I just purchased this vehicle on XXX and on XXX, with less than 300 miles on it, we were in a parking garage at the Galleria Mall in [XXX]. While driving slowly uphill in the garage looking for a parking space, the vehicle suddenly went into neutral and the engine shutoff. After verifying all was well, I restarted the truck and continued to drive. It happened two more times with the same situation, low speed going uphill. I have not taken it to a Chevy dealer yet for diagnostic as there were no lights like the check engine light and no codes presented. My onstar app reports that all is well and no issues, so I know it will not be something they can find when they try to pull the codes. I did a search on the internet for this issue and found others with the same problem, no known fix, and in one case the person was able to return the truck and they purchased a different truck from a different manufacturer. That gentleman said he did try to work with GM at the regional level with a service rep. He said something is causing the voltage to drop in lower gears, causing it to invoke a safety feature to stop the vehicle by going into neutral and shutting off the engine. On an incline, with no engine, the brakes become very hard to push and the vehicle could roll back. In a parking garage, that could mean rolling back into a person or another car. When it happens there are in internal codes that do not show up, but are wiped when you re-start. There are threads on Reddit and in Facebook groups that discuss this issue as well for further evidence. My purpose is to get this documented outside of GM, it seems like a niche problem, but one that could be dangerous in the right circumstances. When GM announced the recall involving the potential for catastrophic engine failure, I immediately contacted Bridgewater Chevrolet in NJ to schedule the required recall inspection. I was told the dealership could not perform the inspection because it had not yet received the required 'diagnostic tool' from GM. Once they received the equipment, they contacted me and on Oct 1, 2025, they completed the required recall inspection and informed me that my Tahoe had passed the recall inspection and that the vehicle was safe to continue driving. On XXX while driving over the Driscoll Bridge on the [XXX] with my daughter and our dog, the engine catastrophically failed without warning as I began to exit on exit [XXX] to [XXX] into heavy traffic. With no warning, the vehicle flashed that it was in neutral and then suddenly lost all power and stopped completely in a traffic lane, creating an extremely dangerous situation. After a few mins, I was able to get it started briefly, enough to coast along to a spot a few hundred feet away where there was a shoulder. From there the wheels locked and it would not move at all and windows would not open. It had to be dragged completely inoperable with locked wheels onto a flatbed three separate times. It was first dragged and flat bedded off the Parkway to a place we felt unsafe so we stayed locked in the car in July heat with no windows down, then to my mechanic and then to the dealership. I am unsure of the damage that may have caused. The dealership received the vehicle on XXX and within a day told me that it was indeed what my mechanic and they suspected - the catastrophic failure recall they had inspected and passed the vehicle for. As of today, XXX, I was told they just received an engine and can start a full replacement this week. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Fuel Pump Power Control Module failed. GM Part Number: 13554821 My vehicle immediately became immobilized in a lane of traffic. The engine would crank but not start. It appeared that no fuel was pumped. This was confirmed with engine code P149F. GM diagnostic code P129F indicates a Fuel Pump Driver Control Module (FPDCM) fuel pump speed signal fault… My vehicle with 51,000 miles was serviced on 5/6/2026 for PA inspections, oil change and transmission flush. The VIN has no open recalls. GM has recognized the failure rate of the Fuel Pump Power Control Module, GM Part Number: 13554821 on many 2021vehicles and issued 2 recalls: 21V739000 issued September 23, 2021 And N252516900 issued September 11, 2025 Recall N252516900 was issued as a “phased launch” and yet (per GM) the Reason For This Recall: Certain vehicles may have a condition which can cause the fuel pump power control module to fail. If this were to occur, fuel flow to the engine could be interrupted. And Consequence Summary: An engine stall increases the risk of a crash. The vehicle instantly can become immobilized regardless of its location, in the middle of an intersection or in a lane of traffic. Without the engine running, the Tahoe cannot be shifted out of Park and cannot be pulled, pushed or dragged without causing damage. It must be properly loaded on a flatbed and be hauled from the failure site. GM “phased launched” the September 2025 recall to handle parts shortage. GM realizes how CRITICAL this recall is because a vehicle can become totally immobilized instantly. The dealership says my VIN shows no open recalls and are resistant to making a request for authorization to apply this recall. I think the failure of the Fuel Pump Power Control Module needs to be escalated until recall indicates ALL vehicles involved. I am glad I wasn’t entering a busy intersection when my vehicle became fully immobilized. As it was, vehicles were unable to go around for the 45 minutes I waited for a tow truck.

My drivers side view mirror has been malfunctioning. Nearly ever time I get in my car the side mirror will randomly adjust to a different position, typically all the way up or all the way down. My safety is at risk as sometimes it adjusts and I forget, then when I go to merge on to a highway or even make an evasive driving maneuver the mirror is incorrectly positioned. While I try to adjust it before I get in the car, I should not have to do this every time. The vehicle has not been taken to a dealer yet as I am out of warranty and do not want to pay $250 for exploratory work. The vehicle has also not been inspected by any third party. There are no warning lamps and this change happened after a recent software update.
