurring despite continuous braking input. This resulted in the vehicle striking the building and causing significant damage to both the vehicle and the structure. This incident has occurred twice within a 10 day period, both times during low-speed parking/stopping maneuvers. There were no warning lights, alerts, no automatic emergency braking engagement, no air bag deployment, or prior indications of malfunction. I did report this to the police and they filed an Incident Report. Lexus was notified and an independent analysis engineer examined the data from the vehicle's CDR System. This report was sent to me and to Lexus.

2025 LEXUS GX- Berwyn, IL
The contact owns a 2025 Lexus GX 550. The contact stated that while driving at various speeds, the hood of the vehicle was fluttering up and down. The vehicle was taken to the dealer; however, the vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 4,865.
