2024 MAZDA CX-90- Rowlett, TX
While driving the 2024 Mazda CX-90 and making turns the steering wheel become difficult to straighten/reduce deflection from the turned position. This has caused the vehicle to depart from our lane in traffic on the freeway multiple times. This has caused safety risk for ourselves with traffic barriers as well as with vehicles in other lanes. It is possible to over power the “sticking point” however it seems to be getting worse. A mazda service center has been contacted and they denied that they are required to remedy this because they already completed the recall for this issue in the past. The vehicle is available for inspection. The issues seems to be somewhere in the steering linkage or steering wheel mechanism based on the feel of the issues.

2018 MAZDA CX-5- plantsville, CT
Vehicle Information Year/Make/Model: 2018 Mazda CX-5 Engine: 2.5L Mileage at Failure: Approximately 115,000 miles Component: Engine Complaint Summary: I am reporting a sudden and unsafe engine failure involving a cracked cylinder head in my 2018 Mazda CX-5. This failure occurred without warning at approximately 115,000 miles and resulted in significant oil leakage from the engine. Description of the Failure and Safety Risk: While operating the vehicle, I detected a strong burning oil odor followed by visible smoke coming from the engine compartment. Upon inspection, it was determined that the cylinder head had cracked, allowing engine oil to leak onto hot engine and exhaust components. This condition presents a serious safety hazard, including the risk of fire due to oil contacting high-temperature surfaces. Notably, there were no warning lights, messages, or prior indicators of any issue before the failure occurred. The vehicle has been properly maintained and has not experienced overheating or misuse that would reasonably lead to this type of failure. Repair Information: The vehicle was diagnosed by an authorized Mazda dealership, which confirmed a cracked cylinder head. The estimated cost of repair is approximately $5,130. Additional Information / Defect Pattern: Through independent research, I have identified multiple reports of similar failures involving Mazda 2.5L engines in comparable vehicles. These reports describe nearly identical symptoms, including sudden oil leakage, smoke, and cracked cylinder heads at relatively low mileage. This pattern suggests a potential manufacturing or material defect rather than normal wear and tear. Manufacturer Response: I have contacted Mazda Customer Experience and escalated the matter. Mazda has declined to provide any assistance, citing the vehicle being out of warranty, despite the nature of the failure and associated safety concerns. 2007 MAZDA CX-7- Cleveland, TX
One of the Prinx HiCountry HT2 tires (size 235/60R18) developed a structural failure shortly after installation. The tire repeatedly lost air with no visible puncture. I first took it to a local tire shop, which could not locate the source of the leak. The problem continued, so I brought the tire to a second independent tire shop, where a technician inspected it and confirmed the issue was caused by a thin steel belt wire protruding through the tread near the shoulder area. They marked the defect with white tire chalk and determined the tire was not repairable under safety guidelines. The failure originated from inside the tire, not from an external object. The tire is still in my possession and available for inspection. No warning lights appeared prior to the failure; the only symptom was repeated pressure loss.

2016 MAZDA CX-5- Nashville, TN
My screen begins to change the display without me touching it. It has interrupted my GPS directions, causing me to get lost. It has gotten worse over time, now causing my back up camera to either not display at all or become partially blocked by another application randomly starting on the screen. My emergency brake lights flash on my dash, and auto technicians tell me it is a malfunction with the electrical system, not something in my brake system that needs to be replaced. We have tried that. These problems I have been told are not the responsibility of the manufacturer. I don’t understand why. It is distracting, alarming, and because it is getting more frequent with more systems being affected, it puts the driver in danger. This is not due to wear and tear - it is due to a faulty electrical program.

2025 MAZDA CX-50- Marietta, GA
I am reporting a potential safety-related defect in my 2025 Mazda CX-50 Preferred, which is approximately four months old. On one occasion, the vehicle moved backward while in Drive mode. This occurred twice on the same day under similar conditions on a slight incline. After starting the vehicle, I shifted into Drive and applied acceleration; however, the vehicle moved backward instead of forward. The rearward motion continued despite active acceleration input. This behavior is not consistent with normal rollback on an incline. The continued backward movement under acceleration indicates a possible issue with transmission engagement, gear recognition, or electronic control systems. I reported the issue to an authorized Mazda dealer. I was informed that shifting the gear too quickly may cause a delay in system response, resulting in the vehicle behaving as though still in Reverse. I consider this explanation unacceptable for a modern automatic vehicle and indicative of a potential defect. This condition presents a safety risk. Unintended rearward movement while in Drive could lead to collisions, especially in situations involving other vehicles, pedestrians, or objects behind the vehicle. Although the issue has not reoccurred since that day, the intermittent nature raises concern about reliability and the potential for recurrence without warning. I request that this issue be investigated as a potential safety defect.

2025 MAZDA CX-50- Woodbridge, NJ
After about six months of ownership, a knocking noise started happening from the front of the vehicle when turning the wheel all of the way to the right. A month or so later I brought it to a Mazda dealership in Green Brook, NJ. I was told by the service advisor that she had the same vehicle and was advised that this was normal. She explained everything exactly as I had experienced it. She told me I could wait in the dealership for 3/4 hours, but the technicians would most likely just tell me the same. Over time, the suspension deteriorated. Bumps felt more and more noticeable and a lot of noise inside the cabin. Specifically, a lot of vibrating on the right side of the vehicle: windows, front passenger seat, and the trunk window. Looking over the internet, it seems like this problem is apparent in multiple vehicle years, but never properly addressed via a recall: [XXX] . The vehicle is currently in Sansone Mazda in [XXX] . They replaced the right CV and the problem did not go away. I was told they would have to contact Mazda corporate. They did and I was then advised that they would need to replace the left CV axel, as well. Not surprisingly based on the frequency of this issue, that part was unavailable and on back order. 2021 MAZDA CX-5- Arlington, TX
The contact owns a 2021 Mazda CX-5. The contact stated that while driving at an undisclosed speed, the Forward Collision Avoidance Automatic Emergency Brakes activated unintendedly, and the vehicle jerked to the left and independently accelerated and moved back in the lane intended lane. The vehicle then jerked to the right, unintendedly accelerated, and then went back into the intended lane. Then the vehicle resumed normal operation. While driving approximately 60-65 MPH a week later, the vehicle started jerking. The “Smart City Brake Malfunction” message was displayed, and the engine warning light was illuminated. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 114,000.

2023 MAZDA CX-50- Burleson, TX
I am reporting a potential airbag system/emergency brake failure in my 2023 Mazda CX-50. I was involved in a rear-end motor vehicle accident at approximately 60 mph while my vehicle was completely stopped. I then hit the person in front of me. At the time of the impact, the airbags did not deploy, and I received a dashboard message indicating an airbag system malfunction. Since then, the emergency brake turns on and off on it's own at any time. Due to the severity of the crash, I am concerned that the airbags should have deployed, and their failure raises a serious safety concern. I sustained injuries (multiple fractures) as a result of the collision. I am requesting a formal investigation into possible defect or malfunction in the airbag system and emergency brake system.
