  • 2023 CHEVROLET TAHOE
    - FLAGLER BEACH , FL

    A couple weeks ago, the truck suddenly started running rough and the front collision avoidance system failure message displayed. Immediately took it to the dealership. Computer read out showed #7 fuel injector failed. Repaired and was fine. Yesterday, 8/27/26, same thing happened again. Immediately went to the dealership. Now #4 fuel injector failed. Service manager just called and states that Chevrolet does not have that injector and on backorder with no date of expected delivery. No other Chevy dealer has #4 fuel injectors either. Tells me that there is an overwhelming demand for fuel injectors that indicates a defect. Meanwhile, the truck is out of service and GM is not addressing the issue to replace all the defective injectors.

  • 2022 CHEVROLET EQUINOX
    - Charleston, WV

    I was driving in town at normal speed getting on entrance ramp to XXX in [XXX]. As I made my turn the car engine stopped working. I pulled over to the side of the ramp. The car would not start. I called a tow truck and had it taken to my mechanic. He diagnosed it as a failed fuel pump module. He also said that this car had been recalled in 2023 and the fuel pump module replaced.   • 2027 CHEVROLET BOLT EV
    - Sacramento, CA

    The brake pedal can easily become disconnected from the electronic brake booster behind the firewall. When this happens, it stops all driver aids. The safety issue is it can happen while driving, and if using adaptive cruise control, or GM super cruise, if one pedal driving is on, it can without warning substantially slow the car, and has the potential to cause an accident. Its not too far to think the dislodging can cause the boost arm to be placed in a position to prevent full brakeing either. I have not taken it in because it is a design flaw of the system I believe. From searching online, I found the assembly, and it lacks a locking pin in the pedal to the shaft as you can see in the service diagram attached. Speaking to someone at a GM dealer they verified the issue on another 2027 Chevy Bolt, but not reproducible on other GM vehicles with the same electronic brake boosting system.

  • 2026 CHEVROLET SILVERADO 1500

    The contact owns a 2026 Chevrolet Silverado 1500. The contact stated that while driving at an undisclosed speed, all warning lights illuminated and the vehicle stalled. The contact stated that after several seconds, the vehicle was restarted and shifted into park(P). The vehicle was taken to a dealer to be diagnosed; however, the cause of the failure could not be determined. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was approximately 300.

  • 2019 CHEVROLET TAHOE

    When driving, the vehicle will randomly pull to one side cause the vehicle to severely swerve and almost cause a collision.

  • 2020 CHEVROLET SILVERADO 1500
    - Port Charlotte, FL

    The contact owns a 2020 Chevrolet Silverado 1500. The contact stated that the infotainment center intermittently went black, and there was water leaking into the vehicle through the top and lower center of the front windshield and the rear windshield. The vehicle was taken to a dealer, where the rear windshield was replaced without a diagnostic being performed; however, the failures persisted. Additionally, the side view mirrors intermittently failed to unfold when starting the vehicle. The contact stated that while driving approximately 40 MPH, white smoke emitted from the exhaust, the vehicle hesitated, and it lost power. The Check Engine warning light illuminated, along with "Service Electronic Stability Control", "Trailer Brake System", "Steering Assist Reduced", "Service Brake Assist", and "Brake System Failure" messages, and the vehicle failed to exceed 40 MPH. The vehicle was towed to a second dealer, Arcadia Chevrolet Buick (210 S Brevard Ave, Arcadia, FL 34266), where it was diagnosed that the lifter and torque converter had failed. The lifter and torque converter were replaced, and the system was flushed. Approximately 800 miles later, the engine failed again. The vehicle was towed to an independent mechanic, who diagnosed that the torque converter and transmission had fractured. The torque converter and transmission were replaced; however, the warning lights and messages illuminated again, and the vehicle continued to hesitate when driven over 42 MPH. After being parked for a few days, the warning lights were no longer illuminated and the mirrors opened, but the engine hesitation persisted. The vehicle was not further diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 10,000.

  • 2025 CHEVROLET SILVERADO 1500
    - Ponte Vedra, FL

    Total engine failure . While driving on the highway car started shaking violently and rattling. Car completely shut down was towed to Chevy dealership and was told it needed a full engine replacement. They said there was a hole in the engine the size of a tennis ball and said it was not user error but a defect . They also indicated we were lucky it didn’t blow up. Car is still being repaired . Being replaced with a gm remanufactured engine

  • 2011 CHEVROLET EQUINOX

    The contact owns a 2011 Chevrolet Equinox. The contact stated that while the mother was driving, the vehicle suddenly stalled. The vehicle was towed to the residence and was later diagnosed by an independent mechanic, who discovered metal shavings in the engine oil pan and informed the contact that the engine had seized and needed to be replaced. The contact stated that prior to the engine failure, the vehicle experienced excessive oil consumption over a 14-month period. The local dealer was notified of the failure, but no assistance was provided. The manufacturer was not notified of the failure. The failure mileage was 117,000.

  • 2026 CHEVROLET SILVERADO 1500
    - Charleston, SC

    On XXX, my Chevrolet Silverado experienced a sudden, severe loss of normal braking shortly after I dropped my child off at school. When I attempted to stop at a stop sign, the vehicle would not stop normally. The brake pedal remained firm, but braking effectiveness was drastically reduced and required extreme force. The dashboard simultaneously displayed multiple warnings involving the brake/ABS system, ESC, steering-column lock, and trailer-brake system. OnStar generated a diagnostic/safety report identifying fault/error code P062F. The vehicle then entered a reduced-speed/limp-mode condition. I was extremely fortunate this occurred within approximately one mile of Rick Hendrick Chevrolet of [XXX] and with little traffic around me. I was able to reach the dealership without a collision. Had this occurred at highway speed, the inability to stop normally could have resulted in a fatal crash. The incident also occurred immediately after school drop-off. Had I used the normal car line instead of parking and walking my child inside, this failure could have occurred among stopped vehicles while children were actively exiting them. I immediately brought the vehicle to Rick Hendrick Chevrolet. Despite the seriousness of the event, I do not believe the cause was adequately investigated. To my knowledge, a computer/software update was performed and I was essentially told to “let them know if it happens again.” They also asked what i was towing - nothing.I questioned whether an electrical fault or short could have caused the multiple simultaneous failures, but I am not aware of that possibility being investigated. Because another occurrence could cause serious injury or death, I refused to continue driving the vehicle and traded it in. I have now learned that the same vehicle is being offered for sale again. I am reporting this to NHTSA because I am concerned the underlying safety defect remains unresolved and could represent an early instance of a broader defect.   • 2015 CHEVROLET SILVERADO 1500
    - North Haven, CT

    January of last year, I was driving, coming off the highway, my engine came on and the system send to shutdown immediatly. I was close enough to my machanic, upon inspection, the AFM System failed at only 76,622 miles. This is part of the building issues with this value lifter problem I wanted to document it with the NHTSA as you are actively opening case for this ongoing issue with GM. i am hoping that a class action law suit will hopfully gain traction. this was a know issue, i was outside of standard warrenty, I was forced to replace the engine out of my own pocket to avoid having a truck worth almost nothing. I will bring this up with GM corporate also, reciept uploaded. Millions of 2014-present trucks and SUVs, while federal investigations specifically target nearly 878,000 to 1 million newer vehicles (2019–2026). GM should be helad accountable
