2022 LEXUS NX- Mount Prospect, IL
The contact owns a 2022 Lexus NX 350. The contact stated that upon starting the vehicle, the blind spot cameras failed to properly operate. The contact stated that the Mobile App was used to activate the blind spot monitoring system while driving manually. The dealer was notified of the failure; however, the vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 47,748.

2015 LEXUS RX- Broken Arrow, OK
I purchased this vehicle on January 5, 2026, from a family-owned dealership in Broken Arrow, OK. The vehicle was purchased “as-is” with a guarantee of sale and a Carfax report. After purchasing the vehicle, I drove it home and it remained parked until January 8, 2026. On Thursday, January 8, 2026, the vehicle experienced an unexpected engine stall while I was driving. At the same time, the slip indicator light and traction control appeared to engage. I contacted the dealership immediately to report the issue, and the vehicle was towed back to JT Wholesale Auto Sales on Friday, January 9, 2026. On Tuesday, January 13, 2026, I was informed by the dealership that the vehicle had undergone a complete inspection and that no mechanical issues were found. Relying on this representation, I retrieved the vehicle and drove it home. However, after retrieving the vehicle, I continued to have concerns regarding its condition. On Thursday, January 15, 2026, I took the vehicle to Stellar Auto Service located at 2209 N. Yellowood Ave., Broken Arrow, OK 74012 for an independent inspection. Stellar Auto Service specializes in Lexus and Toyota vehicles. During their inspection, they identified the following issues: • Engine timing chain cover leak • Worn drive belt Stellar Auto Service was informed of the slip indicator light activation and the engine stalling that I had experienced. However, at the time of their inspection, they were unable to identify or reproduce any issues related to the slip indicator light or the stalling concern that I reported. On Sunday, February 15, 2026, the slip indicator light illuminated again, and the vehicle stalled while in operation. I was forced to safely pull off the roadway. I turned the car off, waited, restarted the vehicle, and drove home. The same issue occurred again on Sunday, March 8 and 15th, 2026. While driving, the slip indicator light illuminated and the vehicle stalled. I pulled off the road turned the vehicle off, restarted it.

2025 LEXUS TX- Palm Bay, FL
While my 2025 Lexus TX was parked in my residential garage, the vehicle suddenly locked itself with my [XXX] and [XXX] children and my phone inside. The doors had been unlocked prior to placing them in the vehicle. Without warning, the vehicle locked all doors and windows. The key fob failed to unlock the vehicle electronically or manually. My husband and I attempted for over 30 minutes to gain access while the children remained trapped inside before calling the dealership for technical help. They instead transferred us to Lexus roadside assistance. Lexus roadside assistance advised calling [XXX] to break the windows or otherwise enter the vehicle forcefully. However, before we could call, my husband figured out how to get a narrow opening to access the vehicle through the trunk to enter the interior and lower the windows in order to remove the children safely. The vehicle was taken to the dealership for inspection. This malfunction created a serious occupant entrapment safety hazard involving infants. 2025 LEXUS RX- Sugar Land, TX
On November 11, 2025, while reversing inside a garage, the rearview camera image— which had initially displayed correctly on the vehicle’s screen—suddenly froze while the vehicle remained in reverse mode. At the same time, the Pre-Collision System (PCS) failed to engage the brakes while the vehicle was traveling at approximately 2–3 miles per hour. As a result, the vehicle struck the garage structure and sustained damage. Toyota investigated this incident (Case #XXX) but based its conclusion on the incorrect assumption that I had reported either a brake system malfunction or a complete absence of an image on the display (a dark or blank screen). Because of this, Toyota determined that it could not provide assistance regarding the resulting vehicle damage. This characterization is inaccurate. The issue was not a blank screen. The rearview camera image appeared normally and then froze while the vehicle was still in reverse. My vehicle is currently subject to Recall Campaign No. 25LA06, which addresses reliability issues involving critical safety-related electronic systems. The rear visibility system is a federally mandated safety feature under 49 CFR §571.111 (Rear Visibility Requirements). When the image freezes during active vehicle operation, the system no longer provides the required real-time visibility necessary for safe reversing. Given the existence of an active recall campaign aimed at addressing reliability issues within the vehicle's electronic safety systems, this malfunction should be considered potentially linked to a defect covered by Recall Campaign No. 25LA06. The failure of the rear-view system during vehicle operation directly contributed to the occurrence of the collision.

2017 LEXUS IS- Midland, TX
The vehicle is experiencing a failure of the dual-zone HVAC system where the passenger side consistently blows hot air while the driver side is set to cold. This indicates a failure of the HVAC blend door actuator (servo motor), which is a known issue for this model year. This failure poses a safety risk as it can prevent proper defrosting and defogging of the windshield during inclement weather, obstructing driver visibility. Also should note While driving, the vehicle becomes excessively loud and behaves as if it is stuck in the wrong gear, accompanied by a significant loss of engine power. An onboard diagnostic alert has indicated a malfunction in the electronic control of the engine throttle or automatic transmission. This 'limp mode' behavior creates a hazardous situation during highway driving or merging, as the vehicle cannot maintain speed or respond to throttle inputs, increasing the risk of a rear-end collision.

2020 LEXUS ES- Pebble Beach, CA
I was driving on a freeway. I had my adaptive cruise control turned on. The car in front of me slowed down. My adaptive cruise control did not slow me down so I applied the breaks. The breaks did not work. The breaks did not slow me down or stop me. My pre-collision system did not activate. I collided with the rear end of the car that was in front of me. I got displays on my car screen stating "Pre-Collision System Malfunction", "Breaking Power Low", and "Cruise Control Malfunction". The safety of ourselves and others definitely put the safety of ourselves and others at risk. We did not report this to the dealer. Our car was totaled. The vehicle components involved were not inspected to the best of our knowledge. The warning messages are described above. We do not know exactly when they appeared.

2023 LEXUS ES- Belmont, CA
The contact owns a 2023 Lexus ES 300h. The contact stated that while sitting idle with the engine powered off in a local parking lot, the vehicle suddenly began to roll back independently into the nearby underground garage and crashed into the rear end of another parked vehicle's front passenger-side bumper that was unoccupied. No warning lights illuminated. The air bags did not deploy. No injuries were sustained, and no medical attention was sought. A police report was not filed. The local dealer was not contacted. The vehicle was not diagnosed or repaired. In addition, the contact stated that a 3rd party inspector investigated the failure; however, the diagnosis was not available. The manufacturer was made aware of the failure. The failure mileage was approximately 32,186.

2020 LEXUS RX- Mission Viejo, CA
I own a 2020 Lexus RX 450h with approximately 129,803 miles. The check engine light came on and the vehicle stored diagnostic trouble code P0A2D, which indicates a failure of the drive motor/temperature sensor circuit that is integrated into the transmission/hybrid motor assembly. According to the Lexus dealer’s diagnosis, the temperature control sensor located on the transmission/hybrid drive unit has failed. Because Lexus designed this sensor as part of the transmission assembly, the dealer has stated the sensor cannot be replaced separately and that the only repair is to replace the entire transmission. Lexus is refusing to acknowledge this as a manufacturing defect and is requiring me, the consumer, to pay the full cost of a complete transmission replacement, estimated at approximately $9,040.03, solely due to the failure of this internal temperature sensor. This is a critical component used to monitor drive motor/transmission temperature. A malfunctioning sensor of this type can lead to improper system operation, possible overheating, loss of power, or unpredictable behavior of the hybrid drive system while the vehicle is in motion, which presents a safety risk to occupants and other road users. This failure occurred during normal use on public roads and not due to any accident, abuse, or modification. I am requesting that NHTSA investigate whether there is a design or manufacturing defect in the drive motor/transmission temperature sensor and its housing or circuitry on the 2020 Lexus RX 450h (and related Toyota/Lexus hybrid models) that forces owners to replace the entire transmission when this internal sensor fails. This appears to be an unreasonable safety-related defect and an unreasonable repair burden on consumers.

2009 LEXUS IS- Roseville, CA
I am submitting this complaint regarding a safety concern with my Lexus IS 250. The dashboard in my vehicle is melting and has become extremely sticky, shiny, and reflective. As the material deteriorates, it creates significant glare on the windshield when sunlight hits the dashboard. This glare interferes with my visibility while driving and creates a potentially dangerous situation for myself, my family, and other drivers on the road. This issue is not merely cosmetic. The deteriorating dashboard material directly affects safe vehicle operation by impairing the driver’s ability to clearly see through the windshield, especially during daylight conditions when sunlight reflects off the dashboard surface. In addition, the dashboard material appears to be chemically breaking down. The surface remains sticky and continues to degrade over time, raising concerns about possible chemical exposure inside the vehicle cabin for my family I contacted my local dealership and also reached out to **Lexus corporate customer service. I was informed that Lexus previously had a Customer Support Program addressing melting dashboards, but the program expired in September 2021. I was told notification letters were sent in March 2019, but I never received one. Had I received the notice, I would have addressed the issue because it directly affects driving safety. They said there was nothing else they can do. Given the ongoing safety risk, I respectfully request that the **National Highway Traffic Safety Administration review this defect and determine whether further investigation or action is warranted. 2016 LEXUS NX- Union, NJ
My 2016 Lexus NX200t with approximately 157,000 miles experienced sudden engine stalling, loss of acceleration, and the check engine light illuminated while driving. The loss of power created a dangerous situation because the vehicle was unable to accelerate normally in traffic. These symptoms are consistent with the defective low-pressure fuel pump recall affecting certain Toyota and Lexus vehicles that can cause engine stall and loss of power. However, my VIN is not currently included in the recall even though my vehicle is experiencing the same safety defect. I am requesting that this issue be investigated to determine whether additional vehicles, including my model year, should be included in the recall due to the risk of sudden engine stall while driving.
