2024 MAZDA CX-90 PLUG-IN HYBRID- Haddonfield, NJ
Mazda Radar Cruise Control - while on [XXX] and passing a truck that is in the right-hand lane, especially on curves, my CX-90 PHEV system brakes, but there is no vehicle in front of me. It has difficulty determining what is in front as opposed to on the side, especially through curves. This happened more than once. I almost got rear-ended due to this issue. 2024 MAZDA CX-90- Bethlehem, PA
I was traveling at 40mph on August 14 when the engine stopped running suddenly and without warning. I lost all power. I tried pressing the gas pedal and the engine shuddered and sputtered with the speed ranging from 0 mph to about 18mph with no steady duration longer than a second or two. At the same time an engine icon appeared and a message displayed reading engine malfunction. I safely exited the travel lane despite having very little power. I waited 10 seconds and restarted the car and everything worked fine. I called the dealer explained the incident and to made arrangements to have the vehicle serviced. The appointment was for Friday. When I asked whether I should continue driving the car. I was told that so long as the engine icon was not flashing the car was ok to drive. The next day I started the car and did not see any warning message or engine icon. I thought all was ok so I cancelled the appointment. A second incident occurred later in the morning. I called the dealer again and was told to bring the car in that afternoon, Thursday. On the way over to drop off the car a third incident occurred with the same characteristics as the first two. Each time waiting 5 to 10 seconds restored power to the car. One week later the car was returned to me with the diagnosis of low fuel pump pressure problem. I am wary of driving this car on any trip especially on a speed highway for fear this situation will occur when I am driving at 70mph. I appreciate your taking the time to read this description of a very scary and unnerving situation that no one should face.

2024 MAZDA CX-90 PLUG-IN HYBRID- Holtsville, NY
The contact owns a 2024 Mazda CX-90. The contact stated that on several occasions while driving at various speeds, the message "Vehicle Failure Detected - Hybrid System Malfunction", "Possibility of Restricted Power Output", "I-Activsense System Malfunction", "Dynamic Stability Control Malfunction", and "Push button Start System Malfunction" were displayed. Additionally, the I-Activsense warning light was illuminated. The vehicle was taken to a dealer where it was diagnosed that an unknown software needed to be updated. The vehicle was repaired but the failure reoccurred. The vehicle was taken back to the same dealer where it was diagnosed that the connector to the battery needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 3,960.

2024 MAZDA CX-90- Norfolk, MA
On 3 separate instances the car starts to buck, speed dropped from 75mph to 50mg in first instance. Then the engine malfunction light comes on. The engine looses power and will only drive at 30mph. Once pulled over and shut car off and restart, attempt to drive again with zero power from the engine.

2024 MAZDA CX-90 PLUG-IN HYBRID- Fuquay Varina, NC
July 11, 2024 – Odometer – 7,604, Gas – Full, Electric – 19 Miles range - From work to Home, Receive the following errors “Engine Malfunction, Have Vehicle Inspected” and “Hybrid System Malfunction, Have Vehicle Inspected” warning lights come on. Cars behavior – Air conditioning motor running but not cooling air, Loss of power and car very sluggish (have to be careful at intersections), seems to maintain speed even when taking foot of gas and requires extra breaking, seems to be clunky when transmission upshifts/downshifts at times. When leaving work, did not seem to like being in EV mode even though fully charged (Chargers at work) and would switch itself out of that move and say unavailable. Outside air temperature 90 degree (Air Conditioning not working, very uncomfortable). This defect is dangerous and I serious concerns driving this car such as unpredictable loss of power when entering traffic and hesitation / unpredictability when hitting the accelerator. In addition, with the car maintain speed while not touching the gas pedal and requiring extra breaking is a scary. June 13 – Issue cleared and wanting lights were gone before I was able to take it in for service. July 25th – Recurrence of the same issue - Odometer – 8,162, Gas – Full, Electric – 14 Miles range - From work to Home, Receive the following errors “Engine Malfunction, Have Vehicle Inspected” and “Hybrid System Malfunction, Have Vehicle Inspected” warning lights come on. 2024 MAZDA CX-90 PLUG-IN HYBRID- Peyton, CO
On July 15, 2024 this vehicle at 50 mph on cruise control suddenly flashed white warning words indicating problems with the cruise control and immediately thereafter started flashing “hybrid malfunction” and to return to service dealer. At the time of the hybrid malfunction warning which was right after the quick cruise control warning, the engine operation, specifically the gas pedal no longer delivered power to vehicle, either electric or gas. Additional warning was to pull off to the side and stop and turn off vehicle. This occurred and vehicle was pulled to the side put into park and turned off. Waited approximately 3 to 5 minutes and restarted the engine which started in gas with no electric although there was electrical charge on the battery. Was able to continue to our final destination approximately 15 minutes later and no more than 10 miles driven. Hybrid malfunction was still visible on the dash to include the warning stating that vehicle needed to go to the service department. Approximately an hour later, vehicle was turned on again with the same warning about the hybrid malfunction, however, engine started in electric. Drove vehicle to dealership and stated what happened. Dealership the next day stated that this was a phantom code known by Mazda, but no problems associated with the code. Vehicle picked up per service advisors request two days later. Mazda dealership stated that headquarters had eyes on this particular vehicle. A day later after the pick up Mazda dealership requested vehicle be brought back for a “DC box” repair. Dealership stated this process would take a week or thereabouts to replace this component. Vehicle currently in dealership service.

2024 MAZDA CX-90- Minneapolis, MN
The driver's side mirror vibrates excessively when traveling on the highway or at higher speeds (50+ mph). This has been present ever since taking delivery of the new vehicle on [XXX] and poses a safety risk as it compromises visibility to adjacent traffic. The problem was confirmed by the dealer on [XXX] and repaired according to manufacturer TSB 09-018/24. However, the problem continued to persist after the repair. The defect was reported again on [XXX] and the dealership claimed that they contacted the manufacturer where no additional fix was available. The defect was reported yet again for a third time on [XXX], at which time the dealer still claimed that there was no updated fix. Going down the road with cruise control I have been used to it dropping a little bit in curves but today and car behind me had to slam on there brakes because the cruise control in a small curve dropped me down immediately from 60 to 45 jerk and all. If the driver behind me had not been paying attention they would have slammed into me since we were both going 60 no other cars around just a small curve on a straight road

2024 MAZDA CX-90- Colonia, NJ
The contact owned a 2024 Mazda CX-90. The contact stated that while driving at various speeds, a clunking noise would emit from underneath the engine compartment. The vehicle was taken to a dealer where it was diagnosed that the control arm had failed. The vehicle had been repaired. The contact stated 3 days later while driving approximately 35 -45 MPH, the brake pedal was depressed but failed to respond. In addition, the vehicle was attempted to be veered to the left or right and it failed to drive in either direction. As a result, the vehicle crashed into the rear-ended of the vehicle in front. The air bags deployed. The vehicle was towed and deemed totaled. The contact stated that the other party's vehicle was damaged and towed away. A police report was filed. The contact sustained a left leg, right arm, shoulder, wrist, and neck injuries that required medical attention via hospital. The contact's wife was seated in the passenger seat at the time of the crash and sustained injuries to the right shoulder, wrist, and both the left and right foot's toes that required medical attention via hospital. The contact was unsure of any injuries to the occupants of the other vehicle. A dealer was contacted. The vehicle was not diagnosed or repaired. The contact received notification of NHTSA Campaign Number: 24V349000 (Forward Collision Avoidance) however, the part to do the recall repair was unavailable. The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. The manufacturer had been made aware of the failure and opened a case. The failure mileage was 23,209. VIN tool confirms parts not available.

2024 MAZDA CX-90- Beachwood, OH
I was pulling into a parking spot. When I do this, my foot is always on the break. I'm an experienced driver (30+ years) and have never had an incident when I felt the car accelerate without my initiation. When I was maybe 5-7 ft from the car in front of me, the car suddenly accelerated (like it was possessed). I immediately slammed on the break and turned the wheel. Thankfully, and amazingly, I avoided hitting the car both in front of me and immediately on my left (the direction I swerved the wheel). No warning lamps, actions, etc were taken by the car itself (other than willing me to smash into the car ahead of me). This incident occurred in a school parking lot. Thankfully, there was no harm to anyone but my confidence in Mazda. But, if I had chosen to pick my children up through the pick-up line, I may have easily hit the car in front of me or a child! Obviously, this can be a tragic situation. Prior to this incident, I have felt that the car regularly does not operate smoothly at slow speeds, especially when accelerating from stop. This is my 3rd Mazda, and I have never felt this lack of control before. I have also noticed that the i-stop works erratically, including when I am moving at 10-30 mph. The light goes on and off when I am slowing down at a light, etc. Plus, when I am stopped at a light, the i-stop will go on for about 20 seconds, then suddenly turn off and the engine will rev up. I shard the stoplight information with the dealer the week after I took possession of the car and was told by the sales manager that this was just normal and a weird part of the electronics of the car. Following this incident, I called the dealer. The service department and manager said that they weren't aware of this issue but took my concern seriously. I then did an online search and found a conversation string on [XXX] with others sharing the same problem.
