2019 CHEVROLET SILVERADO 1500- Dripping Springs, TX
I am reporting a potential safety defect involving my 2019 Chevrolet Silverado 1500 Z71 Trail Boss (VIN [XXX]), currently at approximately 63,000 miles. My truck suddenly developed a rough idle, engine ticking, and a cylinder 3 misfire. The check engine, traction control, parking brake, and “Service ESC” warnings appeared. The transmission also began shifting extremely hard, at times feeling as though another vehicle had struck the truck from behind. The oil pressure gauge also stopped functioning. Due to these symptoms, I stopped driving the vehicle. A diagnostic scan produced P0303, P0449, P0498, P0499 and P0500. After replacing a failed cylinder 3 spark plug, the truck initially ran perfectly for approximately 20 minutes before the rough idle and warning lights returned. The vehicle was subsequently diagnosed by a Chevrolet dealership. The dealership determined that the engine suffered a collapsed lifter that damaged the camshaft and other internal engine components. The engine repair estimate is approximately $10,000–$12,000. The dealership has also reported that the transmission is slipping and believes the torque converter/transmission requires approximately another $6,300 in repairs. I am concerned that a lifter/DFM-related failure causing a misfire, rough running and potential loss of engine power could occur while a vehicle is traveling at highway speed and create a significant safety hazard. The severe transmission shifting and subsequent slipping are also concerning because unexpected vehicle behavior during acceleration or while in traffic could increase the risk of a crash. The truck has been regularly maintained and has relatively low mileage for its age. I am requesting that NHTSA review whether similar engine/lifter/DFM and related powertrain failures represent a broader safety defect and whether an investigation or recall is warranted. 2026 CHEVROLET SILVERADO 1500- Olathe, KS
I was making a sharp turn going about 6 miles an hour. The vehicle suddenly stalled and shifted to neutral. A message appeared on the dash stating, “Please restart vehicle.” The vehicle restarted without issue and no error codes were displayed.

2025 CHEVROLET TAHOE- Rochester, NY
Was driving to dinner with my wife and 2 kids and we heard a loud sound like a shot gun went off. After pulling into a parking lot we a hole in the moon roof and the glass push outward like it exploded.

2024 CHEVROLET MALIBU- Wauwatosa, WI
The Contact owns a 2024 Chevrolet Malibu. The contact stated that upon reversing their vehicle, the rear view camera display was blank. No warning lights were illuminated. The contact emphasized their concern for safety referencing NHTSA Campaign Number : 26V212000 (BACK OVER PREVENTION) as probable cause for the failure. The vehicle was taken to a local dealer where the contact was advised that the parts were not available for remedy of the recall. The vehicle was repaired. The manufacturer was notified of the failure. The failure mileage was 15,736.

2021 CHEVROLET MALIBU- pottstown, PA
I purchased this vehicle in April of 2026, 4 days after bringing it home it began displaying a “Shift To Park” message in the dash, essentially making it so the vehicle will not shut off even when the vehicle is in park. Took it to a shop and they diagnosed it as a common issue across the 2021 Malibus and stated the entire shifter assembly needed replaced. Have not had it replaced yet due to not having the funds to replace the entire assembly, but was informed eventually the problem will progress to a total loss of being able to turn the vehicle off resulting in the vehicle essentially dying and requiring a jump start where the issue will display on the dashboard again until the internal micro switch is replaced. I believe this issue should be brought to GMs attention and a recall should be issued, seeing as they are aware it’s a known issue in malibus. A chevy dealership specifically told me that replacing the entire assembly may likely not even repair the issue itself.

2024 CHEVROLET EQUINOX- Richmond, VT
The vehicle’s software, steering, and Advanced Driver Assistance Systems (ADAS) create severe, active driving hazards. First, the steering and traction programming cause violent, unprompted lane deviations. While driving 20 mph on a snowy dirt road, the system automatically engaged and violently pulled the vehicle directly into the lane of an oncoming snowplow. A similar erratic swerving event occurred at 25 mph on [XXX] during icy conditions. Most recently, the steering violently pulled the vehicle 6 feet to the left at 50 mph, despite the suspension and wheels being in perfect factory specification with summer tires mounted. As a professional heavy-vehicle driver, I had to manually fight the wheel to maintain the lane. Second, the Adaptive Cruise Control unpredictably fails to recognize stopped traffic on both interstates and town roads. It repeatedly fails to apply the brakes when a lead vehicle stops, requiring emergency manual braking to avoid rear-end collisions. The system also triggers severe phantom braking without warning. We are forced to manually disable traction control and parking assist upon every vehicle start just to avoid losing control of the vehicle. The dealership has evaluated the vehicle multiple times but has failed to resolve these core software and ADAS defects, despite months of ongoing negotiations with GM Corporate. 2023 CHEVROLET BOLT EUV- Round Lake, IL
When driving and turning, the steering wheel does not come back to center and the car keeps turning after releasing the steering wheel causing the car to veer off the road if not turned back manually. This is very unexpected and an unsuspecting driver might not realize this until too late. I am in the process of contacting the dealer I bought the car from. But, searching the internet it seems to be a know problem.

2022 CHEVROLET TRAILBLAZER- Phoenixville, PA
When I shut off the engine, it says “Shift to park” and the car will not turn off. To get it to turn off, I have to turn the car back on (usually multiple times), until it recognizes that it IS in park.

2011 CHEVROLET EXPRESS- WARREN, OH
Vehicle broke down with a crank no start condition. Verified no fuel pressure. Diagnostics revealed no continuity between tank fuel pump positive lead at fuel pump relay, and ground. Removed fuel tank, disconnected fuel pump wiring harness, and found burned pin at positive terminal of fuel pump and mating harness connector. Harness displayed significant distortion and charring consistent with a poor connection.

2016 CHEVROLET IMPALA- Magnolia, AR
The contact owned a 2016 Chevrolet Impala. The contact stated that while driving approximately 35 MPH and turning into a parking lot, the vehicle lost power and there was black smoke coming from the right side of the engine compartment. The contact then parked the vehicle and opened the hood, and there were flames coming from the engine compartment. The Fire Department arrived on the scene and extinguished the fire. A Police report was not filed. The vehicle was towed to an impound lot. The manufacturer was notified of the failure. The failure mileage was approximately 200,000.
