While driving on the highway in traffic, I was applying the brakes (slowly and smoothly) and moving with the flow of traffic when the SBS system activated the brakes harshly and slammed me to a stop without reason. In addition to throwing me forward against my seatbelt, it also resulted in the car behind me rear-ending me. I did not hit the car ahead of me, since I was far enough behind them BEFORE the smart brake support engaged, but the feature caused an accident that could have been avoided.

    - Cambridge, MA

On multiple occasions while driving in rain/snow/ice, the vehicle will initially throw an "i-ACTIVESENSE Partially Disabled Due to Camera Malfunction" error. This initially disables adaptive cruise control, lane keeping, blind spot detection, etc. This error immediately then cascades to throw a "Dynamic Stability Control Malfunction" error as well as an "ABS Malfunction" error, suggesting that both dynamic stability control and ABS are disabled. This issue has occurred roughly 50% of the time it rains/snows, and usually persists for 1-2 days even after fully clearing the windshield where the front camera system is located. This has caused me to have to drive the vehicle several times now in the rain and snow without ABS and dynamic stability control active -- this is a serious safety issue. It is understandable for a front camera malfunction to disable sensing features (like lane keeping), but this should not disable necessary safety features such as ABS and stability control. It is clear that there is a cascading sensor malfunction in this vehicle (possibly related to an earlier similar recall in 2024 models), that causes serious safety concerns.

There is an intermittent issue where the dashboard will indicate that the rear side radar is obscured and turn off all the safety features of the car. This happens typically on a clear day when there should be no issues. Turning off the car resets this. Its unsettling because as there are quite a few blind spots in this vehicle you are dependent on the blind spot warning, especially in heavy traffic. Reported it to dealer but they indicated since its intermittent there isn't much they can do. Ridiculous for a new $45k vehicle

My driver side front headlight fan is malfunctioning. It stays on and makes a loud noise when the car is parked and off and the lights are still on. This is a known defect and my dealership is asking for 2k to pay for it. I am concerned the light will go out causing safety concerns. (TSB) SA01323 and 09-015/23. This should be recalled as is it not a general wear and tear issue, it is a manufacturing issue. My car is out of warranty but this should still be covered.

2025 backup camera not working with 8,500 miles on the car. This is a safety hazard and so far neither Mazda of Everett who broke it will not fix it unless I pay just about $1,300. They claim I have to talk to Mazda to see if they will warranty the part even though I have a 4 year 48,000 mile bumper to bumper warranty. There is no damage to the car what so ever and it has been inspected by the dealer since new with a 190 point inspection at 3,700 miles and never a problem with the camera. EITHER THE BACKUP CAMERA IS A BAD DESIGN THAT DOES NOT ACCEPT NORMAL USAGE OR THE DEALER DOES NOT TAKE SAFTEY FEATURES SERIOUSLY and broke it when washing the car.
