2025 MAZDA CX-90- McKinney, TX
The vehicle repeatedly displays warnings including “Acceleration Limited Due to Low Fuel and/or Battery Condition,” Hybrid System Malfunction, and Engine System warnings. These events result in reduced acceleration and loss of power while driving. The condition occurs despite normal fuel levels and routine driving conditions. The vehicle has been taken to the dealership multiple times. Each time, I am told no fault codes are present or the issue cannot be replicated. The warnings continue to recur and remain unresolved. This creates an unsafe driving condition due to unpredictable power limitation. This issue appears to be a known problem affecting Mazda CX-90 vehicles and may be related to hybrid system software or battery management. The vehicle was being driven on a city road (Lake Forrest and University dr in McKinney, TX). Turned right off of a shopping center exit toward Lake Forest Dr. The vehicle lost power all of a sudden and would not accelerate with traffic heading in my direction. The vehicle signaled to stop and place the gear into park. I was able to slowly change lanes and pull into the Chipotle parking lot at 5151 W. University Dr at 10:56 am. I called Mazda Roadside and they came to tow it to El Dorado Mazda dealership. This incident: Date: 01/10/26 Time: 10:56 am Temp: 45° / clear and sunny Previous Incident: 12/04/25 6 pm Parked - Idling at Prosper High School. I-stop and hybrid system malfunction warnings appeared Called El Dorado Mazda- advised it was not a safety issue and sometimes it just needed a software update or the battery was bad. They scheduled a service appt until 12/17/26 due to lack of loaner vehicles available. 12/08/25 / 12:05 pm Clear weather Inspected at El Dorado Mazda Could not be Duplicated. Battery OK Nothing came up on diagnostic machine. Cleared recall. Advised to return if it happened again. Reassured me it was safe to drive and I was not in danger. 12/16/25 Car started from park at home Approx 40-50° 9-10 am 1/10/26-

2020 MAZDA CX-30- MT VERNON, NY
My 2020 Mazda CX-30 has an intermittent no-start condition caused by a complete electrical failure. This occurs randomly, often overnight and after long or short trips. When it happens: The car does not respond to the key fob or ignition Dashboard warning lights are off Vehicle cannot be started, creating a safety risk if it occurs during travel or in an emergency The defect began during car's full warranty period and has persisted despite multiple service visits and battery replacement. The dealer has been unable to diagnose or correct the issue, and Mazda Corporate is aware of the defect under case Case Number 00398310. After about two years of these intermittement failures, and parasitic draws ruled out, I paid to have the Battery replaced, independently of Mazda and the battery failed yet again. Routine diagnostics have not resolved the defect Issue is intermittent and unpredictable This is a Safety Risk as the Car has failed to start when needed on numerous occasions, and has left me stranded. ALL of these Failures have occured without warning Mazda Corporate is aware and has requested dealership diagnostics. Dealership has not reproduced the problem. Issue remains unresolved as of 1/10/2026 Video documentation exists showing dealer inspection and inability to find failure source. I am submitting this complaint to NHTSA because the defect: Impairs the vehicle’s safe operation Has not been reliably repaired Represents a potential safety risk to the driver and public I request that this complaint be recorded for investigation and included in any potential recall or safety evaluation. 2019 MAZDA CX-5- Sierra Madre, CA
The vehicle has suffered a crack of the Cylinder Head Assembly which leads to engine oil leaking onto hot exhaust and fire hazard. This defect is officially documented by the manufacturer in Service Alert SA-031/21, which identifies a trend of aluminum heads cracking and leaking pressurized oil directly onto the hot exhaust manifold. Although Mazda has issued a 10-year/120,000-mile warranty extension (Program CSP11) for the exact same casting failure in its Turbo-equipped models, it has refused to provide similar "Goodwill" relief for my vehicle, despite it suffering from the identical engineering flaw. Mazda’s refusal to cover a verified latent manufacturing defect that poses a significant fire hazard is arbitrary and unfair. I am requesting that Mazda North American Operations provide full reimbursement for the $6,788.51 repair

UNKNOWN The infotainment system started malfunctioning on its own. Changes channels on radio, goes into settings and makes changes. Calls phone numbers etc. Does it more in the cold weather

2017 MAZDA MAZDA6- Palmdale, CA
The car seat involved is a Nuna RAVA convertible car seat that was sent to the manufacturer solely due to a safety recall. I declined the self-repair recall option and requested that Nuna perform the recall service, which they agreed to. During recall servicing, Nuna inspected the car seat and advised in writing that internal Styrofoam/EPS foam in the headrest was missing or broken and that the car seat is not recommended for continued use and is not safety tested in that condition. Despite determining that the seat is unsafe, Nuna returned the car seat to me without providing a replacement, credit, or other safety remedy. The manufacturer has stated that the damage is unrelated to the recall and has refused to replace the seat, offering only a discount toward purchasing a new car seat. As a result of the recall process, I am left without a usable car seat for my child. The car seat has been inspected by the manufacturer and remains available for inspection upon request. I am requesting assistance with recall compliance, as the recall has resulted in an unusable child safety product with no remedy provided.

2024 MAZDA CX-90- Midvale, UT
After having stopped at red light, we made a left turn. I immediately felt a jerking sensation as I turned the wheel both left and right. I moved to a parking lot as I feared loosing steering function altogether, and felt the same jerking while parked and turning my wheel right and left. As my home was only a couple of blocks away, I managed to get my vehilcle home. Again, I can only describe the steering issue as a jerking sensation, or notchy sensation, while turning the steering wheel right or left. Dealership duplicated issue and indicated they contacted Mazda and are awaiting a back-ordered part that is a steering gear (my simple understanding). This event occurred months after Mazda replaced recalled steering issue (6524A)

2016 MAZDA MAZDA3- Derry, NH
The contact owns a 2016 Mazda Mazda3. The contact stated that while driving and while starting the vehicle, the instrument cluster was inoperable. The contact also stated that the instrument cluster screen was peeling, and critical information for the vehicle was not displayed. Additionally, the infotainment system became intermittently inoperable due to the failure. The dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The contact expressed concerns that the inoperable instrument cluster created a hazardous and unsafe condition due to the complete loss of instrument panel functionality. The approximate failure mileage was 142,000.

2022 MAZDA CX-5- Bethlehem, PA
Brake rotors on all four wheels measured below minimum thickness specification at only 17,000 miles, requiring complete replacement of all rotors and pads. Rotors replaced at Koch Mazda (Easton, PA) on January 7, 2026 and no longer available for inspection. SAFETY RISK: Rotors below minimum spec have reduced structural integrity and heat dissipation capacity, creating risk of warping, cracking, or failure during emergency braking or sustained braking. Under-spec rotors can cause brake fade, increased stopping distances, and potential catastrophic failure. At 17,000 miles, this represents premature failure of a critical safety system. DEALER CONFIRMATION: Koch Mazda initially charged $89.99 for diagnostics claiming "nothing wrong" despite customer providing TSB 04-003/24 documentation. After formal complaint, dealership measured rotors below minimum spec and replaced all four rotors and pads as "non-warranty goodwill repair" (value $1,184.97). Calling it "goodwill" rather than warranty avoids documenting as defect claim. Dealership stated Mazda corporate approved the goodwill repair, indicating manufacturer awareness of widespread defect being managed case-by-case rather than through recall. WARNING SYMPTOMS: Popping/clicking noise when first moving forward or reverse after sitting idle. This symptom is specifically described in Mazda TSB 04-003/24 as caused by deteriorated grease between brake pads and shims. SYSTEMIC PATTERN: Research reveals hundreds of similar complaints across 2013-2025 CX-5 model years involving premature rotor/pad failure at 11,000-35,000 miles. TSB 04-003/24 addresses this exact pattern across multiple model years, confirming manufacturer awareness. Normal rotor life is 50,000-70,000 miles; failure at 17,000 miles suggests manufacturing defect, improper heat treatment, or design flaw affecting entire CX-5 brake system across 12+ model years.

2018 MAZDA MAZDA6- North Charleston, SC
I recently discovered a significant oil leak near the exhaust manifold of my 2018 Mazda 6 (2.5L NA), which has resulted in visible smoke and a burning oil odor. Upon taking the vehicle to the nearest Mazda dealership for a diagnosis, the service department informed me that the vehicle has suffered a cracked cylinder head. An active oil drip is visible behind the engine near the exhaust manifold immediately upon starting the car; consequently, the engine is leaking oil directly onto the exhaust components. This condition not only renders the vehicle inoperable but also presents a significant safety and fire hazard. At the time of this writing, my vehicle has only 46,996 miles and has been meticulously maintained. This mileage is well under the 60,000-mile powertrain threshold and represents a premature failure of a major internal engine component. There are numerous reports on Mazda owner forums describing this identical issue occurring in Mazda 6, CX-5, and CX-9 models. Some owners have even reported instances of minor fires or active burning under the hood due to oil contacting hot engine components. Mazda released Technical Service Bulletin TSB #MC-10241612-0001 (SA-037-23), which includes a redesigned cylinder head for 2018–2020 Mazda 6, 2019 CX-5, and 2016–2020 CX-9 models, demonstrating Mazda’s awareness of this design flaw. While the TSB currently only specifies the 2.5L Turbo engines and not the naturally aspirated variants, there is substantial evidence that this is a widespread defect across these model years, unrelated to owner misuse or vehicle mileage. Mazda should take full responsibility by issuing a formal recall or providing an extended warranty to cover naturally aspirated engines, thereby addressing this problem and preventing further safety risks.

2021 MAZDA CX-5- Palmer, AK
I was entering a traffic circle when i slid slightly as there was some ice on the roadway and came to rest against a curb, bumped lightly. There was no damage to the vehicle, but the entire driver side curtain airbags as well as the driver seat airbag deployed. The speed at which I was traveling was roughly 10 mph or less. My wheels never hit hard nor did they leave the pavement.
