    I am reporting a safety defect in my 2023 Mazda CX-5 involving the right headlight assembly. The internal LED cooling fan has failed, causing loud noise and excessive vibration. This is a confirmed manufacturing defect (documented in Mazda Service Alert SA-013-23). Safety Risk: The cooling fan is essential to prevent the LED from overheating. A failure of this component can cause the headlight to dim or shut off completely without warning while driving at night, creating a severe visibility hazard and increasing the risk of a crash. Repair Details: An authorized Mazda dealer confirmed the internal failure and verified no external damage. Despite being a 2023 vehicle and a known safety issue, Mazda North American Operations has refused to cover the repair.

    Transmission failure when driving. Car was towed to dealership and diagnosed. Transmission and torque converter replacement required under factory warranty. Very long lead time for dealer to acquire replacement parts, about 4 months.

  • 2022 MAZDA CX-5

    I have a failed leveling motor on my led headlight assembly causing a constant noise as it is attempting to level. This is apparently a known problem and has had a TSA technical service bulletin of 09-015/23. My car is past the 3 yr warranty but under the 36,000 mile warranty period with only 33,000 miles. Mazda refuses to cover it since it has not had a recall yet even though it is a common and known problem. They are quoting a $1200+ repair cost. Can you please help in any way and address this? Thank you

    Engine has a crack in the head, well under 100,000 miles. It's a known defect but Mazda refuses to do anything about it. It is currently a very expensive paperweight sitting under our carport.

  • 2018 MAZDA CX-9

    My 2018 Mazda CX-9 experienced catastrophic engine damage after a radiator failure allowed coolant to enter the engine. The vehicle provided no dashboard warning, overheating alert, or coolant warning before the failure occurred. Modern vehicles are designed to alert drivers to overheating or cooling system failures. The lack of warning prevented me from addressing the issue and resulted in catastrophic engine damage. I believe this represents a potential safety defect in the vehicle’s cooling system monitoring and warning system.

  • 2025 MAZDA CX-30

    The vehicle has major electrical issues. The ABS and all over driver safety options would shut off for days at a time. I had purchased this cx30 as new on January 02, 2026. It totally died and wouldn't start on February 03, 2026. It had to ve towed by mazda roadside assistance to Towne Mazda dealer in Orchard Park NY. It is now March 12, 2026 and the dealer has had my vehicle for over a month. They put a new wiring harness in it ans other components. Very frustrating.

  • 2024 MAZDA CX-90

    What failed and is it available? The factory-equipped Yokohama Geolandar tires (specifically the X-CV model, 265/55R19) have experienced extreme, premature wear. They are currently on the vehicle but are in a failed state. How was safety put at risk? As of March 11, 2026 (22,567 miles), tread depth has reached 1/32" and 2/32". This renders the vehicle legally unroadworthy in Pennsylvania and creates a severe safety risk for hydroplaning and increased braking distances. Has the problem been confirmed by a dealer/manufacturer? Yes. Dealer records from July 2025 (12,402 miles) show the tires were already at 4/32", meaning they were 70% worn after only 10 months of use. The dealer contacted the tire manufacturer, who denied a defect claim. Mazda North American Operations (Case #00423913 as filed with Mazda Customer Experience Center) also formally denied assistance on March 10, 2026. Were there symptoms or warning signs? No dashboard lights appeared. The issue was discovered during routine maintenance. Based on the extremely rapid and uniform wear documented so early in the vehicle's life (at 12k miles), I believe this is either a defect in the factory-supplied tire materials or a systemic issue with the vehicle’s factory alignment/suspension geometry. As the consumer, I am left with an unsafe vehicle that the manufacturer refuses to rectify.

  • 2025 MAZDA CX-30
    - El Centro, CA

    While driving on the highway our sunroof exploded with no impact. The glass blew up outwards causing a loud gunshot like sound. We had the sunroof closed (not in use) and there were no vehicles around us for any asphalt or rocks to cause impact.

  • 2024 MAZDA CX-90

    The contact owns a 2024 Mazda CX-90. The contact stated that while driving at various speeds and attempting to make a left or right turn, the steering wheel failed to return to center. There was an unknown warning light illuminated. The vehicle was taken to a local independent mechanic, where it was diagnosed with an electrical failure. The local dealer was contacted; however, the vehicle was not diagnosed or repaired due to the cost. The manufacturer was made aware of the failure. The failure mileage was approximately 40,000.

  • 2019 MAZDA CX-5

    My 2019 Mazda CX-5 with approximately 78,000 miles developed a sudden major engine oil leak. The Mazda dealership diagnosed the issue as a cracked cylinder head and quoted approximately $5,600 for repair. The vehicle was properly maintained and this failure occurred without warning. A cylinder head failure at this mileage appears premature and may represent a defect. The oil leak created a large puddle under the vehicle and could pose a potential safety hazard due to loss of engine oil.
