2016 MAZDA CX-5- PARKER, CO
My son was driving the car when it suddenly lost power and would not shift correctly. This occurred without warning while in moderately fast traffic that was moving at 40-50 mph. There were no warning lights illuminated prior to the problem occurring. Upon reviewing trouble codes, it is likely that the low pressure fuel pump located in the fuel tank has begun to fail. When the tank gets low on fuel, about 1/4 tank, the low pressure fuel pump is unable to supply enough fuel to the high pressure fuel pump. This results in sudden loss of power, and also hard starting. I believe the fuel pump is the problem because it was manufactured by Denso, who recalled 2,000,000 similar fuel pumps two years ago. Those pumps were recalled because they created the exact problem we experienced. Toyota recalled vehicles using these pumps as far back as 2013. However Mazda and other OEM's were allowed to only recall vehicles going back to 2018. EVEN THOUGH IT IS THE SAME PUMP. When the manufacturing defect that causes a safety problem is well known, why are some manufacturers allowed to limit their recall? Why aren't all cars using the pump forced to be recalled? If a recall is excessive, why aren't manufacturers required to provide an extended warranty? NHTSA, you dropped the ball on this, and you need to fix it.

2022 MAZDA MAZDA3- Canandaigua, NY
Purchased vehicle new at dealership April 15th 2022 w/ 17 miles on the odometer. After 500 miles on the odometer, began receiving multiple daily warnings through the MyMazda app indicating airbag/seat belt pretensioner malfunction. Subsequently noticed the same warning lit up on the dash while driving for approximately 10 seconds and then light went out. Discovered technical service bulletin dated 317/2022 no: 08-001/22 indicating this a known problem for Mazda 3 vehicles between 2019 and 2022. Showing design flaw in path layout for wire harness connecting to side airbag module in seat back. Recommends modifying plastic module cover by removing several inches of plastic, several inches of foam padding, and pinning section of wire harness to steal seat frame with plastic grommet. This is a clear safety hazard concern that should have been addressed before I was allowed to drive the vehicle away from the dealership. Mazda's position is not to address the issue until they receive a customer complaint. If I had been in an accident I may have been severely injured or killed because of this malfunction.

2017 MAZDA CX-5- SAN DIEGO, CA
Mazda has termed the issue Ghost Touch in their CX-3 models, which the NTSB issued a Technical Service Bulletin. It is dangerous as the infotainment system begins to switch between functions on its own, without driver or passenger input, and while the cars is in motion; changing radio stations, dialing phone numbers, turning the system display to Night Mode making it impossible to read while using the vehicle navigation system, exiting navigation maps altogether. The part number given to me by Mazda is K123611J0C. The dealer diagnosed and confirmed this issue in April 2022. They stated it is a common problem but it is not covered under warranty. Replacement costs quoted was over $1400, plus $180 to diagnose the problem.

2013 MAZDA CX-5- Panama City, FL
When you shut the car off the radio and other components do not shut down. You have to start the car back up and put in gear then back in park and shut the car off (sometimes two or three times) to get everything to shut down. This is getting worse over time.

The TCM - Transmission Control Module defected - gear lights disappear, down shifting problems, driving, stop in the middle of highway or road while driving, kick / jerk when ATF lights comes on goes off then check engine lights comes on all fluids has been changed drove good for a few days started again took to advance auto parts and auto zone codes stated electrical TCM computer issues, delayed shifting, check engine and ATF lights comes on , DTC codes U1000, LOST COMMUNICATION WITH TCM. The TCM overheats because it is sitting directly on the transmission, under the battery box. My child safety was put at risk daily. This car fails to stop at a traffic light, a slow response when approaching a stop sign or traffic light is dangerous and scary. Mazda's are not safe to drive with a bad TCM, When a driver cannot anticipate the acceleration or deceleration of this vehicle or any Mazda with a TCM the situation is very dangerous. 2017 MAZDA CX-9- Commerce Twp, MI
On several occasions, the system would activate the panic braking while driving at low speeds in a few different situations. One situation is while pulling into a 2 car garage with objects along the garage walls, i.e. shelves in front of vehicle, lawn equipment on the side, grill on opposite side. While entering the garage the system would engage well before the front of vehicle is close to the shelving. Another occasion happened in a parking lot while driving at low speeds with no other vehicles or objects near the vehicle.

2015 MAZDA CX-9- Northbridge, MA
On December 14, 2015, I bought, a Mazda CX-9, at Sentry West Mazda located at 940 Boston Turnpike, Shrewsbury MA 01545. Unfortunately, the car's engine contains a defect in design, manufacturing, materials and/or workmanship that causes the water pump to suddenly and prematurely fail well-before the end of the useful life of the engine, which leads to catastrophic engine failure or a costly replacement due to the water pump’s inaccessible placement in the engine. On March 7, 2022 while driving the car to the dealer to take care of a Check Engine Warning Light, the car started to shake, all the dashboard lights turned on and the car stopped. My car was immediately towed to the dealer, and hours later I was told that my car needed a new engine. I felt paralyzed with awed when I heard that my 2015 Mazda CX-9, which was just 6 years old with no more than 90,000 miles, needed a new engine. I have been taking this vehicle to the Mazda dealer for all its repairs and maintenance needs, so I can be confident that my family were safe in this car. Unfortunately, this has not been the case. In addition, please note that there is a pending litigation against Mazda for this kind of Engine. Please see more information in this link. https://www.ktmc.com/mazda-water-pump-engine-failure I strongly believe that a car like this is a threaten to the life of anyone driving in and to the people around it. Therefore, I am asking to this organization to take immediately action on this case. Thanks in advance for your attention to this matter, [XXX] INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).
