I was hit by another car. Both cars were totaled. My seatbelt did not lock and my air bag did not deploy. I hit my head so hard against the steering wheel. I got a concussion and now I have daily headaches and my neck is killing me. The seat belt should have locked and the air bag should have deployed.

The dashboard is defective in that it melts when exposed to heat and becomes sticky. Although initially it started off as a cosmetic issue, the melting of the dashboard causes it to be very glossy to the point where it reflects onto the front windshield and creates a blind spot on the front windshield when driving. This poses an immediate safety hazard when driving as it's sometimes difficult to see objects that are right in front of the car while driving. The defective dashboard has been confirmed by the dealer and manufacturer. Although the manufacturer issued a customer support program to replace the dashboard, it was not a mandatory recall and the support program has since expired. Additionally, the support program initially described the malfunction as only a cosmetic issue. However, over time, this cosmetic issue has started to impact drivability in certain conditions while putting the driver and others at risk.

Motor Vehicle accident with damage to vehicle front end and drivers side quarter panel. Vehicle was not drivable and required towing service at the scene. Front seat belts of the vehicle failed to engage resulting in the driver being bounced up in the seat and then down resulting in a serious compression fracture of the vertebrae of the spine. An inspection was completed of the vehicle by a third party and reported to the Toyota Claims department on August 19, 2022. Toyota did respond with notification related to the results of the inspections to include: "The seatbelts were found to be operational. The front drivers' and passengers' seatbelt ELR (Emergency Locking Retractor) were tested and found to be operational. The EDR readout recorded that the drivers' and passengers' front seat belts were buckled at the time of the incident, and the seat belt pretensioners were not commanded to deploy. The owners manual advises that the pretensioners help the seat belts to quickly restrain the occupants by retracting the seat belts when the vehicle is subjected to certain types of severe frontal or side collision or a vehicle rollover." The owners manual related to seat belts on page 28 also documents that "the pretensioners do not activate in the event of a minor frontal impact, a minor side impact or a rear impact." NTSB Safety Alert SA-005 July 2012 documents "What can you do to save lives and reduce injuries? Make sure seat belts are worn by everyone in your vehicle and talk to your state and local lawmakers and urge them to support NTSB recommendations for seatbelt use laws." Why encourage all to use seat belts if they do not engage in the event of an accident and the general public believes that the seat belt will engage in the event of an accident, again to help prevent not only death but serious injuries... Al Law related to seatbelts, section 32-5B-4; Requirement of front seat occupants of passenger car must be fastened about body when car is in motion.

Sunroof exploded while driving in middle lane of 5 lane interstate. Cars, semi trucks all around me, center section of sunroof glass blew up and back into a pickup truck directly behind me. Pieces of glass flew in all directions. Luckily the moonroof/sunroof and interior panel were closed. No one was hurt. Other than a loud POP! then 2-3 seconds later an explosive BOOM! no warning light or message came on. Took the car to a local windshield shop, then a local Lexus dealer, another repair shop next week - all within 10 mi radius from where I live. Problem could not be reproduced, no one will confirm how it could have happened. Filed this report after reading about other Lexus - other mfg - having same issue.

I have a 2022 Lexus NX. The driver's side mirror shakes excessively and consistently. It is a serious safety concern because at night there is blurry lights reflecting and makes seeing cars difficult. It is also a safety concern because during the day you cannot see the distance cars or objects are away from the vehicle as the mirror is constantly shaking. This puts myself and others at risk of collision. This also causes me to become carsick when driving. I took the car into the dealer, Lexus of Riverside, in Riverside, California two times since purchasing the vehicle for this to be fixed. They refused to fix it, acknowledged Lexus knows about the problem and said not enough people have complained for them to fix it. They are currently saying this is a normal characteristic of the vehicle but this simply cannot be. It is extremely unsafe. This issue first appeared when I purchased the vehicle and took delivery. This seems to be wide spread based on information I have seen on social media and online forums. The mirror shaking happens at all speeds.

On 5/8/2020, Northtown Lexus replaced or repaired the vacuum pump. Northtown Lexus required the vehicle to be returned for repair twice, following the initial repair on 5/8/2020. After three attempts, the vehicle's vacuum pump (per a factory recall) is not functioning properly. Northtown Lexus refuses to further diagnosis this defective vacuum pump and continued to decline any and all requests for repair. The vehicle operates abnormally ever since this factory recall was originally addressed by Northtown Lexus on 5/8/2020.

My 2008 Lexus ES350 has been repaired at Northtown Lexus since 10/18/2019. The dealer and manufacturer indicated the door lock actuators are defective as original equipment. The dealer replaced only two door locks and referred me to Lexus Customer Service. Since 10/18/2019, I have NOT been able to obtain any relief from Northtown Lexus or Lexus Customer Service for these defective, factory original, door locks.
