2023 MAZDA CX-50- Chicago, IL
1) Climate Control System Malfunction I am reporting a safety-related defect involving the climate control system in my vehicle. Specifically, the temperature control knob is malfunctioning and behaves erratically. When adjusting the temperature, the system does not respond consistently and will unpredictably change temperature settings without corresponding input. This was diagnosed at The Autobarn Mazda of Evanston to be a failure within the HVAC control system. I see it is a common issue discussed in online forums. The issue has been diagnosed and confirmed by a service technician. At this time, the vehicle has not been fully repaired and remains available for inspection upon request. This malfunction creates a potential safety risk because it may cause a distraction while driving. The erratic behavior requires repeated adjustments and attention to the climate control system, diverting focus away from the road. The problem has been reproduced and verified by a dealership technician, who confirmed the abnormal operation of the temperature control knob and system. A repair was recommended, with replacement cost $690.24. The vehicle has been inspected by an authorized dealership, and I also contacted Mazda to request goodwill repair assistance. However, they declined to cover the repair cost. My car has 55,000 miles, but this is an issue that has been present early on if not since day 1. Prior to the failure, there were no consistent warning lights or system messages indicating a problem. Given that this appears to be a known issue with this type of system, I am concerned that other vehicles may be affected by the same defect.

2018 MAZDA CX-5- Jacksonville, FL
I received notice from Mazda about a recall on the infotainment system. The notice required me to provide as much proof of claim as possible. I provided proof from the dealership where the system was replaced. I was within filing time guidelines. Mazda administrator sent notice that my claim was denied with no reason as to why. Claim # XXX VIN [XXX] I am requesting the full amount of parts and labor that I spent for the replacement of the infotainment system. 2025 MAZDA MAZDA3- Orlando, FL
I am reporting a catastrophic failure of the rear seatbelt restraint system in my 2025 Mazda 3. The rear seatbelt guide geometry is badly designed, allowing the webbing to bunch and jam during normal operation. This physical obstruction prevents the seatbelt from retracting, leading to three severe safety hazards: 1) Total failure to restrain occupants. 2) Structural damage to the vehicle's safety cell. 3) Public hazard where the obstructed door rebounded open into oncoming traffic. Restraint Failure: On multiple occasions, the rear seatbelt jammed in the guide clip while in use. Due to the lack of retraction, a 200lb adult passenger was able to fully exit the vehicle while the belt remained buckled. Most alarmingly, the vehicle's safety logic failed to trigger any occupancy warning alarms. The system remained "satisfied" because the buckle was engaged, despite providing zero tension or restraint to the occupant. Structural Damage & Public Hazard: Because the belt failed to retract, it remained loose and obstructed the rear door's closing path. The door rebounded off the slack belt into oncoming traffic, resulting in a significant structural dent to the C-pillar (door jamb) immediately adjacent to the striker. This deformation occurred in a critical area for side-impact protection. Efforts to Resolve: The vehicle was inspected by a dealer, who claimed the system was functioning normally despite being shown the jamming. I escalated this to the Mazda North American Operations Executive Office. The executive team issued a final dismissal, refusing to provide technical answers or acknowledge the documented failure points. Crucially, they denied my request for a Regional Technical Manager (RTM) to perform an engineering review of the guide geometry and refused to inspect the damage to the C-pillar. 2025 MAZDA CX-5- York, PA
This vehicle was purchased two months ago. In that time the windshield has sustained multiple significant chips on multiple occasions during routine highway commuting. Some of these are in the line of sight of the driver, causing distractions or visibility issues. Prior to purchasing this vehicle I owned a 2012 Toyota Corolla, which traveled the same route every day and did not receive any significant windshield chips over a period of years. It seems that the windshield of the Mazda is particularly fragile and/or brittle compared to other windshields and suffers chips and cracks too easily.

2019 MAZDA CX-5- albuquerque, NM
Cylinder head is cracked and leaking oil. The Cylinder head has an active warranty extension but Mazda only applies this to coolant leaks and not oil leaks. The root cause is a casting defect along with a exhaust manifold design issue that imparts undue stress onto the cylinder head causing cracks. The limitation of the warranty to the type of fluid that leaks rather than the failure due to design issues with the exhaust manifold and cylinder head does not make sense given that both components have been redesigned due to the prevalance of both coolant and oil leaks from cylinder head cracks. Problem has been confirmed by Quality Mazda Service departmetn on April 4, 2026 but no warranty coverage extended to repair the failure due to the fluid leak being oil and not coolant. No warnings or messages given prior to leaks. Leaks began at roughly 76,400 miles.

2024 MAZDA CX-90- Edinburg , TX
This issue I’ve seen on different forums just happened to me. Lower radiator hose leaking. Car is at 37000 miles and under no circumstance should this be happening to a new vehicle, Mazda has known about the issue apparently in my model year the manufacturing over tighted the clamp causing it to fail prematurely. This can be catastrophic especially since these leaks are just waiting to happen. If someone was on the highway it would be dangerous

2025 MAZDA CX-5- Princeton, NJ
Life-threatening, unoccupied rollaway vehicle with personal injury. Possible and/or suspected issues: "Not-P" switch circuit malfunction; "false park"; lack of cross-module redundancy; lack of BCM and PCM redundancy; Diagnostic Blind Spot; lack of Occupancy-Brake interlock; systematic safety failure culminating in unintended, driverless, runaway Zombie vehicle with personal injury. Background Info: * Low mileage, mainly in-town errands driving. * Not previously reported quirks, possible latent defects: liftgate opening that was hit-or-miss requiring multiple unlock trunk-unlock doors + get out of the car manual attempts; intermittent off-shutdown double-tap issue to power off car; ill-timed "rear seat alert". * Reported "engine won't start" resulting in three key FOB battery replacements and a locked key FOB. Day of Incident: * Auto-lock and the EBS were not used. * Driver intent was to park and unload, then permanently park. Car was in-park twice before ultimately placed in reverse and backed into position, facing downhill on a slight slope and along a left-side stone retaining wall. * Driver thought vehicle was fully in Park. * Attempted open liftgate via "open trunk, open door" routine, which did not work, so I exited the vehicle to try the manual liftgate button, which also did not work. * Three back-and-forth "unlock trunk/doors + manually press liftgate button" did not work. * Note: Upon opening the door or anytime after, I did NOT hear an audible warning that the car was not fully in Park. Nor did the car move/creep in any direction. * Deducing I must need to shut off the car to open the liftgate -- I leaned in and depressed the on/off button. * This caused the car to move forward, driverless, door open and scraping the stone wall, across a secondary road, over an 11" curb, and down a hill before stopping due to a tree. * I was injured. Pinned between a narrowing door and the car, I tried to turn the wheel right before being knocked down/falling

2024 MAZDA CX-90- Shingle Springs, CA
The steering sticks both left and right while driving. The danger of overcorrection and/or running off the road. This has not been looked at by anyone yet. I know this is a reportable defect but it's not attached to this vehicle yet. No warning lights. This was noticed within the first week of owning the vehicle (second owner).

2021 MAZDA CX-5- Caledonia, WI
2021 Mazda CX5 AWD- Non Turbo Engine. Sunny Day, Dry. My Car was clean. No Precipitation. Some shadows of the interstate light poles across the road 200-300 feet in front of me, but they were thin shadows. Sun Was to my Left. I was driving South on May 01 2026 about 10am on the Interstate going Highway Speeds, with adaptive cruise control "on", in the 2nd lane of a 4 lane interstate. Thankfully no other traffic ahead or behind me. Car slammed on the brakes suddenly, pretty hard. I counteracted and added more gas pedal to keep in going. I turned off the Cruise Control. After a while I re-engaged the Cruise Control and had no issues. This could have been a huge accident in busier traffic. Will get the car checked out at Mazda Dealership Service this week.

2018 MAZDA CX-5- Round Rock, TX
another case of a 2018 cx-5 cracked cylinder head. 70k miles and regularly maintienced. no warning other than oil showing on the ground. took it in to mazda dealership and they came back after looking at it and declared it was a cracked cylinder head priced at 6.8k. after looking it up it appears to be a casting issue that has affected many mazdas but mazda corp. will not take fault.
