Vehicle: 2014 Lexus RX 450h Component: ABS / Brake Actuator System I am submitting this complaint regarding a safety-related brake system defect in my 2014 Lexus RX 450h. The vehicle experienced ABS/brake actuator failure consistent with a known defect that was previously addressed by Lexus/Toyota through a Customer Support Program (extended coverage program, not a recall). I was never notified of this program. I have confirmed that Lexus mailed notification to an incorrect address (Temple City), despite the fact that my vehicle registration address has never changed and has always been accurately maintained with the DMV. As a result, I was deprived of the opportunity to repair a known safety-related defect within the coverage period. Key facts: - Brake/ABS actuator system failure occurred at approximately 150,000 miles - Lexus had previously issued a Customer Support Program for this known defect - No notification was received by me during the coverage period - Lexus confirmed notices were sent to an incorrect address - My DMV registration address has remained unchanged I raised this issue directly with Lexus Customer Assistance, including escalation to a supervisor. Lexus denied goodwill assistance and stated no remedy is available despite the documented notification issue. This raises concerns regarding: - Adequacy of manufacturer notification procedures for safety-related defects - Potential failure to reasonably notify owners using accurate registration data - Loss of repair opportunity for a known braking system defect due to administrative error Because this involves a braking system (critical safety component), I am requesting that NHTSA review whether the notification process used for this defect was adequate and whether other owners may have been similarly affected. Supporting documentation is available, including proof of DMV address history and evidence of incorrect mailing address used by the manufacturer.

Within approximately three weeks of purchasing this vehicle, the blind-spot monitoring and rear traffic sensor systems were found to be completely inoperative. All dashboard warning lights related to these systems were illuminated the night before discovery, then went dark the following morning. A subsequent inspection by a licensed Lexus dealership confirmed the failure was caused by prior undisclosed rear collision damage. Both rear blind-spot modules showed zero CAN bus communication. The following diagnostic trouble codes were confirmed: U0233, U023387, U0232, P057113, P157800, P1152A, P160400, C13807E, U0163, C142571, and B1576. The rear bumper had been improperly repaired using adhesive rather than factory fasteners. The rear lamp was barely installed and misaligned. A screw was installed in an incorrect location. The dealership noted that attempting to remove the bumper to visually inspect the blind-spot modules would cause additional damage due to the poor prior repair. The blind-spot monitoring system is a primary active safety system. Its complete failure due to concealed structural damage constitutes a material safety defect. The problem was confirmed in writing by a licensed Lexus dealer. Supporting dealer inspection documentation is uploaded.

The panoramic sunroof has a shade, which is not closing. This causes a lot of sunlight to enter the vehicle which might be harmful to your skin and causes headaches. The repair is very expensive and there is no other way around it. I seen many owners raising concerns about the same issue happening to them, on youtube, reddit, google and other websites. This is an issue on the same model from around 2017-2022. This must be repaired by lexus, and improved so it doesn’t keep happening.

The contact owns a 2013 Lexus ES300 Hybrid. The contact stated that while driving at an undisclosed speed, the vehicle experienced a sudden and significant reduction in braking efficiency. The contact stated that the brake pedal became extremely stiff and rigid, requiring excessive force to be depressed to stop the vehicle. Simultaneously, the Anti-lock Braking System (ABS) warning light illuminated. The dealer was notified of the failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 127,000.

Subject: Urgent Safety Concern – Driver Seatbelt Malfunction I am writing to formally report a serious safety concern regarding the driver-side seatbelt in my vehicle. The seatbelt does not retract properly and frequently becomes caught in the door. In addition, the retractor fails to pull the seatbelt back completely, leaving slack in the belt. More concerning, there have been multiple instances where the seatbelt did not properly engage or lock when braking was applied, even though it was securely fastened. I brought the vehicle to the dealership for inspection, but I was advised that this condition is “normal.” I strongly disagree with this assessment. A seatbelt that does not retract correctly or fails to lock during braking poses a significant safety risk and does not meet expected safety standards. This issue raises serious concerns about the reliability of the restraint system in the event of an accident. I am requesting a thorough inspection of the driver-side seatbelt system, including the retractor mechanism, and that any defective components be repaired or replaced immediately. Please document this concern formally and provide written confirmation of your findings and recommended corrective actions. I am very concerned for my safety and expect this issue to be handled with the urgency it deserves.

The panoramic sunroof shattered while the car was parked and there were no occupants. Lexus USA refuses to acknowledge this defect and the dealership insists there was an impact that caused this damage. There was no impact. I have asked the dealer for a written report of their determination of the point of impact and to return all broken glass to me. I do not know yet if they will comply with either request. As the car was parked, there were no immediate safety concerns, but due to the risk of additional glass falling onto the driver and passenger seats, the car was not in a drivable condition. Had this happened while driving, the sound of the shattering and the glass falling may well have resulted in a reaction from me causing immediate danger to myself and others on the road. UNKNOWN if problem has been reproduced, but this is not an isolated incident. Vehicle component had not been inspected by manufacturer, police , insurance reps or others. There was no warning or other symptoms of this problem.

The GX 550 has many people online complaining about the windshields cracking from the bottom for no reason. The crack is getting worse and close to line of sight. This is happening without something striking it.

My engine failed as I was driving down the highway at high speed. I was thankfully able to move over lanes and park safely on side of the rode before it came to a complete stop. It never turned back on. Lexus had to send a tow truck to pick it up and has been @ Dealership in Lubbock tx since 3/9/26

The issue started with the engine stalling while I’m driving, and one day the car failed to start. I towed it to the dealership who said there is nothing wrong with the fuel pump and advised me to replace the battery. I replaced the battery and drove it for a couple more months, while the engine continued to stall from time to time and eventually failed to start again yesterday. It’s very apparent that the issue is with the fuel pump, and there is a recall for the fuel pump for the 2015, 2016, 2019, and 2020 models of the Lexus GX460, and my car is the 2017 model. I believe the recall should have been issued for all the models from 2015 to 2020.
