2019 LEXUS NX- Simi Valley, CA
The contact owns a 2019 Lexus NX-300H. The contact stated that while driving uphill at an undisclosed speed, the vehicle stalled. The failure was a recurring failure. The dealer was contacted, and the vehicle was diagnosed. The contact was informed that there was coolant leaking into the engine block and that the engine needed to be replaced. The contact associated the failure with NHTSA Campaign Number: 20V064000 (Engine and Engine Cooling); however, the VIN was not included. The manufacturer was not made aware of the failure. The failure mileage was 103,755.

2013 LEXUS IS- Indio, CA
2013 Lexus IS 350C with approximately 56,000 miles experienced engine smoking on startup. The vehicle has been regularly maintained according to the manufacturer's recommended service schedule. The Lexus dealership diagnosed oil entering the combustion chamber and documented that the cylinder walls are scratched, allowing oil to pass the oil-control piston rings. Based on the internal engine damage, the Lexus dealer has recommended complete engine replacement. I am concerned that this type of significant internal engine damage at only approximately 56,000 miles could result in progressive oil consumption, loss of engine performance and potentially engine failure while the vehicle is being operated. The vehicle has not been abused or subjected to unusually high mileage and has received regular maintenance. Lexus corporate was contacted regarding the premature engine failure and a request for goodwill assistance was denied. I am submitting this complaint because I believe an engine requiring complete replacement due to scratched cylinder walls at approximately 56,000 miles warrants review to determine whether similar failures involving the piston rings, cylinder walls or oil entering the combustion chambers have occurred in other Lexus vehicles equipped with this engine.

2010 LEXUS HS- El Dorado Hills, CA
The contact owns a 2010 Lexus HS-250H. The contact stated that while driving 10 MPH, the Hybrid failure message was displayed. A dealer contacted. The vehicle was taken to the dealer where it was diagnosed and determined that the battery needed to be replaced. The battery was replaced, and the vehicle was repaired. The manufacturer was notified of the failure and informed the contact that the warranty had expired. The approximate failure mileage was 119,700.

2012 LEXUS IS- Topanga, CA
The dashboard (instrument panel) on this 2012 Lexus IS 250 has degraded and deteriorated, developing a shiny, reflective surface. In daylight and bright conditions, this degraded surface reflects onto the windshield and produces a significant glare that obstructs the driver's view of the road. The glare materially impairs forward visibility and makes safe operation of the vehicle dangerous, particularly given the driver's reduced tolerance for compromised visibility. This is a known and widespread defect. Lexus formally acknowledged the cracking/melting dashboard condition through Customer Support Program ZLZ, which covered approximately 584,800 vehicles. The defect is understood to result from the dashboard material breaking down over time due to heat exposure. Numerous owners of affected Toyota and Lexus vehicles have reported the identical hazard to NHTSA: a degraded dashboard casting a glare on the windshield that impairs the driver's view. In this vehicle, the condition did not manifest until 2025–2026 — after the support program's coverage window had closed — which is consistent with the latent, heat-driven nature of the defect. The owner contacted Lexus and requested repair. Lexus declined, stating the vehicle was outside the program's coverage limits, and on escalation confirmed it would not take responsibility. The owner is filing this report because the condition is an active safety hazard affecting road visibility, not merely a cosmetic issue, and because Lexus has declined to remedy it despite having formally recognized the defect across hundreds of thousands of vehicles. No crash / no injury / no fire (assuming that's accurate — the form will ask; answer honestly).

Air conditioner condensers being subject to road hazards, rocks/debri. Causing to lose all refrigerant. Multiple claims on Lexus forums. Seems to be a design flaw.

2008 LEXUS RX- Huntsville, AL
1. Ignition System: While driving the key in the ignition lock came out loose! Exremely frightened and managed to pull the car over to a driveway. But, the car could NOT be stoped using the key fob. Key will not work to stop the engine. Very unusual and unexpected incidence. 2. Lighting system switch: Suddenly the knob of the light switch came out loose with a plastic shaft and will not turn on the light. Very unsafe while driving and it became dark due to storm and rain, and I wanted to turn the headlights for safety. I beleive it is a manufaturing defect and causes severe safety issue.

2023 LEXUS LX- Austin, TX
I received notice over a year ago that the engine in my Lexus was defective and failure could cause damage, injury or death because the engine would seize up. We drive in rural areas, often outside of cell service. We are [XXX] and to be stranded, especially at night, would be very dangerous. I have made several calls to dealers and Lexus in Plano, and no one is willing to do anything about the problem they have created. They offer nothing. 2026 LEXUS TX- Maple Grove, MN
We were traveling 80 miles west of Fargo ND on I94 when we heard a loud bang. It became clear immediately from the wind noise that the sunroof had broken. We did not hear or see that anything was dropped on it. Thankfully the shade was closed. We pulled off at the next possible stop to inspect and noted a large whole in the sunroof and that the glass bowed outward suggesting the explosion was an outward force rather than an item falling on the car. There were no warning lights etc. prior to this happening. This was a very frightening experience and had the shade not been closed our family including young son could have been hurt or even blinded from the falling glass. We called the dealer (we only had the car for 3 weeks) and they were able to get the car in right away to look at it but cannot guarantee the window is covered under warranty.

2023 LEXUS RX- Overland Park, KS
had transmission replaced 2 times and update to transmission. this is a brand new car purchased in 2023. first transmission replaced at 20,963 miles, 2nd replaced at 33,543 miles, 35,221 miles transmission update. It was cumulative 33 out of service. We feel this vehicle is unsafe to drive. Even after all the transmissions replaced om the is vehicle had 2 more incidents. One was loss of power in traffic and another was on a highway transmission would not shift into higher gear. This is not new there have been hundreds of people with the same exact problems. Lexus has internal TSB L-SB-0059-23 detailing these problems.

2024 LEXUS RX- Maumelle, AR
While on a long road trip on a hot day, on the interstate, the car suddenly lost speed and would not accelerate. It was like the car had shifted into neutral. I was coasting on the interstate, steadily slowing, and was forced to pull over to the shoulder. I turned the car off and waited a couple of minutes then turned the car back on. No error messages were displaying at that time. About 5-10 minutes later, I pulled over for a planned stop. My husband assumed driving responsibilities when we got back into the car. Upon trying to get back on the interstate, the car would not accelerate, and multiple errors error messages appeared, including: ""System Malfunction/Visit Your Dealer," "Check Engine/Visit Your Dealer," "Drive-Start Control Malfunction/Visit Your Dealer," "Secondary Collision Brake System Malfunction/Visit Your Dealer," and "Traction Control Turned Off." He pulled into a nearby parking lot where we had to have the car towed to the dealership. It felt like the car was in neutral when this happened both times.
