2018 TESLA MODEL X- Brecksville, OH
I am reporting a serious FSD safety-related software issue. While FSD was engaged, my 2018 Model X 100D (HW3, software 2026.26.6.1, FSD 12.6.4) unexpectedly came to a complete stop in the middle of the street twice. Please do not treat this as a repair estimate. I am requesting that Tesla review the FSD telemetry/event logs for both incidents and escalate the issue to the FSD/Autopilot engineering team. The vehicle stopping in the middle of an active roadway creates a potential collision hazard. Please document this as an FSD safety complaint/software defect and confirm that the events have been submitted for engineering review. I am not authorizing any paid repair based on this complain. I had a major incident today XXX at 10:50,in-front of Home Depot at [XXX]. I called the car through summon option to come to me from parking lot and it moved forward and hit the curb and damaged my front bumper also i had numerous FSD safety issues in last week which i reported to Tesla but they are unresolved. 2018 TESLA MODEL X- Riverside, CA
The contact owns a 2018 Tesla Model X. The contact stated that while attempting to park the vehicle, the vehicle became stuck in drive(D) and failed to shift to park(P). Due to the failure, the safety system locked the doors, and the contact was trapped inside the vehicle. The front end of the vehicle struck the garage door, causing damage to the garage door. The vehicle then rolled backwards and came to a complete stop after depressing the brake pedal. No air bags had deployed during the failure. The message “Self-Driving Feature Disabled” was displayed. A Police report was filed. No injuries were reported. The Fire Department arrived at the scene and had to open the frunk to cut the wires and disconnect the battery to turn off the vehicle, and the contact safely stepped out of the vehicle. The dealer was notified of the failure. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 72,929.

2018 TESLA MODEL X- Ypsilanti, MI
My vehicle intermittently and without warning disables Autopilot/cruise control while driving, most consistently during hard acceleration from a stop or low speed. The touchscreen displays "Take Over Immediately" and Autopilot becomes completely unavailable for the remainder of the drive. The system only resets after the vehicle is parked and allowed to sit. This has occurred repeatedly over multiple weeks, documented via the vehicle's own Service Mode diagnostic logs, showing the following recurring fault codes occurring together, dozens of times between approximatelyXXX and XXX: PM_w041_dasMIA – "Pedal Monitor Warning: DAS CAN node is missing" DI_w124_cruiseFault – "Drive Inverter Fault: Cruise faulted," user-facing message "Cruise control disabled" I brought the vehicle to a Tesla Service Center. Technicians stated they could not identify a specific cause, but acknowledged they were aware of similar cases from other owners of this vehicle model reported on Tesla's internal systems. No repair or resolution was offered. This vehicle is equipped with an HW3 (FSD Computer 3) Autopilot computer and Full Self-Driving Capability (purchased package). Tesla CEO [XXX] publicly confirmed on the company's Q1 2026 earnings call (XXX) that HW3 computers do not have sufficient capability for current and future Full Self-Driving software, and announced a remediation plan for affected owners. I have not been offered any remedy. I am concerned this represents a safety-relevant defect: the sudden, unpredictable loss of Autopilot/cruise control during active acceleration, particularly on-ramps or intersections, could create a hazardous situation for the driver and surrounding traffic. While driving on FSD (Full Self Driving) the vehicle was turning off the road into a parking lot and sped up and hit a vehicle while I was pressing the brake to the floor! I had to brace for impact when I hit the vehicle. I have requested the data and telemetry logs from Tesla. Everything was fine before the crash. Husband had a similar incident a few weeks ago where he was turning with FSD engaged and the vehicle stopped in the middle of the turn with a vehicle coming right at him, he had the gas floored to get out of the way but was completely stopped, could not move for 15 seconds or so.

The falcon doors on the model X, when it is a hot day, and the sun is hitting the door, these doors will not open under any circumstance, resulting in a major safety issue. should there be an accident, a fire or other issue where someone needs to exit the vehicle, they are simply not able to do so under any circumstance! This is a known problem with these vehicles, if the sun hits these doors or it’s a hot day. There are hundreds, if not thousands of posts about these doors, not opening when the sun is hitting the door for a prolonged period of time or the door gets hot. There needs to be a recall for this issue. From my research, seems that there’s some sort of sensor adhesive that comes loose or something similar to that. And either case this is a major safety issue, as it does not allow the passenger to exit the vehicle under any circumstance. Even holding the override button on the screen does not open the door. My kids have been stuck in this car on a repeated basis due to this issue of the door, not opening due to this failed sensor. Tesla is aware of this issue, and has never issued a recall, even though they know it affects all of the model X models

The Automated Emergency Brakes did not activate. This car hit the next car on the street. I was driving when I noticed the brake lights on the car in front of me, so I applied the brakes. Though it did not work, as this car is equipped with the Automated Emergency Brakes feature, it was expected that the car would activate the feature to stop to avoid impact and an accident. Which did not happen, and this car has a tremendous loss; the car in front also suffered a loss. The car manufacturer has given data in Excel files showing the events; it has no data at the time of the impact, but it has data well before and well after. Due to the failure of this important feature, the car has suffered a loss (potentially a total loss), the insurance process has been affected, and there has been a mental impact on my family members and me. Hence, requesting help in this regard.

The falcon doors will not open under any circumstance if the car is sitting in the sun. This is a known issue by Tesla with thousands of online complaints by owners. It seems the glue that holds the sensor comes loose in the heat. This is a major safety issue by NHTSA’s own standards. Not being able to exit the vehicle is directly listed as a reason for a recall by NHTSA. You need to force Tesla to issue a recall for this known issue by Tesla

2018 TESLA MODEL X- Charlestown, MA
For the past year, the battery will not hold it charge. I am getting about 30% of predicted mile range in colder weather and about 50% of predicted range in warmer weather. This makes the car unusable except for short trips and even then requires frequent charging. In addition, intermittantly, it will not charge quickly at a Supercharger. When others cars present are charging 180 range mph, I am charging at 20-50 range mph. I have had Tesla evaluate the issue and they claim there is nothing wrong but refused to share their evaluation with me. The rep would not give me any specific reason for my problem other than they found nothing wrong. I started the process before the end of the battery warranty (8 years) which has now expired. I feel Tesla service is committing systemic fraud in their approach to warrantied battery failure.

I own a 2018 Tesla Model X that has multiple breakdowns ( 3 times to date: Feb.28, 2026, Mar.7, 2026, Mar.14, 2026) resulting in the vehicle becoming undriveable and could not be shifted out of Neutral. During driving on separate occasions, the vehicle displayed multiple safety warnings and critical systems became disabled. Alerts shown on vehicle included: 1. Automatic Emergency Braking unavailable 2. Power braking Assist reduced 3. Parking brake not applied 4. Safety restraint system issue 5. ABS disabled 6. Park Assist unavailable 7. Service required Following these alerts, the vehicle became undriveable and could not be shifted properly, remaining stuck in Neutral. The vehicle therefore could not be driven safely. The vehicle was previously taken to a Tesla service center after an earlier breakdown. Tesla reported cleaning corrosion during service, but shortly after the vehicle was returned, the problem occurred again with the same or similar safety warnings. This issue is concerning because multiple braking and safety appear to fail simultaneously, including ABS, braking assist, and restraint system alerts. The vehicle becomes undriveable and may create a dangerous situation. Given the repeated breakdowns and the disabling of critical safety systems, I am reporting this issue for investigation.

2018 TESLA MODEL X- Atlanta, GA
The 2018 Model X experienced a sudden and complete loss of power and became totally inaccessible. All doors and access points were inoperable and the vehicle could not be unlocked, disconnected, powered on, or placed into any service or tow mode. Emergency front-trunk release cables had to be used and external 12-volt power applied at the jump terminals to wake the vehicle. The inability to access the vehicle was unexpected and alarming. Children, pets, elderly, and others could have been inside of the vehicle. Thankfully, they were not. Tesla inspected the vehicle and stated the failure was related to the battery coolant heater. The failure resulted in a complete vehicle immobilization and access failure. I had previously experienced restart warnings on the dashboard, but I did not expect a total lockout and loss of access. The inability to access the vehicle was unexpected and alarming. It may be a known safety issue. It should be recalled. This condition presents a serious safety risk because occupants could be trapped inside the vehicle and the vehicle cannot be accessed or moved without external power. In extreme heat or cold, or in a medical or emergency situation involving a child or pet, this failure mode could create a life-threatening situation. Importantly, even after multiple requests, I have not received a copy of the diagnostic report, or anything other than informal emails or calls. Tesla has not provided information regarding this failure other than broad statements, and visuals of the location of the battery pack, in an attempt to deny warranty coverage. The Tesla reports a High Voltage System alert currently. The vehicle is subject to Tesla’s Battery and Drive Unit Limited Warranty. Tesla has stated that the failed component was the battery coolant heater, but has refused to cover the repair under warranty. Tesla confirmed the failure but has not provided the diagnostic report, fault codes, or vehicle logs supporting its determination.
